Psycholog a bariatrická léčba obezity PhDr.Jitka Herlesová Školitelka: PhDr.Tamara Hrachovinová, CSc Abstrakt Cílem disertační práce v její teoretické i empirické části je zmapování psychických, zdravotních a sociálních aspektů spojených s obezitou a jejich změny po bariatrické operaci. Teoretická část se zaměřuje na východiska, na která navazuje druhá část práce. Obezita je onemocnění, jehož zastoupení v populaci stoupá. Bariatrická a metabolická chirurgie je jednou z nejúčinnějších metod léčby obezity a jejích komorbidit. Zejména s vysokým stupněm obezity souvisí psychické poruchy. V rámci bariatrické léčby je standardní součástí přípravy k zákroku psychologické vyšetření. Po operaci dochází s redukcí váhy ke zlepšení až normalizaci psychosociálního fungování a slábnutí příznaků psychických poruch, v některých případech může ale dojít ke zhoršení psychického stavu. Empirická část se soustřeďuje na charakteristiky kandidátů bariatrické operace získané při psychologickém vyšetření a na změny po zákroku. Vyhodnoceny před operací byly údaje z polostrukturovaného rozhovoru, Beckovy sebeposuzovací škály depresivity, Dotazníku jídelních zvyklostí a dotazníků specifické kvality života (OWLOQOL, WRSM). Po zákroku byly sledovány změny v 6, 12 a 24 měsících. Ke statisticky významným změnám ve všech zkoumaných... Abstract The aim of the dissertation thesis is to map psychosocial characteristics associated with obesity and their changes after bariatric surgery. The Empirical part follows the points settled in the Theoretical part. Obesity is a disease, whose prevalence rises. Bariatric and metabolic surgery is one of the most effective treatment of obesity and its comorbidities. The mental diseases are associated with high levels of obesity. The standard part of pre-bariatric examination is the psychological assessment. Several changes accompany the weight loss after bariatric surgery, such as normalization of psychosocial functioning, the decrease of mental disorders but in some cases the changes might be also negative. The Empirical part focuses on the psychosocial characteristics of bariatric surgery candidates assessed during the psychological evaluation and the changes after operation. The data from semi-structured psychological interview, Three factor eating questionnaire, Beck Depression Inventory-II, Obesity Weight Loss Quality of Life and Weight Related Symptom Measure were evaluated.
