Renální karcinom (RCC) je třetí nejčastější urologická malignita, u které je zhruba u 30% v době diagnózy zjištěn metastatický proces. Vzhledem k rezistenci na ozáření a chemorezistenci je prognóza těchto pacientů špatná. Optimální léčbou je proto kompletní odstranění nádoru lokalizovaného onemocnění. Ke zlepšení prognózy pacientů v posledních dekádách došlo díky zvýšení počtu náhodně zjištěných nádorů ledvin a též zavedením nových typů léčby. Vzhledem k obecnému trendu nárůstu náhodně zjištěných nádorů ledvin, jsme byli inspirování k zhodnocení pacientů operovaných pro nádor ledvin s ohledem na typ záchytu onemocnění a metodu primárního vyšetření. K vyšetřování cirkulujících nádorových buněk (CTC- z angl. Circulating tumor cells) nás vedl fakt, že pro sledování pacientů po operacích nádorů ledvin nemáme žádný biomarker odpovídající léčbě, případně rekurenci onemocnění. Vyšetření CTC patří do skupiny testů tekuté biopsie a jednou z jeho hlavních výhod je relativně minimální invazivita (odběr krve), která skutečně umožňuje sledování nádorové dynamiky. Primární otázkou bylo, zda CTC buňky mohou hrát nějakou roli při sledování pacientů po operační léčbě a event. při nastavení imunoterapeutické a antiangiogenní léčby díky molekulární CTC- analýze na úrovni genové exprese. Do epidemiologické studie z... Renal cell carcinoma (RCC) is the third most common urological malignancy, in which about 30% are diagnosed with a metastatic disease at the time of diagnosis. Due to radiation resistance and chemoresistance, the prognosis of these patients is poor. Therefore, the optimal treatment is complete removal of the tumor from at the stage of localized disease. The prognosis of patients has improved in recent decades due to an increase in the number of incidentally detected kidney tumors and also the introduction of new types of treatments. Given the general trend of increasing randomly diagnosed kidney tumors, we were inspired to evaluate patients operated for kidney cancer with respect to the type of disease detection and the method of primary examination. The fact that we do not have a biomarker corresponding to the treatment or recurrence of the disease for monitoring patients after kidney cancer surgery led us to the CTC investigation. CTC examination belongs to the group of liquid biopsy tests and one of its main advantages is the relatively minimal invasiveness (blood collection), which really allows monitoring of tumor dynamics.
