Úvod: Nejvýznamnějšími geny spojovanými s hereditárním karcinomem prsu a ovarií jsou BRCA1/2 a vliv na zvýšení rizika se předpokládá též u NBN. Cílem této práce je stanovit frekvenci rekurentních mutací BRCA1/2 v neselektovaném souboru pacientek s karcinomem prsu a frekvenci nejčastějších patogenních mutací NBN v ČR, zhodnotit efektivnost indikačních kritérií ke genetickému vyšetření a zvážit rozšíření spektra testovaných genů o NBN. Metody: K analýze rekurentních mutací 5382insC a 300T>G v BRCA1 bylo užito RFLP, exon 11 téhož genu byl analyzován pomocí PTT, vybraný úsek 11. exonu BRCA2 pomocí DHPLC a 6. exon NBN pomocí HRMA.Všechny zachycené mutace byly potvrzeny sekvenováním. Výsledky: V souboru 679 neselektovaných pacientek s karcinomem prsu bylo v BRCA1 zachyceno 7 mutací 5382insC, 3 mutace 300T>G a další 4 jiné mutace. V BRCA2 byly identifikovány dvě lokálně prevalentní mutace. U 730 kontrol byla zachycena pouze jedna mutace, a to 5382insC v BRCA1. Ve sledovaném úseku exonu 6 genu NBN bylo v souboru 600 vysokorizikových pacientů zachyceno celkem 5 mutací, z toho dvě 657del5 a jedna R215W; v souboru 703 neselektovaných pacientek s karcinomem prsu bylo identifikováno celkem 8 mutací, z toho dvě 657del5 a tři R215W; v souboru 915 kontrol pak bylo detekováno celkem 9 mutací, z toho dvě 657del5 a... Summary: Background: An increased risk for development of hereditary breast cancer is associated with germline mutations in BRCA1/2 and the influence of NBN mutations is also supposed. The aim of this study is to specify the frequency of recurrent mutations in BRCA1/2 in unselected breast cancer patients and the frequency of most common pathogenic mutations in NBN in Czech republic, to assess current criteria for genetic testing and to consider the addition of NBN to the tested genes. Methods: Screening for recurrent mutations 5382insC and 300T>G in BRCA1 was performed by RFLP, screening for mutations in exon 11 of BRCA1 was performed by PTT, screening for mutations in a selected region of exon 11 of BRCA2 by DHPLC, and screening for mutations in exon 6 of NBN by HRMA. All the mutations were confirmed by direct sequencing. Results: In 679 unselected breast cancer patients 7 carriers of 5382insC, 3 of 300T>G, and 4 of other mutations in BRCA1 were identified. 2 locally prevalent mutations were found in BRCA2. In 730 controls only one 5382insC BRCA1 mutation was identified. Out of 5 NBN mutations found in 600 high-risk patients two were 657del5 and one R215W.
