Průjmová onemocnění patří u dětí na celém světě mezi nejčastější infekční onemocnění, a zejména v rozvojových zemích jsou stále signifikantní příčinou morbidity a mortality. Ročně jsou průjmová onemocnění na celém světě zodpovědná za smrt 1,5 milionů dětí v prvních pěti letech jejich života. Cílem práce bylo prokázat rozhodující podíl virových gastroenteritid na etiologii průjmových onemocnění u dětí do pěti let, a také poukázat na důležitost elektronové mikroskopie (EM) v jejich diagnostice. V prospektivní studii byla sledována etiologie průjmových onemocnění u dětí do pěti let hospitalizovaných na Infekční klinice NNB od září 2006 do června 2009. U všech dětí byla bakteriální etiologie vyšetřována kultivací stolice, virová etiologie pomocí latexové aglutinace (LAG) a EM. Do studie bylo zařazeno 1059 dětí. Průměrný věk dětí byl 1,9 roku s průměrnou délkou hospitalizace 5,2 dne. Etiologické agens se podařilo objasnit u 1051 dětí (95,9 %). Bakteriální etiologie byla zjištěna u 22 dětí (2,1 %), duální infekci (bakterie a viry) jsme ověřili u 183 pacientů (17,3%) a u 810 dětí (76,5 %) byla prokázána virová etiologie. Nejčastější původci akutních gastroenteritid (AGE) v naší studii byly rotaviry u 535 dětí, následovány caliciviry 43, koronaviry 43, adenoviry 20 a astroviry u 19 dětí. Velkou skupinu tvořily... Acute gastroenteritis is one of the most frequent diseases in children worldwide and remains the main cause of childhood morbidity and mortality, particularly in developing countries. Each year as many as 1.5 million children less than first five years die in the world from acute gastroenteritis. Our study has been aimed at demonstrating of the main role of viruses in the aetiology of acute gastroenteritis in children less than five years old and at pointing out the diagnostic potential of electron microscopy in viral gastroenteritis. A prospective study was conducted to analyse the aetiology of diarrhoeal diseases in children less than five years of age admitted to the Department of Infectious Diseases of the Na Bulovce Hospital, Prague, between September, 2006 and June, 2009. All children were tested by faecal culture for bacterial aetiology and by latex agglutination and electron microscopy for viral aetiology. A total of 1059 children were included in the study. The mean age of children was 1.9 years and the mean length of hospitalisation was 5.2 days. An aetiological agent was detected in 1051 children (95.9%).
