Michaela Stupková Tittle: Case Study of Physiotherapeutic Treatment of a Patient after total knee replacement Objectives: The aim of this bachelor's thesis is to summarize the theoretical knowledge of the issue of total knee replacement and to process the case report of a patient who underwent this surgery. Methods: The bachelor's thesis consists of two parts - general and special. The theoretical part summarizes the knowledge about kinesiology and biomechanics of the knee joint, gonarthrosis, knee replacements, physiotherapeutic care and physiotherapeutic methods. The special part includes a case report of a physiotherapeutic care of a patient after total knee replacement. The study was carried out as part of a clinical work placement in January and February 2024 at the Poliklinika AGEL Praha Italská. It consists of a kinesiology initial examination, a description of all therapeutic sessions, an exit kinesiology examination and an evaluation of the therapy effect. Non-invasive physiotherapeutic methods and procedures were used for the examination and the therapy within the scope of knowledge of the bachelor's degree in physiotherapy at the Faculty of Physical Education and Sport at Charles University. Objectives: The aim of this bachelor thesis is to gain knowledge about the diagnosis of TEP of the knee joint and the most common indication for this procedure. Furthermore, the aim was to prepare a case report of a patient with this diagnosis and to evaluate the procedures used and the results of the examination Methods:The case study report includes an initial and an initial kinesiological analysis, eight therapeutics units, the objectives of the short- and long-term therapeutic pla and the design of the therapy. The use of suspension systems for physiotherapy in patients after TEP of the knee joint

This bachelor thesis focuses on the utilization of suspension systems post total knee replacement. The aim of the thesis was to explore the possibilities of physiotherapy using suspension systems in patients after total knee replacement, propose an exercise unit within the Redcord system, and evaluate how exercising in this system influenced individuals' conditions. The thesis is divided into theoretical and practical parts. In the theoretical part, I summarize knowledge about the anatomy and kinesiology of the knee joint. Additionally, I describe osteoarthritis and its surgical solution - total knee replacement. Finally, in the theoretical part, I mention the possibilities of rehabilitation after total knee replacement and information about the Redcord suspension system. The practical part was conducted in the form of qualitative research using case studies. Two patients aged 55 and 70 after total knee replacement participated in the research. Each case study consists of a kinesiological analysis, including anamnesis, examination of scar, standing and walking, anthropometry, goniometry, muscle testing, and examination of the initial position in the Redcord system. It also included descriptions of six therapies and proposals for shor - term and long - term rehabilitation plans. An initial and final kinesiological analysis was conducted for each case study, which was subsequently compared. The results show that patients experienced increased muscle strength, improved range of motion, increased overal fitness, and strengthened deep stabilization system. Therefore, exercising in the Redcord system has a positive impact on the condition of patients after total knee replacement. This bachelor thesis can serve as a useful source of information for therapists or clients interested in utilizing Redcord for exercise.
Biomechanical studies of total knee replacement and alternatives to the implantation

It contains an overview of the total knee replacement currently produced in the Czech republic and abroad. The description of the strain stress analysis of the tibial component TKR, which was made using the ANSYS software, is part of thesis.
Lubrication of knee joint replacement

The work deals with the lubrication of total knee replacement using fluorescence microscopy method, which allows unique insight into the contact between femoral and tibial component. The aim was to determine the effect of composition of synovial fluid (i.e. albumin, -globulin, hyaluronic acid and phospholipids) on film thickness and protein behaviour in contact, and also to determine changes of contact area during rotation. Since this is the first experimental work dealing with a knee replacement lubrication primarily, only simple rotation and load cycles were applied by the knee simulator. The output of the experiments was fluorescence intensity, which corresponds to dimensionless film thickness, over time. Another important output are the images directly showing the fluorescently labelled proteins in the contact area. The results show, there are fundamental differences in lubrication in different positions of rotation, due to changes of position, shape and behaviour of the contact area. The composition of the lubricant is also essential, since the proteins themselves form a relatively strong lubricating film and their mixture leads to a substantial reduction of film thickness, due to significant formation of clusters. Complex fluid, although it does not form the strongest layer, is able to create a quite continuous film.
Advanced production technology for joint implants made by the EBM method

Tato práce se zabývá pokročilou technologií výroby personalizovaných kloubních implantátů metodou EBM za použití titanové slitiny Ti6Al4V-ELI a navrhuje nový unikátní design kolenního implantátu společně s metodologií jeho inserce, přičemž tato řešení jsou součástí patentové přihlášky č. PV 2020-459. Toto neinvazivní řešení náhrady kolenního kloubu je šetrnější k pacientovi, maximálně chrání jeho zdravé tkáně a kosti, navíc se dá předpokládat vyšší životnost implantátu ve srovnání s tradičními dostupnými řešeními. Byla uskutečněna výroba vzorků z materiálu Ti6Al4V-ELI metodou EBM, proveden rozbor jejich materiálových, mechanických, technologických a únavových vlastností. Dále byly popsány pokročilé metody zobrazování, úpravy a tvorby kloubních ploch a použity k vyvinutí nového designu personalizovaného kloubního implantátu společně s inovační technologií jeho inserce a nástroji potřebnými k její úspěšné realizaci. Toto nové řešení bylo úspěšně ověřeno mnoha testy i výrobou Ti6Al4V-ELI a CoCrMo prototypů implantátů metodou EBM. Proveditelnost a použití v praxi bylo konzultováno a schváleno odborníky v této oblasti.

Title: Case report on physiotherapeutic care of a patient with total left knee replacement on the left side caused by the gonarthrosis. Summary: This thesis focuses on case report of a patient after total left knee replacement. The work contains two parts - the general part and the specialized part. The general part deals with structure, kinesiology and biomechanics of the knee joint, general definition of gonarthrosis, its cause, clinical symptoms and its treatment. It also describes the gonarthrosis, indications and the types of total knee replacement prosthesis. The most important topics of the general part contain descriptions of therapeutic methods and procedures, focused on improving patient's health in the post- operation phase. The specialized part constitutes the main part of this thesis. It is processed via case report form and describes the physiotherapeutic treatment of a specific patient, who underwent a total knee replacement surgery. In the conclusion of the specialized part of the thesis, I evaluate the effects of the theraphy. iv Abstract Author: Adam Viktora Supervisor: PhDr. Lenka Žáková, Ph.D. Title of the thesis: Case study of physiotherapy treatment of a patient after Total Knee Replacement Objectives: The goal of this thesis is to gather and understand the theoretical knowledge of the knee joint, osteoarthritis, total knee replacement surgery and its rehabilitation, and then evaluate the therapeutic sessions that were applied. Clinical findings: This study evaluated the condition of a 73-year-old patient after total knee replacement of the right knee. The examination showed a reduced range of motion, reduced mobility of joints, muscle imbalances and differences in conditions of soft tissues. Methods: All methods used for this study were based on the knowledge learned and the literature provided by the Faculty of Physical Education and Sports, Charles University. Results: Following the eight therapeutic sessions, the patient had progressed well overall. Progress was seen in terms of pain, range of motion, muscle imbalances and condition of soft tissues. Conclusion: The therapies applied were effective and valuable in this specific diagnosis of the patient.
