UNIVERZITA KARLOVA Lékařská fakulta v Plzni Dizertační práce Endoteliální glykokalyx - možnosti diagnostiky a intervence MUDr.Jiří Pouska ABSTRAKT Endoteliální glykokalyx (EG) je tenká vrstvička na povrchu endotelu. Její studium v posledních letech vedlo k formulaci Revidovaného Starlingova principu, zpřesňujícího popis chování tekutin v kapilárním řečišti. EG má zásadní vliv na polopropustnost endotelu a tedy pro udržení tekutiny v krevním řečišti a snížení míry filtrace na kapiláře. Stanovit integritu EG je klinicky in vivo obtížné, v současné době jsou dostupné laboratorní a zobrazovací metody stanovení. Do dnešní doby bylo prokázáno několik patologických stavů, které vedou k poškození EG (např. sepse). U mnoha těchto stavů hraje nedílnou součást léčby tekutinová terapie. V rámci našeho výzkumu jsem se zaměřili na stanovení změn EG v závislosti na různém způsobu podání tekutiny a jejím různém složení a změn EG způsobených různými patologickými stavy jako sepse nebo celková anestezie. After extensive research in past years, revisited Starling principle was finally formulated. It describes fluid physiology in capillaries precisely. EG has pivotal role in keeping endothelium semipermeable and thus avoiding extensive filtration of fluids to interstitium. Assessment of EG is clinically difficult. Many pathological conditions lead to damage of EG (sepsis etc.). Intravenous fluid therapy is mainstay of treatment of such conditions. Our aim was to determine the changes of EG integrity depending on the choice of intravenous fluid and its infusion time in physiological and pathological conditions. Key words: Endothelial glycocalyx, infusion therapy, anaesthesia, sepsis, microcirculation.
