Hypochondroplázie (MIM 146000) je skeletální dysplázie, která se projevuje různě závažným disproporčním malým vzrůstem s rhizomelickým nebo mesomelickým zkrácením horních i dolních končetin. Pacienti mají také často makrocefalii s normálními obličejovými rysy. Hypochondroplázie je onemocnění s autozomálně dominantní dědičností. U části nemocných je příčinou onemocnění zárodečná mutace v genu FGFR3, u části nemocných však příčina onemocnění zůstává stále neznámá. Gen FGFR3 kóduje tyrosin kinázový receptor, který negativně reguluje přeměnu chrupavky na kost. Mutace v genu FGFR3, které jsou příčinou hypochondroplázie, vedou ke konstitutivní aktivaci receptoru a tím inhibici růstu dlouhých kostí. V této práci byly molekulárně genetickými metodami analyzovány vybrané oblasti genu FGFR3 u 98 pacientů s disproporčním malým vzrůstem a klinickou diagnózou hypochondroplázie, z nich bylo 18 nemocných z 12 rodin s familiárním výskytem onemocnění a 80 bylo sporadických případů. U všech zařazených pacientů byla již dříve standardním molekulárně genetickým vyšetřením vyloučena přítomnost častých rekurentních zárodečných mutací v exonu 13 (v kodonu 540) a v exonu 15 (v kodonu 650). Genomová DNA pacientů byla izolována z leukocytů periferní krve, vyšetření bylo provedeno s informovaným souhlasem pacienta nebo jeho... Hypochondroplasia (MIM 146000) is a skeletal dysplasia characterized by disproportional dwarfism with rhizomelic or mesomelic shortening of the upper and lower extremities, with variable severity. Patients often have macrocephaly with normal facial features. Hypochondroplasia is a disease with autosomal dominant inheritance. In some patients it is caused by germline mutations in the FGFR3 gene, in others the cause of the disease remains unknown . The FGFR3 gene encodes a tyrosine kinase receptor. This receptor negatively regulates the conversion of cartilage to bone. FGFR3 gene mutations that cause hypochondroplasia lead to constitutive activation of the receptor and inhibit the growth of long bones. In this study, we analysed selected regions (exons) of the FGFR3 gene in 98 patients with disproportional dwarfism and clinical diagnosis of hypochondroplasia. Eighteen patients from 12 families had familial and 80 patients had sporadic form of the disease. All patients were previously tested negative for frequent germline mutations in exon 13 (codon 540) and exon 15 (codon 650). Genomic DNA was isolated from patient's peripheral blood leukocytes.
