1 ABSTRAKT Přínos provedení mikrofraktur acetabula při artroskopické terapii chondrálních defektů u femoroacetabulárního impingement syndromu- dvouleté výsledky multicentrické prospektivní randomizované studie. Cíl studie: Prezentujeme dvouleté klinické výsledky prospektivní randomizované multicentrické studie u pacientů s artroskopicky ošetřeným femoroacetabulárním impingement syndromem a současným provedením mikrofraktur chondrálních lézí acetabula IV. stupně. Materiál a metodika: Zhodnotili jsme soubor 55 pacientů z původně 92 zařazených se základní diagnosou FAI syndromu s peroperačně prokázanou chondropatií acetabula IV. stupně velikosti do 4 cm2, kteří podstoupili dvěma zkušenými operatéry komplexní artroskopický výkon na kyčli (korekce strukturální cam a/nebo pincer deformity, refixace nebo částečné odstraněné labra, atd.). Pacienti byli peroperačně randomizováni obálkovou metodou do dvou skupin. Ve skupině 1 (31 pacientů) byly provedeny mikrofraktury defektu chrupavky a ve skupině 2 jen debridement defektu. Zhodnocenou skupinu celkem tvořilo 7 profesionálních a 48 rekreačních sportovců (33 mužů a 22 žen) s průměrným věkem 34,4 ve skupině 1 a 31,1 ve skupině 2. Předoperačně a dále pooperačně v 6., 12. a 24. měsíci jsme hodnotili parametry mHHS a VAS a dále jsme zaznamenávali revizní operace, konverzi... 1 ABSTRACT Benefits of the Acetabular Microfracture Technique in Arthroscopic Treatment of Chondral Defects in Femoroacetabular Impingement Syndrome - Two-year(s)? Results of a Multicenter Prospective Randomized Study. Purpose of the study Two-year clinical results of a multicenter prospective randomized study in patients with arthroscopically treated Femoroacetabular Impingement syndrome and concurrently performed microfracture for grade IV chondral lesions of the acetabulum. Material and methods The study evaluated a group of 55 patients of the originally enrolled 92 patients with the underlying diagnosis of FAI syndrome with intraoperatively confirmed grade IV acetabular chondropathy of up to 4 cm2 in size, who had undergone a comprehensive hip arthroscopy (correction of structural cam-type and/or pincer-type deformity, labral refixation or partial labral resection etc.) performed by two experienced surgeons. The patients were randomized intraoperatively using a closed envelope method into two groups. In group 1 (31 patients), microfractures for chondral defects was performed, while in group 2 the patients underwent a defect debridement procedure only.
