[cze][eng] SOUHRN ÚVOD. V posledních dvou dekádách došlo, především u regurgitačních vad, k rozvoji záchovných operací neboli plastik srdečních chlopní. Záchovné operace aortální chlopně se rozvíjejí spíše v posledním desetiletí, zatímco výhoda plastik mitrální chlopně oproti náhradě je již delší dobu jednoznačně prokázána. Obecně platí, že záchovné operace s sebou nesou prokazatelné výhody plynoucí z absence rizik spojených s implantací umělé, ať mechanické či biologické chlopně. Nevýhodou záchovných operací srdečních chlopní, především té aortální, je jejich vyšší technická náročnost, riziko reziduální regurgitace a zejména riziko selhání plastiky s nutností následné reoperace. CÍL. Cílem práce je zpracování a vyhodnocení krátko- a střednědobých výsledků záchovných operací aortální chlopně u pacientů s aortální insuficiencí, zkoumání vlivu typu aortální chlopně (bikuspidní nebo trikuspidní) a typu záchovné operace (bez a s výkonem na aortálním kořeni) na krátko- a střednědobé výsledky se zaměřením na přežívání, reoperaci aortální chlopně a recidivu/progresi aortální insuficience. Dalším cílem je zhodnocení efektivity a pulzatility externí aortální anuloplastiky prstencem na malé podskupině pacientů pomocí kardio-CT. METODIKA. Od listopadu 2007 do října 2012 byla u 100 pacientů s aortální regurgitací provedena...INTRODUCTION. In the last two decades, the vlave-conserving surgery of regurgitant heart valves has been evolving. The aortic valve-sparing operations have been developing rather over the last decade, despite the clear benefit of mitral valve repair has been shown. There is an indisputable advantage of valve-sparing operations attributed to the elimination of certain risks related to aortic valve replacement. Conversely, aortic valve repair is technically more demanding and there is a substantial risk of repair failure. AIM: The aim of this study is to analyze and assess short- and mid-term results of aortic valve-sparing operations in patients with aortic regurgitation, to examine the impact of aortic valve cuspidity (bicuspid or tricuspid) and different type of aortic repair (without and with root replacement) on the short- and mid-term results with regards to survival, reoperation and recurrence of aortic regurgitation (AR). Another aim is to assess the effectiveness and pulsatility of the external aortic ring annuloplasty in the patients' subgroup using cardiac-CT. METHODS.
