Alzheimerova nemoc (AN) je pomalu progredující neurodegenerativní onemocnění mozku charakterizované extracelulární akumulací senilních plak a intracelulárními neurofibrilárními tangles. Vedle těchto typických změn je AN velmi heterogenní onemocnění, a to zejména věkem nástupu, prvních klinických obtíží a rychlostí progrese. Klíčovými modulátory této variability jsou genetické polymorfismy. Jedním z nich je polymorfismus v genu pro Brain-derived neurotrofic factor (BDNF), neurotrofin podílející se na formování synaptické plasticity v oblasti hipokampu a mediálního temporálního laloku, tedy oblastí primárně postižených u AN. Cílem disertační práce bylo prozkoumat vliv polymorfismu BDNF Val66Met, společně s hlavním genetickým rizikovým faktorem sporadické AN, Apoliproteinem E (APOE), na kognitivní funkce a strukturální změny mozku. Prokázali jsme, že kombinace rizikových alel BDNF Met a APOE ɛ4 je spojena s výraznějším narušením epizodické paměti, egocentrické orientace v prostoru a menšími objemy struktur mediálního temporálního laloku u jedinců ve stádiu amnestické mírné kognitivní poruchy (aMCI), a allocentrické orientace u jedinců bez kognitivního deficitu. Tato zjištění naznačují, že nositelé kombinace BDNF Met a APOE ɛ4 mají vyšší riziko progrese do těžších stádií AN, a současně podporují význam... Alzheimer's disease (AD) is a slowly progressing neurodegenerative disease characterized by an extracellular accumulation of senile plaques and intracellular neurofibrillary tangles. Apart from these hallmarks, AD is very heterogeneous, especially at the age of onset and the rate of progression. Genetic polymorphisms are key modulators influencing these factors. One of them is a polymorphism for the Brain-derived neurotrophic factor (BDNF), a neurotrophin involved in the synaptic plasticity in the hippocampus and medial temporal lobe structures, areas primarily affected in AD. The thesis aimed to investigate the effect of BDNF Va66Met polymorphism together with the main genetic risk factor for sporadic AD, Apolipoprotein E (APOE) polymorphism, on cognitive functions and structural brain changes. We have shown that the combination of risk alleles BDNF Met and APOE ɛ4 is associated with more severe impairment in episodic memory, egocentric orientation and smaller volumes of medial temporal lobe structures in individuals with amnestic mild cognitive impairment (aMCI), and allocentric orientation in cognitively unimpaired individuals compared to non-carriers.
