Úvod: Spontánní potrat (SA) je nejčastější komplikací v těhotenství. Cílem práce bylo prověřit kauzalitu vybraných faktorů genetických - apolipoproteinu E (APO E), mutace faktoru V Leiden (FVL), mutace protrombinu (PT G20210A) a faktorů negenetických - tyreoidálního stimulačního hormonu (TSH), volného tyroxinu (fT4), protilátek proti tyreoidální peroxidáze (a-TPO) v etiopatogenezi časného SA. Materiály a metody: Ke genotypizaci polymorfismů APO E byla využita metoda PCR-RFLP. K detekci mutací v genech FV a FII byla použita metoda HRM. Laboratorní tyreoidální markery TSH, a- TPO a fT4 byly stanoveny na automatickém analyzátoru využívajícím chemiluminiscenční imunoanalýzu. U infertilních žen po SA proběhlo do 8 týdnů od krevního odběru sonografické vyšetření štítné žlázy. Výsledky: APOE genotypy ve sledovaném souboru 410 vzorků embryonální /fetální tkáně se významně neliší od kontrolní skupiny 2 606 adultních jedinců (P = 0.653). Ve sledovaném souboru 75 žen s izolovaným SA byla FVL mutace zjištěna v heterozygotní konstituci s prevalencí 12 %. Prevalence FVL v souboru žen s izolovaným časným SA byla signifikantně vyšší oproti kontrolní skupině 76 zdravých žen (12 % vs. 2.6 %, P = 0,031). Při vzájemném statistickém porovnání zjištěných prevalencí PT G20210A mezi soubory žen s izolovaným SA a kontrolní... Introduction: Spontaneous abortion (SA) is the most common complication in pregnancy. The aim of the study was to investigate the causality of selected genetic factors - Apolipoprotein E (Apo E) gene polymorphisms, factor V Leiden (FVL), Prothrombin (PT G20210A) and nongenetics factors - Thyroid stimulating hormone (TSH), free thyroxine (fT4), antibodies against thyroid peroxidase (a-TPO) in the role of early SA. Materials and methods: For genotyping of APO E polymorphism was used PCR-RFLP. The detection of mutations in genes FV and FII was performed using by HRM. Laboratory markers of thyroid (TSH, a-TPO and fT4) were determined by an automated analyzer using chemiluminescent immunoassay. Results: APOE genotypes of investigated group of 410 samples abortioned embryonic/ fetal tissues were not significantly different from 2 606 adult controls (P = 0.653). In observed infertile group of 75 women with isolated SA was FVL detected in heterozygous constitution with a prevalence of 12 %. The prevalence of FVL in a group of women with early insulated SA was significantly higher than 76 controls (12 % vs. 2.6 %, P = 0.031).
