SOUHRN V rámci studia doktorského studijního programu byla navržena, vypracována a do klinického provozu laboratorní diagnostiky akutní formy leptospirózy zavedena real-time PCR metoda založená na detekci genu kódujícího povrchový lipoprotein LipL32. Metoda vykázala velmi dobrou analytickou pozitivní i negativní specificitu, kdy se obě hodnoty rovnaly 100 %. Na 230 laboratorních kmenech leptospir ze sbírkových kultur Royal Tropical Institute v Holandsku byla real-time PCR odzkoušena. Mez detekce byla stanovena na 1 - 5 kopií genomu / ml tekutého biologického materiálu. V praxi byla metoda ověřena na biologických materiálech 295 pacientů se suspektní leptospirózou, vyšetřených v období duben 2010 - duben 2013. Devět osob (3,1 %) a jim přináležejících 15 (3,2 %) biologických materiálů bylo vyhodnoceno jako LipL32 pozitivní. Real-time PCR byla použita při analýze vzorků z vnějšího prostředí na přítomnost leptospir. Z celkového počtu 680 vzorků (povrchové vody, čistírny odpadních vod, vlhké substráty) bylo 5 real-time PCR reakcí (0,7 %) vyhodnoceno jako hraniční, ovšem výsledek jejich sekvenační analýzy byl opakovaně "nekultivovatelné bakterie". Všechny vzorky byly vyhodnoceny jako negativní. Z uvedené skutečnosti je zřejmé, že metoda není vhodná pro testování takto náročných materiálů Multilokusová sekvenační... SUMMMARY Title: "Molecular biological methods in laboratory diagnosis of pathogenic leptospires" During Ph.D. study programe real-time PCR method based on detection of gene encoding surface lipoprotein LipL32 was designed, devised and introduced into clinical laboratory practice of laboratory diagnosis of acute form of leptospirosis. Positive and negative analytical specificity 100 % in both cases was defined and limit of detection in range 1 - 5 copies of genome / ml of liquid biological material was determined. Real-time PCR method was in clinical practice on biological materials gained in period April 2010 - April 2013 from 295 patients suspicious on leptospirosis verified. Total number of 9 persons from whom 15 biological materials originated as LipL32 positive were evaluated. Real-time PCR was tested during the analysis of environmental samples for the presence of Leptospira. From 680 samples (surface water, waste water, wet substrates) were 5 real-time PCR reactions (0.7%) evaluated as borderline, results sequence analysis were repeatedly "uncultivable bacteria." All samples were as negative evaluated. From mention is clear that this method is not suitable for testing these materials.
