Abstrakt: [cze][eng] Tato bakalářská práce se zabývá ošetřovatelskou péčí o ženy před a po LAVH (laparoskopicky asistovaná vaginální hysterektomie). Popisuje přípravu pacientek před výkonem, ošetřovatelskou péči před výkonem a po něm. Práce se také zaměřuje na psychickou přípravu pacientky, neboť psychika ženy hraje v tomto případě velkou roli. Práce je rozdělena na část teoretickou a část praktickou. Teoretická část bakalářské práce byla vypracována na podkladu odborné literatury a nejnovějších poznatků. Praktická část bakalářské práce je zaměřena na výzkum strachu, který mají ženy bezprostředně před operačním výkonem a na změnu tělesného obrazu po operačním výkonu. V bakalářské práci byly stanoveny celkem dva cíle. Cíl 1: Zjistit, z čeho mají ženy největší strach před operačním zákrokem. Cíl 2: Zjistit, jak se změnil tělesný obraz ženy po operačním zákroku. Na základě stanovených cílů byly vytvořeny čtyři výzkumné otázky. První výzkumná otázka zní: "Jaké techniky využívají ženy ke zmírnění strachu?", druhá: "Čeho se ženy nejvíce obávají před operačním zákrokem?", třetí: "Jak jsou ženy informovány o operačním zákroku?", čtvrtá: "Jak vnímají ženy svůj tělesný obraz po operačním zákroku?" Ke zpracování výše uvedené problematiky bylo stanoveno kvalitativní výzkumné šetření. Sběr dat probíhal ve dvou případech formou nestrukturovaných hloubkových rozhovorů. Na základě těchto dvou rozhovorů bylo stanoveno několik nejdůležitějších otázek, na jejichž podkladu výzkum dále probíhal formou polostrukturovaných rozhovorů. Výzkumný soubor prezentovaly náhodně vybrané ženy podstupující LAVH v Příbrami, které souhlasily se spoluprací. Rozhovory probíhaly až po ústním informovaném souhlasu respondentky a byly zcela dobrovolné. Respondentky byly seznámeny s tím, že získané údaje budou anonymně zpracovány v bakalářské práci. Respondentky jsou v práci označeny jako R1-R10. Výzkumné šetření probíhalo v měsíci březnu v Oblastní nemocnici Příbram na základě písemné žádosti (viz. příloha č. 2: Souhlas s provedením výzkumného šetření). Pro kvalitativní výzkum bylo vybráno deset žen, kterým byla indikována LAVH. Informace získané od jednotlivých žen byly průběžně zapisovány a přepisovány do spisovného jazyka. Výpovědi respondentek jsou v textu označeny kurzívou. Výzkum je rozdělen do dvou částí, na část před výkonem a část po výkonu. Na základě výzkumného šetření bylo zjištěno, že oslovené respondentky mají největší strach z narkózy, což potvrdilo osm z deseti dotazovaných. Tělesný obraz se změnil pouze u třech z deseti dotazovaných respondentek, které se změnou počítaly již před výkonem. Na první výzkumnou otázku se nám dostalo odpovědi, že nejčastější využívanou technikou ke zmírnění strachu je rozhovor s lékařem, který respondentky považují za klíčový. Nejlépe jsou poučeny respondentky, které informace čerpaly od svého gynekologa. Zatímco respondentky, které hledaly informace na internetových diskuzích apod., měly informace mnohdy zmatené a nepřesné, což dokazovaly při rozhovorech. Tento fakt nám odpovídá na třetí výzkumnou otázku. Poslední výzkumná otázka se týká změny tělesného obrazu, která úzce souvisí s naším druhým cílem. Celkově lze soudit, že pro mnohé ženy odstranění dělohy neznamená ztrátu ženství. Toto tvrzení potvrzují pouze tři respondentky z deseti.Do budoucna by práce mohla sloužit jako informační materiál pro ženy, které budou absolvovat LAVH a chtějí se více přiblížit této problematice. Podstatnou součástí informačního materiálu jsou samotné názory žen, které již tento zákrok podstoupily. This bachelor thesis deals with nursing care of women before and after LAVH (laparoscopically assisted vaginal hysterectomy). It describes preparation of the patients before the procedure and nursing care before and after the procedure. The thesis also focuses on psychical preparation of the patients, because the woman's psyche plays an important role in this case.The thesis has a theoretical and a practical part. The theoretical part was done after researching scholarly literature and the latest knowledge in the domain. The practical part of the bachelor theses is focused on the research of the fear that the women feel immediately before the procedure and on the research of the change in the body image after the procedure. Two goals were set in this thesis: the first goal was to find out what the women are afraid of the most before the procedure. The second goal was to find out how the body image changed after the procedure. Four research questions were established on the ground of these goals. The first question was: "What techniques do the women use to mitigate the fear", second question: "What are the women afraid of the most before the procedure?", third: "How are the women informed about the procedure?", fourth: "How do the women perceive their body image after the procedure? "Qualitative research procedure was established to process the aforementioned problem. In two cases the data gathering was performed by non-structured depth conversations. On the ground of these two conversations several vital questions were established, which served as the foundation for the rest of the research, which was conducted in the form of semi-structured conversations. The research sample was represented by randomly chosen women that were to undergo LAVH in Příbram, who agreed with the cooperation. The conversations were conducted only after verbal agreement given by the respondents and they were completely voluntary. The respondents were informed that the obtained results would be processed anonymously in this thesis. The respondents are labeled as R1 to R10.The research was conducted during the month of March in the Regional hospital of Příbram after issuing a written request (see Appendix 2: Consent with conduction of a research). The research is broken down into two parts, a part before the procedure and a part afterwards.From the research we found out that the patients are afraid of the narcosis the most as this was confirmed by eight of the ten interviewed patients. The body image changed only by three of the patients who had counted with the change already before the procedure. As to the first research question, we learnt that the most commonly used technique to mitigate fear is conversation with the doctor, which is considered vital. Patients who learnt information from their gynecologists were advised the best. On the other hand, patients looking for information on the internet discussion forums had only inaccurate or misleading information, which they showed during the conversations. This answers our third research question. The last research question relates to the change in the body image and also closely relates to our second goal. We can say that the fact that patients' uteruses have been taken out doesn't mean the loss of their womanhood for most of them. The opposite is true for only three of the ten patients. In the future this thesis can serve as an information material for women who are to undergo LAVH and who want to learn something from the domain. Opinions of women patients who already underwent the procedure are important part of this information material.
