Zdeňka Močárková 1 Konzultant rigorózní práce: Prof. RNDr. Jiří Vlček, Csc. 2 1 Lékárna Chrastava 2 Katedra sociální a klinické farmacie, Univerzita Karlova v Praze, Farmaceutická fakulta v Hradci Králové Úvod: Lékovými problémy (drug related problems, DRPs) se rozumí všechny okolnosti týkající se farmakoterapie, které mohou aktuálně nebo potenciálně zasahovat do očekávaného výsledku terapeutické intervence. Řešení DRPs (rizik farmakoterapie) zahrnuje jejich odhalování, posouzení rizika a volbu vhodné intervence a patří mezi stěžejní činnosti farmaceuta. Cíl práce: Cílem práce byla analýza DRPs, jejich využití jako nástroje k diskuzi s lékaři a následné zhodnocení a odhad limitů tohoto modelu spolupráce lékaře a lékárníka. Metodika: Prospektivní observační studie byla realizována v obci Chrastava (1 veřejná lékárna). Záznam DRPs odhalených lékárníkem při dispenzaci léčiv byl prováděn ve dvou shodně dlouhých obdobích (2 x 30 pracovních dní) pomocí elektronické databáze (včetně údajů o pacientovi, užívaných léčivech, intervenci farmaceuta aj). The solution DRPs (risk of pharmacotherapy) includes their detection, risk assessment and selection of the appropriate intervention and belongs to the one of the fundamental activities of a pharmacist. Aim of study: The aim of this work was to analyze DRPs, their use as a tool for discussion with doctors and subsequent evaluation and estimation of the limits of this model of cooperation between doctor and pharmacist.
