[cze][eng] Chronická tromboembolická plicní hypertenze (CTEPH) je chronickým důsledkem akutní plicní embolie. Projevuje se nárůstem tlaku v malém oběhu. Tento stav, pokud není léčen, může vést k pravostrannému srdečnímu selhání. Možnou léčbou CTEPH je chirurgická plicní endarterektomie (PEA), která spočívá v odstranění organizovaného fibrotizovaného trombu s částí cévní stěny plicnice. Předpokladem úspěchu operačního výkonu je výběr vhodných pacientů, především s centrálně uloženým postižením a s absencí periferních remodelačních změn. Právě periferní remodelační změny jsou současnými diagnostickými metodami obtížně odhalitelné. Ze sledování pacientů s hypertenzními aneurysmaty plicnice je známo, že mají relativně dobrou životní prognózu a dilatace plicnice je projevem letitého pozvolného rozvoje onemocnění. Také periferní cévní remodelace u CTEPH je typická pro léta probíhající onemocnění. Proto by míra dilatace plicnice u CTEPH mohla korelovat s periferní remodelací. V naší práci jsme potvrdili, že pacienti s CTEPH s významnější dilatací plicnice mají delší anamnézu obtíží, těžší plicní hypertenzi a vyšší plicní cévní rezistenci. Neprokázali jsme korelaci mezi velikostí dilatace plicnice a poklesem systolického tlaku v plicnici, resp. přítomností reziduální plicní hypertenze 6 měsíců po PEA. Rovněž jsme...Chronic tromboembolic pulmonary hypertension (CTEPH) is believed to result from acute pulmonary embolism. It manifests itself in the growth of blood pressure in pulmonary circulation. This condition, if not treated, can lead to the failure of right heart. The treatment of CTEPH is the surgical pulmonary endarterectomy, which consists in enucleation of organized fibrotic thrombus with the part of the vascular arterial wall. The prerequisite for the successful surgery is the selection of suitable candidates, mostly the patients with the central location of disability and the absence of the peripheral vascular remodelation. The peripheral vascular remodelationis difficult to recognize by contemporary diagnostic methods. A recent study has shown that patients with hypertensive aneurysm of the pulmonary artery have the good life prognosis and the dilation of the pulmonary artery is the manifestation of the long lasting disease. Peripheral vascular remodelationof CTEPH is also typical for the long lasting disease. Therefore, the measurement of dilatation of the pulmonary artery in patients with CTEPH could correlate with the peripheral remodelation.
