The role of the nurse in providing multidisciplinary care in spinal units

The aim of this master's thesis is to map the role of the nurse in the spinal unit, describe the collaboration of multidisciplinary team members in the spinal unit and describe the needs of the hospitalized patients. To meet these objectives, a total of five following research questions were set: "What is the role of the nurse in the care of patients in the spinal unit?", "What are the specifics of nursing care of patients in the spinal unit?", "What is the composition of the multidisciplinary team in the spinal unit?", "How does the cooperation between these workers take place?" and "What are the needs of patients hospitalized in the spinal unit?". The theoretical part of the thesis is designed to provide the reader with a comprehensive overview of the issue of spinal cord injury. At the very beginning, it defines the basic anatomical and physiological conditions of the nervous system, spine and spinal cord. Injury of these structures is the subject of other chapters, which discuss the causes, symptoms, diagnosis and treatment options in more detail. It also describes the development of spinal units in the Czech Republic and abroad. It defines a multidisciplinary team, whose work is an integral part of the care of a patient with a spinal cord injury. The nursing care chapter is devoted to areas that require a specific approach by nurses in patients with spinal cord injury. A qualitative research study was chosen for the empirical part of the thesis. Data collection took place using a semi-structured interview with thirteen nurses from selected spinal units. After finding out their personal information, they were asked a total of twenty-nine questions focused on the functioning of the spinal unit, characteristics of provided multidisciplinary care and the nurses' experience of the patients' condition and needs. The obtained information was subsequently transcribed and processed with the open coding method and colouring text. For easier orientation in the results of the interviews, four categories and seventeen subcategories were created, which clearly present the answers to the above research questions. It was found that the informants perceive their role in the spinal unit mainly as a nurse-caregiver. The cooperation of the multidisciplinary team is conditioned by various forms of verbal and non-verbal information exchange, for example during ward rounds, by change-of-shift handoff, operational meetings or information boards. Most of the nurses rate this cooperation as effective and mostly problem-free. The results also show that the needs of patients are closely related to mobility restrictions and long-term hospitalization. Most often, these are physiological needs and the need for contact with the society. This master's thesis points out the importance of a multidisciplinary and sensitive approach to patients with a spinal cord injury, who experience many worries and tense moments in their return to everyday life. The results may serve as educational material not just for students of healthcare professions, but also for the medical staff themselves. An output of the thesis is a manual for new nurses on spinal units, which aims to make them easier to integrate and prepare for the significant specifics about the nursing care for patients with spinal cord injury.
Nursing care of patient with quadriplegia

The bachelor thesis focuses on nursing care of a quadruplegic patient. The theoretical part describes anatomy and physiology of spine and spinal cord, characteristics of disability, first aid, etiology, symptomatology, diagnostics, therapy, complications and nursing care. The sources for an elaboration of the theoretical part were found in the National Medical Library, via Medvik, on Wikiscriptions, on the thesis repositories and on the Spinal Unit website. For a larger amount of resources, research was made in the National Medical Library based on keywords. For the elaboration of the practical part, a qualitative research method was chosen in the form of a case study of a patient with quadruplegia. Case study includes information from medical records, interviews with the patient and observations. The main goal of the bachelor thesis is to process a case study, which includes a nursing plan of a selected patient with all the specifics, that the care for the quadruplegic patient brings us. The second goal is to raise the awareness of the care provided on the spinal unit amongst general public and healthcare commmunity. The utilization of self-care instructor's activity in the spinal unit from the staff and patient's view.

The theme of the Bachelor thesis is: " Utilization of the work of an self-sufficiency instructor at the spinal unit of the Regional Hospital Liberec ". The object of the research is the work of a physically disabled self-sufficiency instructo r at the spinal unit. The aim of this thesis is a summary of information about work activities of a self-sufficiency instructor and a survey how the nursing staff and patients perceive his or her work . To address the issue by qualitative research, these methods are chosen: brainstorming, brainwriting , SWOT realized using a semi-standardized interview. Sub-objectives result from the chosen method of SWOT and they deal with determining the strengths and the weaknesses, as well as evaluating the potential opportunities and threats that affect the self-sufficiency instructor's work. The total number of participating respondents is 26 people. The research was attended by 11 persons from the nursing section group, 8 patients and 7 respondents from the group of physiotherapists and ergotherapists . The difference in nursing care between patients with paraplegia and quadriplegia.

Consequences of spinal cord impairment with subsequent paralysis have adverse influence on practically all aspects of patient?s life. A patient with paraplegia or quadriplegia is not only limited in terms of motor or sensory loss in some parts of the body, but is often threatened by various health complications and mental problems. The number of such complications in patients remarkably decreases with improvement of treatment and nursing care. The aim of the Bachelor Thesis was to find differences in nursing care for patients with paraplegia and quadriplegia at spinal units and surgery wards, then an assessment of differences in nursing care for paraplegic and quadriplegic patients. The research part of the Thesis was based on quantitative examination in the form of an anonymous questionnaire and quantitative data collection method by means of indifferent concealed observation was moreover used for complementation of the answers of nurses working at the spinal unit. The research sample for the monitoring consisted of 32 patients, 16 of whom with paraplegia and 16 with quadriplegia, from the Spinal Unit of the Casualty Hospital Brno and the Faculty Hospital Brno. The anonymous questionnaire research was performed at spinal units and casualty wards in the Czech Republic. The questionnaire consisted of 30 questions. 133 questionnaires were distributed, 78,9 % returned. There were five hypotheses set in total. The first hypothesis: Nursing care for paraplegic and quadriplegic patients at a spinal unit is mainly focused on the support of self-sufficiency and activation. The hypothesis was confirmed. The second hypothesis: Nursing care for paraplegic and quadriplegic patients at surgical wards is focused on satisfying the basic needs. The results confirmed this hypothesis as well. The third hypothesis: The risk of immobilization syndrome occurrence is higher in quadriplegic patients than in paraplegic ones from the point of view of nurses. This hypothesis was confirmed by the research. The fourth hypothesis: The risk of infection occurrence is higher in quadriplegic patients than in paraplegic ones from the point of view of nurses. In this case the hypothesis was not confirmed. The fifth hypothesis: The risk of mental disorder occurrence is higher in quadriplegic patients than in paraplegic ones from the point of view of nurses. The results obtained did not confirm this hypothesis. The set goals of the Thesis were met upon confirmation of three hypotheses of five. The Bachelor Thesis may be used as studying material for students of the South Bohemian University, Faculty of Health & Social Studies, for healthcare staff involved in caring for paraplegic and quadriplegic patients.
