Souhrn ÚVOD: Mléko a mléčné bílkoviny mají vyšší inzulinogenní index než index glykemický. Tato vlastnost je přisuzována především syrovátkovým bílkovinám. Cílem práce bylo zhodnotit C-peptidovou odpověď po podání syrovátkových bílkovin u zdravých jedinců (studie 1) a u pacientů s diabetes mellitus 2. typu (studie 2) ve srovnání s ostatními často experimentálně užívanými stimulačními testy. METODOLOGIE: Do studie 1 bylo zařazeno osm zdravých neobézních jedinců (BMI 17 - 25,9 kg/mš) ve věkovém rozmezí 20 až 26 let. Každý jedinec podstoupil celkem šest testů C-peptidové sekrece v průběhu jednoho měsíce. Tři testy byly perorální s podáním 1) samotné glukózy (OGTT - reference pro perorální testy), 2) samotných syrovátkových bílkovin (OWT) a 3) glukózy se syrovátkovými bílkovinami (OWGT). Zbylé tři testy byly intravenózní s podáním: 1) samotné glukózy (IVGTT - reference pro intravenózní testy), 2) argininu (AST) a 3) glukagonu (GST). Do studie 2 bylo zařazeno šestnáct neobézních (BMI 26,4 - 29,8 kg/m2 ) pacientů s diabetes mellitus 2. typu s uspokojivou kompenzací a se zachovalou inzulínovou sekrecí (průměrný lačný C-peptid byl 0,94 ± 0, 07 nmol/l; x̅ ± SEM) ve věkovém rozmezí 56 až 80 let. Každý jedinec podstoupil celkem dva testy perorální s odstupem jednoho měsíce: test s podáním 1) samotné glukózy (OGTT -... INTRODUCTION: Milk and milk proteins have higher insulinogenic index than glycemic index. This feature is mainly attributed to whey proteins. The main goal of this thesis was to evaluate a C-peptide response after administration of whey proteins in healthy individuals (study 1) and in patients with type 2 diabetes mellitus (study 2) in comparison to other experimental stimulation tests. METHODS: Eight young, healthy (7 males, 1 female; aged 20-26 years), non-obese (BMI: 17 - 25.9 kg/mš) participants with normal glucose tolerance were enrolled for study 1. Each individual underwent six C-peptide secretion tests in total. Three secretion tests measured C-peptide response to orally administered substances: whey proteins only (OWT), whey proteins with glucose (OWGT) and glucose only (OGTT, reference); while the other three secretion tests measured C-peptide response to intravenously administered substances: arginine (AST), glucagon (GST) and glucose (IVGTT, reference). Sixteen overweight (BMI 26,4 - 29,8 kg/m2) patients with type 2 diabetes mellitus, good glycemic control and with preserved fasting serum C-peptide levels (0,94 ± 0, 07 nmol/l) were enrolled for study 2.
