Nosiči balancovaných chromozomových aberací (BCA) jsou obvykle fenotypově normální. Avšak až 27 % de novo vzniklých BCA může být spojeno s patologií fenotypu způsobenou nejčastěji submikroskopickou imbalancí nebo genovou disrupcí v oblastech zlomových míst, případně vlivem pozičního efektu. Molekulárně cytogenetické techniky oproti karyotypování umožňují detailnější popsání aberace a tím usnadňují korelaci genotypu s fenotypem pacienta. Cílem práce bylo zhodnocení výskytu variant v počtu kopií (CNV) v místech zlomů aberace i v jiných oblastech genomu u pacientů s abnormálním fenotypem, kteří byli nosiči de novo i familiárních BCA. Metodou array CGH bylo vyšetřeno 54 případů BCA (20 de novo, 27 familiárních, 7 bez objasnění původu), z toho 32 reciprokých translokací, šest robertsonských translokací, 12 inverzí a čtyři komplexní přestavby. U klinicky významných nálezů, bylo-li to možné, byli vyšetřeni i rodiče pacienta pro objasnění původu CNV. Za účelem upřesnění nálezů či vyloučení genové disrupce byla u vybraných případů použita metoda FISH. U 31,5 % pacientů (17/54) byla zachycena alespoň jedna (z toho u osmi osob více než jedna) klinicky významná CNV. U čtyř případů byly zjištěny imbalance pouze v oblastech zlomových míst, u deseti pouze CNV jinde v genomu a u tří jedinců v oblastech zlomů i... Carriers of apparently balanced chromosomal aberrations (BCA) are usually phenotypically normal. However, it has been estimated that up to 27% of these BCA may be associated with an abnormal phenotype, most often caused by cryptic imbalances at the breakpoints, gene disruption by the breakpoint or via the position effect. In contrast to conventional karyotyping, molecular cytogenetic techniques enable more detailed BCA characterization and better correlation between genotype and phenotype of the patient. The aim of this thesis was to evaluate the presence of copy number variants (CNVs) at breakpoints or elsewhere in the genome in patients with abnormal phenotype who carry de novo or inherited BCA. 54 BCA were investigated using array CGH (20 de novo cases, 27 inherited and 7 cases of unknown origin) including 32 reciprocal translocations, 6 robertsonian translocations, 12 inversions and 4 complex chromosomal rearrangements. If possible, the parents were also examined to ascertain the inheritance of the relevant CNVs. In order to specify microarray findings or exclude gene disruption, FISH was used in selected patients.
