Cíl: Cílem práce je zhodnotit účinnost a komplikace alternativních stereotaktických metod (radiochirurgie pomocí gama nože a radiofrekvenční amygdalohipokampektomie) v léčbě meziotemporální epilepsie s meziální temporální sklerózou. Metody: Obě skupiny pacientů byly vyšetřeny standardním preoperačním epileptologickým protokolem. Oba zákroky byly plánovány s ohledem na individuální anatomické poměry. V období listopad 1995 až květen 1999 prodělalo radiochirurgickou amygdalohipokampektomii 14 pacientů. Okrajová dávka na 50% izodóze byla 18, 20 a 25 Gy. Mezi dubnem 2004 a říjnem 2009 byla stereotaktickými termolézemi identických struktur léčena skupina 51 pacientů. Léze jsme prováděli strunovou elektrodou zavedenou z okcipitálního přístupu jednou trajektorií. Výsledky: V radiochirurgické skupině bylo po 39 měsících dosaženo Engelova skóre Ib u 1, IIc u 3, IIIa u 4, IVb u 5 a IVc u 1 pacienta. V podskupině 7 neoperovaných nemocných bylo ve 2 letech předcházejících poslední kontrole, která proběhla nejméně 8 let po zákroku (průměrně 116 měsíců), skóre Engelovy klasifikace Ib u 1, IIc u 3, IIIa u jednoho a IVb u 2 pacientů. Nedostatečný efekt léčby nás vedl k indikaci operace u zbývajících 7 nemocných (průměrně 63,5 měsíce po iradiaci). Průměrná doba sledování po operaci u nich byla 43,5 měsíce. 5... Purpose: The aim of the study was to evaluate effectiveness and complications of alternative methods of stereotactic treatment (gamma knife radiosurgery and radiofrequency amygdalohippocampectomy) in mesial temporal lobe epilepsy due to mesial temporal sclerosis. Methods: Both patient groups underwent a standard preoperative epilepsy evaluation. Both procedures were planned according to the individual anatomy of each patient. Between November 1995 and May 1999, 14 patients underwent Leksell Gamma Knife radiosurgical amygdalohippocampectomy with a marginal dose of 18, 20, or 25 Gy to the 50% isodose following. Between April 2004 and October 2009 a group of 51 patients was treated using stereotactic thermo-lesion of the same mesiotemporal structures. Lesions were performed using a string electrode inserted through the occipital approach with a single trajectory. Results: In the radiosurgical group, one patient was classified as Engel Class Ib, three were Engel Class IIc, four were Engel Class IIIa, five were Engel Class IVb and one was Engel class IVc 39 months after treatment.
