Non-bioptic diagnosis of coeliac disease and following a gluten free diet

IN ENGLISH This thesis deals with non-biopsy diagnosis of coeliac disease and following the glu-ten-free diet in children. The theoretical part describes basic knowledge about coeliac dise-ase, its incidence, clinical presentation, diagnostics and prevention. This part also describes the principles of gluten- free diet, gluten-free labeling and financial demand of this diet.The research part deals with adherence to the gluten-free diet in pediatric patients both with bi-opsy and non-biopsy diagnosis. The aim of the study is also to determine if there are any differences between those two groups. The data were collected through the questionnaire that was mailed to selected respondents.Thirty eight respondents out of fifty responded to the study. The responses were recorded in a form of graphs. It was found out that 24 % of respondents knowingly violate gluten-free diet. No distinct difference of behaviour in both groups was recorded. 58 % of respondents considered the transition to a gluten-free diet as very challenging. The high costs of gluten-free food were recognized by respondents as the main problem. My bachelor thesis focuses on postnatal nutrition within the first year. The work is divided into two parts. The theoretical part focuses on the different nutritional values of milk nutrition, breast feeding, and its positive impact on child development, moreover on the issue of alternative milk substitutes and the introduction of various kinds of complementary feeding. The practical part is devoted to the application of theoretical knowledge into practice. I am evaluating and processing questionnaire that I created for this purpose. I identify the duration of breastfeeding in particular, introduction of various baby foods and a selection of infant milk substitutes. Lactose intolerance and diet solution

This bachelor thesis deals with lactose intolerance and dietary solutions. The work is divided into a theoretical and practical part. The theoretical part describes lactose intolerance, for example pathophysiology, incidence, clinical presentation, diagnosis, complications and treatment. The importance of drinking milk for children is explained in the chapter called Milk. The theoretical part is mainly devoted to dietary solutions. Lactose tolerance and principles of a low- lactose diet and lactose-free diet are described there. The table below contains an overview of common dairy products, including their hidden lactose value. Food which should be eatten, if lactose intolerance appears is described in the food section. This section is divided into naturally lactose free, reduced lactose and lactose free and alternative foods. The theoretical part also includes nutrition recommendations in cases of lactose intolerance. The practical part is about public knowledge of lactose intolerance. New regulatory hormones of breast milk

Background: Breastfeeding is supposed to exert long-term protection of the infant from the development of civilisation diseases later in life. Adiponectin, leptin and AFABP are candidate proteins in nutritional programming of the infant. Aim: To determine intraindividual changes of breast milk levels of regulatory hormones (adiponectin, leptin, AFABP) and adipophilin in breast milk during 12 months of lactation. Methods: Proteins were measured using high sensitive ELISA method in 72 healthy mothers within 2 days of beginning of lactation (day 0) and 1, 3, 6 and 12 months following delivery. Results: Adiponectin levels in breast milk on day 0 (D0) were 22.8 ± 0.8 (mean ± S.E.M.), in 1 month (M1) 22.0 ± 0.6, in 3 months (M3) 20.5 ± 0.6, in 6 months (M6) 21.4 ± 0.8, and in 12 months (M12) 25.7±1.4 ng/ml. AFABP levels were 12.3 ± 2.0, 6.2 ± 1.3, 1.3 ± 0.2, 2.5 ± 1.0 and 4.6 ± 1.9 ng/ml, respectively. Leptin levels were 0.3 ± 0.04, 0.2 ± 0.03, 0.1 ± 0.01, 0.1 ± 0.02 and 0.2 ± 0.04 ng/ml, respectively. Adipophilin levels in D0 were 1.98 ± 0.12, in M1 2.83 ± 0.21, in M3 2.39 ± 0.17, in M6 2.57 ± 0.16, and in M12 3.25 ± 0.21 μg/ml. Celiac Disease in Childhood and Problem of Gluten Free Diet

IN ENGLISH This thesis is dedicated to Celiac disease in childhood and problem of gluten- free diet. The theoretical part describes the general knowledge of celiac disease and gluten-free diet. In the research section we examine two main problems: compliance with a gluten-free diet and the change of the proportionality while following the gluten- free diet. We have investigated these two problems in a group of 100 children. We found out that 83,8% of children respected the diet and only 16,2% did not. In the 5-year interval we found out that if they follow a gluten-free diet, the proportionality is getting better. The average proportionality increased by 14,196 percentile. The results are positive, but we should not forget the education. The motivation and education to following the gluten-free diet should be given high priority, because of the later potential complications. Keywords: celiac disease, gluten, gluten-free diet
Anemia in IBD children. Evaluation of soluble transferrin receptor and soluble transferrin receptor - ferritin index in diagnosis and differentiating iron deficiency anemia from anemia of chronic diseases

Introduction: Prevalence of anemia in inflammatory bowel disease (IBD) is high and there is an important impact of anemia on the quality of life, morbidity and mortality in these patients. Iron deficiency anemia (IDA), anemia of chronic disease (ACD) and combination of both of these conditions represent the most frequent causes. It can be difficult to distinguish IDA in condition of inflammation. In fact, the common diagnostic tests (iron, ferritin and transferin saturation) are significantly influenced by inflammation and normal ferritin concentration does not exclude iron deficiency. Soluble transferin receptor (sTfR) and calculated sTfR/F index (sTfR/log ferritin) have a high diagnostic power in diagnosis of IDA in case of chronic inflammation. Aims: 1. To determine the prevalence of anemia and the prevalence of the types of anemia (IDA, ACD, ACD/ID) and 2. To compare common laboratory tests with sTfR and calculated sTfR/F index in diagnosis of iron deficiency in children at IBD onset. Methods: We examined 56 patients with newly diagnosed IBD. Changes Air Transport Industry after Deregulation, Specific Case of Germany

The theme of this thesis is the air transport and its dramatic and dynamic development in the last twenty years. The key driving force behind this change was the deregulation of air agreements and liberalization of international markets, which took place in the seventies in the USA and in the nineties in Europe. This work addresses specific and general effects of deregulation and the development of air transport industry and aims to identify and analyze changes of its structures in space and time. One of the major and direct impacts and consequences of deregulation is that the free market allowed the emergence of new low-cost airlines, which significantly changed the structure and trends of air transport. The changes allowed for greater mobility of people, goods and capital, and also caused a partial change in consumer behaviour of individuals and institutions, and even more interconnected global economy. The source of data and information are primarily articles in professional journals written in English. Knowledge from particular analysis and research approaches to air traffic from the first part of this work is confronted with empirical research.
