Detection of pathogens in joint infections, including total endoprostheses using the BioFire method (Biomérieux)

This bachelor thesis deals with the detection of pathogens in joint infections including total joint replacements by the BioFire method (Biomérieux, France). Compared to conventional methods, the Biofire Joint Infection Panel (BioMérieux, France) offers simultaneous detection of up to 39 pathogens and antimicrobial resistance markers in about one hour of time.This method is a multiplex PCR system for rapid and comprehensive testing. It is an innovative method that, thanks to its speed and comprehensiveness, will allow clinical microbiologists and physicians to make timely decisions on appropriate antibiotic therapy. Early de-escalation of this antibiotic therapy will also be enabled through the timely and targeted deployment of the appropriate ATB. The theoretical part of the thesis discusses the major infections of joints and joint replacements, introduces the reader to the most important causative agents of these infections, and a section is also devoted to microbiological diagnosis. The practical part describes the classical multiplex PCR, the FilmArray multiplex PCR system and with it the BioFire Joint Infection Panel, which has been used in the analysis of synovial fluid in patients with suspected septic arthritis or periprosthetic infection. Data from the analysis are also evaluated in the practical part. The data are from March 2021 to June 2022 and are presented in the practical part in the form of tables and graphs. The conclusion of the paper summarizes the relevance of the method for routine diagnosis and answers the stated research questions.
Stress analysis of the total hip joint endoprosthesis head under oblique loading

This diploma thesis is concerned with the stress analysis of the total hip joint endoprosthesis ceramic head under oblique loading. Load was obtain from experimental in vivo measurement on three patients, whereas two of them was the same weight. As input variables were use contact forces, especially knee bend and standing on one leg. Furthermore they will be considered shape deflections from ideal cone, especially circle deviation, cone deviation and cone plus circle deviation together.

It contains an overview of the total knee replacement currently produced in the Czech republic and abroad. Machining technologies used in medicinal producing

This bachelor‘s thesis is focused on the connection of machining technologies with medical production. The first part includes an overview of a few Czech companies that pursue production of joint replacements. There are also some examples of the joint replacements included in this part. Frequently used materials for implants and constructions of dental replacements are included in the following part. The next section contains a summary of machining technologies used in this area. Additionally, there are a few technological production processes of components of joint replacements which are mentioned in the first part. This part also outlines machining technologies used in constructions of dental replacement production. Also included is an evaluation of the possibilities of producing femoral components of a knee replacement, as well as points out the specificity of machining technologies used in stomatology. The ending section of the thesis contains an overview of machining machines, tools, and software used in these areas. Lastly, there is a final evaluation concluding the work as a whole.

Can the adverse complications of foot drop be prevented by an endoprosthesis: design and development of a prototype device Abstract Foot drop is a very old problem since Jacob from the bible was limping when he was wrestling with the angels. Foot drop occurs very often, and it is a very common condition in trauma, after surgery, and neurological diseases. The aim of this project is to identify an endo-prosthesis and test it bio-mechanically and bio- medically in order to resolve the problem. This endo-prosthesis must improve the quality of life for patients suffering from foot drop due to the injury to the common peroneal nerve. The device that must be created should be small and surgically will be attached to the muscle internally and work as if the muscles and nerve were intact. The device would be tested outside the human body. It is important to throw light on that the device should be made of a material that is accepted by the human body and should have a stiffness that is close to the weight of the foot to ensure a normal motion. The intent of this bio-mechanical device is to help patients suffering from foot drop to restore normal motion. Title: Case Study of Physiotherapy Treatment of a Patient with Diagnosis of Coxartrosis after Hip Endoprosthesis Thesis aim: Theoretical description of the hip joint, coxarthrosis diagnosis, endoprosthesis, and practical description of therapy for the acute phase following hip endoprosthesis. Clinical findings: The patient is 62 years old due to coxarthrosis, she underwent endoprosthesis surgery on her right lower extremity, she has a reduction in ROM at the level of the hip in flexion, extension and abduction, she is unable to fully extend her right knee. Swallen anterior and posterior lateral aspect of the tight, muscle weakness in the right lower limb. Summary: The thesis is divided into two parts. The initial section is theoretical, with a brief summary of hip joint anatomy, biomechanics, and kinesiology, as well as coxarthrosis as a disease and its prevention, hip endoprosthesis surgery clinical views and approaches, and hip endoprosthesis rehabilitation protocol. The second component is a description of the patient case study, which includes a comprehensive initial kinesiological examination, treatments used, a final kinesiological assessment, and the result of the therapies utilized. Milan Martínek Year of defense: 2021 Objectives: The purpose of my bachelor thesis is to compose a case study of physiotherapeutic treatment of a patient after total knee replacement due to knee osteoarthritis during her stay at CLPA. Also, to present the theoretical foundation for the practical part of the thesis which focuses on the knee joint, knee osteoarthritis, conservative and surgical treatment, total knee replacement and following rehabilitation. Method: This bachelor thesis is divided into theoretical and practical part. In the theoretical part the topics of knee joint, knee osteoarthritis, total knee replacement and following rehabilitation are discussed. The practical part contains entry and leaving examination of a patient, goals and plans for rehabilitation and description of therapeutic units. The latter part is based on bachelor practice at CLPA in May 2020. Results: The physiotherapeutic methods used were effective. The physical condition of the patient improved.
