Roztroušená skleróza (RS) představuje demyelinizační onemocnění centrálního nervového systému se známou autoimunitní etiologií. V současné době máme k dispozici diagnostická kritéria, která umožňují stanovení diagnózy již po první atace klinických příznaků. Zároveň je využívána řada léků, kterými můžeme redukovat aktivitu onemocnění a oddálit stádium nevratné invalidity. Prognózu individuálního pacienta a adekvátní léčbu však zatím určujeme jen nedokonale na základě klinické zkušenosti a magnetické rezonance mozku. Přesnější prognostické ukazatele u RS chybí. Cíle práce: 1. Vytipovat vhodný prognostický imunologický parametr periferní krve u pacientů s RS v různém stádiu onemocnění a léčby; 2. Popsat skupinu pacientů léčených autologní transplantací kmenových buněk (ASCT) nebo allogenní transplantací kmenových buněk (alloSCT) ve vztahu k efektu, nežádoucím účinkům a vhodným indikačním kritériím k této léčbě. Pacienti a metody: V první části studie bylo zahrnuto 33 pacientů s klinicky izolovaným syndromem, 17 pacientů s RS léčených natalizumabem a 14 pacientů s agresivní RS léčených ASCT. U pacientů byl hodnocen neurologický nález pomocí standardní škály EDSS (Expanded Disability Status Scale) a přítomnost relapsů před zahájením vybrané léčby a ve 3., 6., 12. a 24. měsíci od zahájení léčby. Ve... Multiple sclerosis (MS) represents a demyelinating disease of the central nervous system with known autoimmune etiology. Currently new diagnostic criteria are used allowing us to diagnose MS early after first relapse of clinical symptoms. Several drugs are available to reduce disease activity and postpone later MS stages with irreversible disability. Prognosis of an individual patient and accurate treatment is however defined only imperfectly based on our clinical experience and brain magnetic resonance imaging. Specific prognostic markers are missing. Aims: 1. To identify suitable prognostic immunological marker from peripheral blood of MS patients in different disease stages and under different treatment regimens; 2. To describe group of MS patients treated with autologous stem cells transplantation (ASCT) or allogeneic stem cells transplantation (alloSCT) with respect to efficacy, adverse events and accurate patient selection. Patients and methods: In the first part of the study we involved 33 patients with clinically isolated syndrome, 17 MS patients treated with natalizumab and 14 patients with aggressive MS treated with ASCT.
