Před více jak 35 lety (1978) se ve Velké Británii narodila Louise Brownová, první dítě na světě počaté ve zkumavce - in vitro (IVF - in vitro fertilization). Předpokládá se, že do současné doby se narodilo s pomocí IVF již více než 4 000 000 dětí. První snahy o oplození měly úspěšnost kolem 20%, dnes je ale úspěšnost až dvojnásobná (40%). To je výsledkem zavádění nových metod, standardizace manipulačních a kultivačních médií a v neposlední řadě i získáním řady nových poznatků. I přes zavádění nových metod je základním předpokladem úspěchu IVF profesionální práce pracovníků IVF klinik a IVF laboratoře, jejich znalosti v dané oblasti a schopnost posouzení kvality biologického materiálu. Permanentní trénink na nevyužitém biologickém materiálu (nezralé a méně kvalitní oocyty) a jeho využití k výzkumu, by měly být pak základním předpokladem k tomu, aby příslušná IVF centra mohla zavádět další nové progresivní postupy a zkvalitňovat léčbu infertility. I to je navíc předpokladem pro to, aby bylo možné zodpovědět některé otázky z asistované reprodukce, které nejsou dosud zcela jasně vysvětleny. V prvním okruhu jsme se zaměřili na posouzení kvality oocytů, které byly získány aspirací folikulů po stimulaci vaječníků pacientek gonadotropiny. Pro značení protilátkami proti acetylovaným histonům byly použity... It is now more than 35 years since the first world test-tube baby, Louise Brown, was born (1978) in England and it is estimated that since then more than 4 000 000 of children were produced by in vitro fertilization (IVF) worldwide. The initial success of IVF was less than 20% in best clinics, but now it reaches about 40%. This is a consequence of introduction of new methods, standardization and exploitation of new manipulation and culture media, as well as the incorporation of research results. Nevertheless, the most important still remains the skill and experience of IVF clinics and IVF laboratories staff, especially their ability to critically evaluate the quality of biological material and to decide which cure and treatment are the best one. At least, some biological material (immature and low quality oocytes) can be used for training and also for some experiments aiming to explain some questions, which are not yet fully understood (for example aneuploidies in human oocytes and embryos). In addition, this training can facilitate the introduction of new progressive approaches and may also improve indirectly the quality of infertility treatments.
