Šindlerová, Barbora ; Kozáková, Alena (advisor) ; Novotná, Irena (referee)
Title: Case Report: Physiotherapeutic Treatment of a Patient Diagnosed with a Bimalleolar Ankle Fracture Thesis goal: The goal of this thesis is to present a theoretical outline of a bimalleolar ankle fracture diagnosis and a practical survey of the respective conservative medical treatment. The material and evidence required for these were gathered during a practical training experience in Kladno hospital. Methodology: The theoretical part of this thesis includes theoretical matter from the fields of anatomy, kinesiology, biomechanics, traumatology, and types of physiotherapeutic treatment of bimalleolar ankle fracture. The following special part of this thesis discusses the methodology, collects anamnesis data and includes the initial kinesiologic check-up on the basis of which a suitable therapy and a short-term physiotherapeutic schedule was designed. This part of the thesis further consists of an account of individual therapeutic procedures and of the final kinesiologic check-up which is compared to the initial check-up. In conclusion, there follows an argumentation of the efficiency of the chosen physiotherapeutic methods. Key words: physiotherapy, case report, ankle joint, fracture

The hip fracture register, optimalization of hip hemiarthroplasty position
Skála - Rosenbaum, Jiří ; Bartoníček, Jan (advisor) ; Chomiak, Jiří (referee) ; Janeček, Miloš (referee)
The study examines the monocentric epidemiological data of patients with hip fractures from 1997-2007. Adult patients treated for hip fracture between the years 1997-2007 were included in the study. Retrospective statistical assessment of continually gathered data focused on epidemiology and demographics. The second part of the study was concerned on risk factors on protrusion of hemiarthroplasty into the acetabulum. The goal was to specify exact indication, proper operative technique and selection of the implant. The study involved 3 683 patients (2 678 women, 1 005 men). Patients older than 70 years accounted for 82 % of all cases. There was an increase in the number of hip fractures resulting in a significant increase in pertrochanteric fractures (AO-31A1+2) (p<0.001). The ratio of trochanteric to neck hip fractures increased from 0.99 to 1.53. Continual monitoring of patients with hip fracture offers data which allows comparisons between regions and countries. The importace of a continual increase in the number of hip fractures has its importance in macroeconomical planning in traumatology. Hemiarthroplasty was conversed to total hip arthroplasty in 12 patients in our department. The indication, time period to conversion and hemiarthroplasty position were retrospectively assessed. After spinal cord injury occur a lot of complication, with resulting from neurological picture. These can be central, peripheral or mix paresis in some cases even plegie. Often an autonomic nervous system can be demaged, causing complication practice in nearly all body systems. Rehabilitation attend belongs to basic care of paretic patients and is closely connected with basic with basic nursing methods from which the physiotherapy of these patients arise. This survey gives a summary of procedures and methods of physiotherapy , which help the patient to prevent and better respond to the complication treatment e.g. on respiratory and movement apparatus. Complex physiotherapeutic treatment concentrates mainly on prevention of complications. The physiotherapist must anticipate formation of complications and adapt the comlexity of the treatment. The diploma thesis deals with problems of depression in older people. Mainly the work is focused on identifying and analyzing the role of nurses in screening for depression in older people in primary care, acute care, long-term care and home care. This thesis was focused on theoretical direction and was used the method of design and demonstration. In this thesis was set one main goals with five research questions. The main goal was to identify and analyze the role of nurses in screening for depression in the elderly. RQ 1: What is the role of the nurse in screening for depression in the elderly? RQ 2: What is the role of the nurse in the primary care in screening for depression in the elderly? RQ 3: What is the role of the nurse in screening for depression in hospitalized patients in acute care? RQ 4: What is the role of the nurse in screening for depression in seniors in long-term and home care? RQ 5: What rating scales and methods are used in screening for depression in the elderly? The thesis introduce the concept of depression. The following are specified the causes of and the important factors that affect depression in the elderly. It also deals the differences in the clinical symptomatology of depression in old age. It explains possibilities and various barriers in the diagnosis of depression. Another chapter introduces complete geriatric examination, diagnostic classification systems, possible screening methods and scales for detection of depression in the elderly population. It also deals methods of pharmacological and non-pharmacological treatment and its possible complications associated with older age. By reason of increased suicide rate caused by depressive disorder the issue of suicidal behavior in the elderly is introduced. The next chapter deals with the nursing process, which is used by nurses in practice. It consists of the evaluation of the patient's health condition, making nursing diagnosis, creating nursing plan and subsequent implementation and evaluation. The nursing process is also needy for providing quality care. The nursing process in the stage of nursing diagnosis, introduces possible nursing diagnosis for a patient suffering from depression, which are based on the latest classification. Finally is described the role of nurses in screening for depression in the elderly in different health facilities and their contribution to the timely evaluation of depression in the elderly. This chapter introduces the role of nurses, nursing screening and collaboration with a physician. The role of nurses in screening for depression in different medical facilities is based on the first phase of the nursing process of assessment. On the basis of objective and subjective information, the nurse will assess the overall health and mental condition of the patient. Primarily, it was investigated what is the role of the nurse in screening for depression. On the basis of content analysis and synthesis it was necessary to used and processed domestic and foreign literature. A number of relevant sources are the results of various studies and Meta-analyzes, mostly from abroad, but also from the Czech Republic. The thesis can serve as a basis for nurses. Jokschová, Lucie ; Mareš, Jiří (advisor) ; Prchalová, Eva (referee) ; Šauer, Karel (referee)
The thesis deals with nursing care of a geriatric patient who suffers from chronic pain and is based on patients' opinions. The theoretical part defines the term "chronic pain" and characterizes its' causes. It also describes various ways of diagnosing chronic pain, ways in which chronic pain can be monitored and how could chronic pain be treated. There is a general nursing plan - according to the model of M.Gordonova - for a geriatric patient with chronic pain treatment worked out at the other part of the thesis. The empiric part of the work deals with patients' opinions. For the research questionnaire was used as the most suitable method for given purpose. 54 geriatric patients suffering from chronic pain, who are hospitalized either in internal department or department for the elderly in Havlíčkův Brod, took part in the research. They were then dividend into 2 groups - men and women - which was also taken into account when evaluating the answers. The questionnaire is focused on character of pain, its' location and intesity with regard to previous week. The target is to find out what patient's preferences in chronic pain treatment are and the level to which daily activities were influenced by pain over the last week. The work results confirmed that the way old people go through and cope with...
