MUDr. Martin Bareš Lékařská fakulta v Hradci Králové, Univerzita Karlova v Praze Název: Prospektivní, randomizované hodnocení účinnosti léčby antidepresivní monoterapií a kombinací antidepresiv u pacientů s rezistentní depresí. Úvod: Pouze kolem 60% pacientů trpících depresivní poruchou odpoví na 1. léčbu antidepresivem. Kombinace antidepresiv (KAD) je poměrně často používanou metodou léčby k překonání rezistence na léčbu u pacientů trpících depresivní poruchou. Cíl studie: Překládaná 6-týdenní, randomizovaná, otevřená studie porovnávala účinnost KAD a monoterapie antidepresivy (ADM), které byly vybrány dle úvahy ošetřujícího lékaře v léčbě pacientů trpících rezistentní depresní. Metoda: Celkem 60 hospitalizovaných pacientů (intent-to- treat analysis) trpících depresivní poruchou (≥1 předchozí neúspěšná antidepresivní léčba) podle kritérií Diagnostického a statistického manuálů Americké psychiatrické společnosti, 4. revize bylo zařazeno do randomizované studie. Pacienti, kteří dokončili akutní část studie a odpověděli na léčbu, byli dále sledováni po dobu dvou měsíců k posouzení stability odpovědi, resp. výskytu relapsu. Primárním ukazatelem výsledku byla změna skóru v Hodnotící škále deprese podle Montgomery-Åsbergové (MADRS) a odpověď na léčbu byla definována jako ≥50% redukce skóru ve škále MADRS.... Background: Approximately 60% of patients respond to first antidepressant treatment trial. Combination of antidepressants (CAD) is the strategy that is often used to overcome resistance to treatment in depressive patients. The aim of the study: This randomized, 6-week, open label study compared efficacy of CAD and antidepressant monotherapies (ADM) that had been chosen according to clinical judgment of the attending psychiatrist in the treatment of patients suffering from resistant depression. Methods: A total of 60 inpatients (intent-to-treat analysis) with major depressive disorder diagnosed according to Diagnostic and Statistical Manual of American Psychiatric Asociation, 4th revision (≥ 1 unsuccessful antidepressant treatment) were randomly assigned to the interventions. The responders who completed the acute phase of study, were evaluated for relapse within two months of follow-up treatment. The primary outcome measure was score change in the Montgomery-Åsberg Depression Rating Scale (MADRS), response was defined as a ≥50% reduction of MADRS score...
