Úvod: Enterotoxiny produkované Staphylococcus aureus (SA) se mohou uplatňovat jako superantigeny a takto, nezávisle na antigenní specifitě, ovlivňovat průběh chronické rinosinusitidy s nosními polypy (NP). Cílem studie bylo prokázat zda antibiotická léčba podaná po endoskopické operaci paranazálních dutin pro NP může pozitivně ovlivnit průběh onemocnění ve srovnání s placebem. Pacienti a metody: 23 pacientů po operaci paranazálních djutin pro NP, s pozitivním kultivačním peroperačním nálezem SA, pozitivně testovaným na schopnost produkce enterotoxinů A-E a TSST-1 bylo randomizováno do 2 skupin. Skupina A (13 nemocných) byla navíc k běžné léčbě intranazálními steroidy a minerálními sprškami léčena 3 týdny anti-stafylokokovým antibiotikem dle minimální inhibiční koncentrace. Skupina B (10) dostávala kromě zvyklé léčby placebo. Obě skupiny byly porovnány předoperačně, za 3 a 6 měsíců po operaci pomocí symptom specifického skóre, endoskopického skóre a dotazníku kvality života SNOT-22 v českém překladu. Výsledky: Mírně lepších výsledků bylo dosaženo u nemocných s přidanou antibiotickou léčbou. Rozdíly však nebyly však statisticky významné. Nejvýraznější rozdíl ve prospěch skupiny A byl dosažen u endoskopického skóre za 3 měsíce po operaci. Bez ohledu na způsob léčby u asi 30 % nemocných došlo k vymizení... Enterotoxins produced by Staphylococcus aureus can act as superantigens and thus, regardless of the antigenic specificity, influence the course of chronic rhinosinusitis with nasal polyps (NP). The aim of this study was to determine if antibiotic treatment administered after endoscopic sinus surgery (ESS) for NP can positively influence the course of the disease compared to placebo. After ESS, 23 patients which tested positive, in a perioperative culture, for SA strains producing enterotoxins A - E and TSST-1, were randomized into 2 groups. Group A (13 patients), which in addition to treatment with intranasal steroids and saline douches, received oral anti-staphylococcal antibiotics (according to the minimal inhibitory concentration) for 3 weeks. Group B (10 patients) received a placebo. Both groups were compared preoperatively, then again at 3 and 6 months after surgery using a symptom-specific score, an endoscopic score and the SNOT-22 quality of life questionnaire. Slightly better results were achieved in patients who received antibiotic therapy. However, the differences were not statistically significant.
