[cze][eng] Úvod: Pacienti s polytraumatem mohou být vyšetření dvoufázově nebo jednofázově s jednofázovým či vícefázovým bolusem kontrastní látky (KL). Jednofázový bolus KL není ke kvalitnímu nasycení parenchymových orgánů a náplni cév dostačující. Moje disertační práce zkoumá možnosti podání KL technikou split-bolus. Cíl práce: Prokázat diagnostickou kvalitu protokolů užívající split-bolus techniku podání KL. Metodika: Pacienti byli rozděleni celkem do osmi skupin, jmenovitě A-H. Po nativním vyšetření hlavy a krku podstoupili všichni pacienti postkontrastní vyšetření v rozsahu od báze lební pod symfýzu. Pacienti skupiny A podstoupili dvoufázové vyšetření, pacienti skupiny B jednofázové vyšetření po jednofázové aplikaci KL a pacienti ve skupinách C-H podstoupili jednofázové vyšetření s technikou podání KL split-bolus. Pacientům byl podaný stejný objem kontrastní látky, a to 100 ml, o stejné koncentraci 350 mgI/ml. Hodnotil se rozdíl v náplni cév v oblasti vnitřních karotických tepen, oblouku aorty, v břišní aortě a pánevních tepnách a dále se hodnotila denzita jater, v. portae, sleziny a ledvin. Ke zhodnocení byla zvolena neparametrická statistická metoda Krusakalova-Wallisova ANOVA, nenormální rozdělení bylo zjištěno u většiny zkoumaných proměnných pomocí Saphiro-Wilkova testu. Výsledky: Byly nalezeny...Objective: The possibilities of CT examinations in patients with multiple trauma are: two- phase, or single-phase CT examination. However, a single-phase contrast medium (CM) administration doesn't suffice quality saturation of arteries along with parenchymal organs. My dissertation is focused on a split-bolus contrast medium administration technique. Aim: To prove diagnostic quality of protocols using split-bolus technique. Materials and Methods: Patients were divided into eight groups, namely A to H. After the non-contrast examination of the head and the cervical spine, all patients were examined after CM administration from the skull base to the symphysis. Group A patients underwent a biphasic examination after CM single-bolus, group B patients underwent single-phase examination after a single bolus of CM, and group C-H patients underwent a single-phase examination after split- bolus technique of CM administration. Patients were given 100 ml of CM at the same concentration of 350 mgI/ml. The densities variables in internal carotid arteries, aorta arch, abdominal aorta, common pelvis arteries, liver, v. portae, spleen and kidney were evaluated.
