Abstrakt disertační práce (český jazyk) První část našeho vědeckého projektu se zabývá chirurgickou léčbou FiS u pacientů, kteří podstupují kardiochirurgický výkon pro jinou základní diagnózu (ICHS a/či chlopenní vadu). Různé ablační procedury se běžně provádí na většině kardiochirurgických pracovišť jako součást výkonu, s očekáváním pozitivního klinického efektu pro pacienty. Nikdy však nebyly provedeny dostatečně silné studie s dlouhodobým sledováním, které by tento předpoklad potvrdily. Naše studie PRAGUE-12 je dosud největší prospektivní, randomizovanou, multicentrickou studií na dané téma. Výsledky ukázaly, že přidání ablace ke kardiochirurgickému výkonu je bezpečné a signifikantně zvyšuje pooperační výskyt SR během prvního roku od operace. Klinický benefit ve smyslu snížení počtu závažných komplikací v prvním roce od operace však prokázán nebyl. Související publikace přináší podrobný popis metodiky a designu této studie a její roční výsledky. Druhá část našeho vědeckého projektu se zabývá chirurgickou léčbou izolované FiS. V roce 2006 jsme jako první klinika v ČR provedli plně torakoskopickou chirurgickou ablaci této arytmie. V následujících letech bylo takto...Surgical treatment of atrial fibrillation: prospective randomized study PRAGUE-12 and EndoMAZE procedure (toracoscopic ablation of atrial fibrillation) Abstract (English) First part of our research project is focused on surgical treatment of atrial fibrillation in patients who undergo a cardiac surgery for some other primary diagnosis (ischaemic heart disease and/or valve disease). Concomitant surgical ablation procedures are widely used with expected positive long-term clinical impact on patients. However, this assumption has never been confirmed by enough powerful randomized studies with long-term follow up. Our PRAGUE-12 trial is the biggest, prospective, multicenter, randomized trial conducted to date. Its results demonstrated that surgical ablation improves the likelihood of SR presence post-operatively without increasing peri-operative complications. However, the higher prevalence of SR did not translate to improved clinical outcomes at 1 year. Related publications describe in detail its design and its one-year results. Second part of our research project is focused on surgical ablation of lone atrial fibrillation.
