Deprese (angl. major depressive disorder; MDD) je časté a chronické psychiatrické onemocnění. Způsobuje výrazné snížení kvality života, snižuje práceschopnost a tím způsobuje zátěž pro postižené pacienty, zdravotnictví, i celou společnost. Patofyziologické znaky deprese jsou často nejednoznačné, a proto je současná léčba deprese nedostatečná. Důležitou roli v patogenezi deprese hraje chronický stres způsobující dysregulaci osy hypotalamus-hypofýza-nadledviny (HPA osa) řídící uvolňování kortizolu, porucha rovnováhy neuropřenašečů a neuromodulátorů a nerovnováha mezi prozánětlivými a protizánětlivými cytokiny. Dalším neurobiologickým znakem deprese je narušení energetického metabolismu v mozku, za který jsou zodpovědné mitochondrie. Mitochondrie tvoří kromě ATP i volné kyslíkové radikály (angl. reactive oxygen species; ROS). ROS jsou při chronickém stresu v nadbytečném množství a narušují buněčnou energetiku. Proto se aktuálně zkoumají látky související s mitochondriální energetikou a antioxidačními účinky. Na mitochondriální metabolismus účinně působí zejména ketamin, dizocilpin a fencyklidin. Avšak klinické užití těchto látek komplikují jejich halucinogenní a neurotoxické účinky. Obnově mitochondriální funkce napomáhají antioxidanty a tím prokazují antidepresivní účinky. Při léčbě deprese by se... Depression (major depressive disorder; MDD) is a common and chronic psychiatric disorder. MDD causes a significant reduction in the quality of life, reduces the ability to work, and thus causes a burden on disabled patients, healthcare, and all of society. The pathophysiological features of depression are often ambiguous, and therefore current treatment for depression is insufficient. Chronic stress plays an important role in the pathogenesis of depression. It causes dysregulation of the hypothalamic-pituitary-adrenal axis (HPA axis) controlling cortisol release, imbalance of neurotransmitters and neuromodulators, and imbalance between pro- inflammatory and anti-inflammatory cytokines. Another neurobiological sign of depression is a disruption of energy metabolism in the brain, for which mitochondria are responsible. In addition to ATP, mitochondria also form reactive oxygen species (ROS). ROS are in excessive amounts during chronic stress and disrupt cellular energy. Therefore, substances related to mitochondrial energy and antioxidant effects are currently being investigated. Ketamine, dizocilpine and phencyclidine are particularly effective in mitochondrial metabolism.
