Abstrakt: [cze][eng] Domácí péče se již stala nedílnou součástí péče o dlouhodobě nemocné pacienty. Ve své práci jsem se zaměřila na sledování kvality domácí péče, na její současný stav v České republice a v Rakousku. Osobní zkušenost v rakouské domácí péči mě zaujala natolik, že jsem se rozhodla pro její bližší zkoumání. Za cíl práce jsem si stanovila nalézt rozdíly v domácí péči v České republice a v Rakousku, zejména ve vztazích mezi pečovateli a klienty. Použila jsem dostupnou odbornou literaturu, platnou legislativu v Rakousku i Česku a internetové zdroje věnující se domácí péči. Má bakalářská práce je složena z teoretické části, která obsahuje čtyři kapitoly. V první kapitole teoretické části se zabývám definicí imobility, jejímž hlavním projevem je ztráta pohyblivosti, i tím, jak nepříznivě působí tento tělesný stav na psychiku člověka. Ve druhé kapitole se věnuji problematice imobilizačního syndromu, který je popisován jako fyziologická odpověď organismu na imobilitu a projevuje se různými poruchami řady orgánů. Věnuji se i potřebám imobilních klientů, pro které je charakteristická dynamická síla, která se vytváří z pocitu nedostatku nebo přebytku, touhy něčeho dosáhnout. Ve třetí kapitole práce charakterizuji domácí a sociální péči, jež je popisována jako vysoce kvalifikovaná a odborná forma péče, ve které se spojuje péče zdravotní i sociální. Čtvrtá, poslední, kapitola teoretické části pojednává o osobě pečovatele, kterým je pracovník, provádějící pečovatelskou činnost v domácnosti klientů. Zabývám se i předpoklady pro práci pečovatele a charakterizuji rozdíl mezi angažovaným a neangažovaným pracovníkem. Součástí kapitoly je i přehled kompetencí a kvalifikačních požadavků na pečovatele v České republice a Rakousku. V závěru srovnávám finančního ohodnocení pečovatele v domácí péči ve sledovaných zemích. V praktické části své bakalářské práci analyzuji data, která jsem získala technikou kvalitativního výzkumu za použití metody dotazování formou polostandardizovaného rozhovoru a také vlastním pozorováním, příp. analýzou osobních dat sledovaných klientů domácí péče. Cílovou skupinu tvořily komunikativní klientky domácí české a rakouské péče, které měly II., III. a IV. stupeň závislosti. Respondentkami byly tři ženy z Dačic a tři ženy z Raabs an der Thaya. Po analýze dat výzkumu jsem došla k závěru, že existuje podstatný rozdíl ve vztahu mezi pečovateli a klienty v porovnávaných službách. Mezi českými klienty a pečovateli nedochází z důvodu časové vytíženosti pečovatelů k navázání bližšího vztahu ke klientům. Z mého výzkumu také vyplynulo, že rakouská péče je historicky starší, a proto je celý systém lépe propracovaný a ekonomicky zajištěnější. Domácí péče se v Rakousku poskytuje komplexněji a do systému této služby proudí více financí. To se kladně promítá jak v personálním obsazení, tak finančním ohodnocení zaměstnanců. I po demonstraci rozdílů v systému poskytování péče, která se ve zkoumaných zemích liší, jsem došla k závěru, že kvalita, míra a obsah poskytované péče jsou v obou zemích stejné. Home care has already become an integral part of taking care of patients suffering of long-term sickness. I focused on the quality of home care and its current state in Czech Republic and in Austria. Experience earned in the Austrian domestic care attracted me the way that I decided to examine it closer. I set the objective to find the differences between the care in Czech and Austrian home care and the differences in relations between care workers and clients. I used available scientific literature, legislation valid in Austria and Czech Republic, and sources from the Internet related to the home care. My bachelor thesis consists of the theoretical part, which contains four chapters. In the first chapter I deal with immobility, where the main manifestation is the motion, which stimulates the motion apparatus and has a positive impact on the psyche of man. The target group for a long-term care are seniors, people with disabilities and chronically sick people. In the third chapter I characterize home and social care, which is described as highly qualified and professional form of care, which is linked with the health and social form. In Czech Republic, 2/3 of care delivered by agencies is focused on health issues, but it is in close cooperation with subsidiary agencies aimed at nursing services. Home care in Austria is not divided into health services, nursing or social services. This care provides complex services. Linking health and social care helps the client to find life stability. This care substitutes hospitalization and it shall be granted in its own environment, what affects the psyche of the client very favourably and it significantly affects whole recovery process. The fourth, and the last, chapter of the theoretical part deals with the personality of carer, when the carer is the worker, who carries out the care activity in the household of clients. Furthermore I describe here detailed assumptions to the work of the carer, the difference between committed and uncommitted worker, an integral part are competences and skill requirements placed upon the carer in Czech Republic and in Austria, and part of the conclusion is the sample of financial evaluation of carer in home care in concerned countries. The practical part of my bachelor thesis consists of collection of data, collected while using techniques of the qualitative research, the method of query based on using semi-structured interview, observation and analysis of personal data. Characteristic of the target group was created by communicative clients of Czech and Austrian home care, who carried II, III, IV degree of dependence. The respondents were three women from Dačice and three women from Raabs an der Thaya. Further I describe the process of the data collection and results of interviews. On the basis of my research, I came to the conclusion that there exists a difference in the relationship between carers and clients in compared services. Due to busyness, there is My research showed, that the Austrian care is historically older, so it may be the reason for better sophistication of whole system, it is economically more stable and, therefore, more financial sources flow to the system of this service, and that is reflected in the personnel and the financial evaluation of the staff. After pointing at the differences in the system of care, which is different in examined countries, my conclusion is, that the degree and the content of delivered services provided in both countries is the same, it is only provided in a different way. Clients are, in both home cares, health and socially secured, and this allows them, under the burden of their diseases, to stay at home in the environment, where they like it and where they feel safe. This theory is one of the objectives of home care, to ensure bio-psycho-socio needs of man. Czech and Austrian home care fully meets these basic objectives.
