Souhrn Laboratorní vyšetření funkce ledvin Ve své práci jsem se věnoval především problematice glomerulární filtrace (GFR). GFR je hlavní ukazatel funkce ledvin. Její pokles je asociován se zvýšenou morbiditou a mortalitou. Při poklesu GFR pod 1,0 ml/s/1,73m2 po dobu delší než 3 měsíce mluvíme o chronickém onemocnění ledvin (CKD). Pacientům s CKD je redukována dávka farmakoterapie s farmakokinetikou renálního vylučování a je podávána nefroprotektivní terapie. Proto je nutné včas zjistit tento pokles GFR. Práce porovnává odhady glomerulární filtrace ze sérového kreatininu (eGFRcreatinine) a cystatinu C (eGFRcystatinC) u čtyř různých kohort pacientů: 101 pacientů s monoklonální gamapatií neurčeného významu (MGUS), 13 intoxikovaných metanolem, 1515 diabetiků a 352 pacientů s CKD. Kreatinin byl u všech pacientů stanoven enzymaticky s návazností měření na mezinárodní certifikovaný referenční materiál NIST SRM 967. Cystatin C byl analyzován imunoturbidimetrickou standardizovanou metodou s návazností na DA ERM 471. Pouze u skupiny pacientů s MGUS, což byla první studovaná skupina, nebyla ještě standardizovaná metoda k dispozici. Pro odhad GFR byly použity nejaktuálnější validované rovnice. U pacientů s MGUS byla pro odhad eGFRcreatinine použita MDRD rovnice, pro eGFRcystatinC Grubbova rovnice. U pacientů... Laboratory diagnostics of kidney function The work deal with issue of glomerular filtration rate (GFR). GFR is the most important indicator of kidney function. Its decline is associated with increased morbidity and mortality. Chronic kidney disease (CKD) is defined as decrease of GFR bellow 1.0 ml/s/1,73m2 for more than three months. Patients with CKD receive nephroprotective drugs and have reduced drugs excreted by kidneys if needed. Early detection of CKD is of clinical importance. We compare GFR estimated from serum creatinine (eGFRcreatinine) and from serum cystatin C (eGFRcystatinC) in four cohorts of patients: 101 MGUS patients, 13 methanol intoxicated patients, 1515 diabetics and 352 patients with CKD. Creatinine was measured by enzymatic method traceable to international reference material NST SRM 967 in all patients. Cystatin C was determined by standardized immunoturbidimetric method traceable to DA ERM 471. This standardized method was not available only for MGUS patients. eGFR was always calculated according to the best available validated equation. MGUS patients were calculated according to MDRD equation for eGFRcreatinine and according to Grubb equation for eGFRcystatinC.
