Analysis of prescription and potential drug interactions in patients with inflammatory bowel disease.
Antoušková, Helena ; Vlček, Jiří (advisor) ; Rudolf, Kamil (referee)
Background: Concurrent use of multiple drugs often increases therapeutic effectiveness, but very often may also pose a risk for drug interactions. These may result in severe clinical manifestation, if they are not paid an attention and unproperly managed. Aim: To assess percentage of potential drug interactions in the population of patients with diagnosis of inflammatory bowel disease (IBD) and suggest their management. Methods: 177 patients with IBD (84 male, 93 female, mean age 36,9, range 16 - 78, 117 CD, 60 UC) enrolled. Their complete medication was gained by means of an interview and subsequently evaluated and processed with Thomson MICROMEDEX® DRUG - REAX® System to reveal potential drug interactions. All the identified drug interactions were taken as potential. There was no prove if patient was harmed by them. Results: In CD, the most prescribed drugs were aminosalicylates (39 %), corticosteroids (both topical and systemic)(21 %) and immunosupressants (23 %). The other prescribed medicines were proton pump inhibitors, iron containing preparates, probiotics and TNF-α inhibitors. In UC, the most prescribed drugs were aminosalicylates (51 %), corticosteroids (both topical and systemic)(22 %) and immunosupressants again (13 %). The other prescribed medicines were proton pump inhibitors, iron...

Effect of antihypertensive therapy was assessed including conformity local prescribing with current guidelines. Methods: Data of the prospective study were collected by a questionnaire. The exploratory sample was consisted of randomly 320 surveyed hypertensive patients living in the Štětí and its surroundings. The research was performed from July 2010 to November 2011. Analysis of drug related problems identificated by pharmacist during provision dispensation and the influence of pharmacist's intervention on prescribing doctors Author: Mgr. Zdeňka Močárková 1 Consultant: Prof. RNDr. Jiří Vlček, CSc. 2 1 Community pharmacy in Chrastava 2 Department of Clinical and Social Pharmacy, Charles University in Prague, Faculty of Pharmacy in Hradec Králové Introduction: Drug problems (drug related problems, DRPs) shall mean all the circumstances involving drug therapy that can actually or potentially interfere with the expected outcome of therapeutic intervention. The solution DRPs (risk of pharmacotherapy) includes their detection, risk assessment and selection of the appropriate intervention and belongs to the one of the fundamental activities of a pharmacist. Aim of study: The aim of this work was to analyze DRPs, their use as a tool for discussion with doctors and subsequent evaluation and estimation of the limits of this model of cooperation between doctor and pharmacist. Methods: A prospective observational study was conducted in the village Chrastava (1 public pharmacy). Record DRPs detected pharmacist during provision dispensation was conducted in two equally long periods (2 x 30 working days) by using electronic databases (including data of patient, patient used...

The mechanism of cell death induction in human malignant melanoma in vitro
Rudolf, Kamil ; Červinka, Miroslav (advisor) ; Mlejnek, Petr (referee) ; Janisch, Roman (referee)
Melanoma is a complex malignant disease characterized by increasing incidence and mortality. Melanoma biology is complicated and comprises many aberrant genes and signaling pathways. Treatment of melanoma relies on surgery, the role of radiation therapy and chemotherapy are limited. In advanced disseminating and metastasizing melanoma chemotherapy largely fails and that is why new approaches are being sought including identification of deregulated molecules or processes which could be used as targets. Some of these targets center on frequently aberrant cell death regulating mechanisms. Some traditional cytostatic drugs such as inhibitors of topoisomerases although useful in other types tumors so far proved useless in treatment of melanoma. Nevertheless, expression of topoisomerases in melanoma is often high, thereby enabling at least theoretically their use as targets for chemotherapeutical intervention. Aim of this study was to assess the cytoxicity and proapoptotic effect of campthotecin (topoisomerase I inhibitor), etoposide (topoisomerase IIα inhibitor) and their combination in selected human melanoma cell lines. We found that campthotecin may induce apoptosis by a combination of mechanisms including p53 as well as p73 and kaspase-2. Etoposide-induced cell death in Bowes melanoma cells included...
