Huntingtonova nemoc (HN) je autozomálně dominantně dědičné neuropsychiatrické onemocnění s obvyklým počátkem ve středním věku. Mutací ležící na krátkém raménku 4.chromozomu je expanze tripletu obsahujícího cytosin-adenin-guanin (CAG) s kritickou hranicí 40 a více repeticí. Hlavními projevy jsou motorické poruchy (chorea, dystonie, poruchy volní hybnosti), postupná kognitivní deteriorace a neuropsychiatrické symptomy (poruchy chování, afektivní symptomy aj.). Potvrzení klinické diagnózy je prováděno genetickým testem, který je možné provést i presymptomaticky u potomků nemocné osoby. Cílem 1. studie bylo charakterizovat rozdíly v psychiatrickém vyšetření a neuropsychologickém testování mezi osobami v riziku (PAR - people at risk), u kterých bylo doporučeno oddálit test a osobami v riziku, kterým bylo doporučeno pokračovat v tzv. prediktivním protokolu. Bylo vyšetřeno 52 osob (32 žen, 20 mužů). Mimo běžného psychiatrického vyšetření jsme administrovali Eysenckův osobnostní dotazník (EOD-A), sebehodnotící škálu všeobecné psychopatologie (SCL-90), tři krátké kognitivní testy - test cesty (TMT-A,B), test verbální fluence (VF) a paměťový test učení (AVLT) a dotazník kvality života (MANSA). PAR s doporučeným odkladem testu vykazovali statisticky signifikantně větší míru neuroticismu a... Huntington's disease (HD) is an autosomal dominant inherited neuro-psychiatric disease with usual onset in the middle age. The mutation, located on the short shoulder of chromosome 4, is an expansion of a nucleotide triplet, containing cytosine, adenine, guanine (CAG), with critical limit of 40+ repetitions. The principal symptoms include motor symptoms (chorea, dystonia, disorders of voluntary movements), progressive cognitive deterioration and neuropsychiatric symptoms (behaviour disorders, affective symptoms and so on). The clinical diagnosis is confirmed by a genetic test, which may also be carried out presymptomatically in offsprings of the diseased person. The objective of the 1st study consisted in the characterization of differences in psychiatric examination and neuropsychological testing among the people at risk (PAR), in whom it was recommended to delay the test, and people at risk, who were recommended to continue in the so-called predictive protocol. The total of 52 people have been examined (32 females, 20 males).
