[cze][eng] Poškození míchy s následným ochrnutím svými důsledky nepříznivě ovlivní prakticky všechny oblasti života pacienta. Pacient s paraplegií nebo kvadruplegií nemá jen omezení ve smyslu ztráty hybnosti či citlivosti na určitých částech těla, ale často je ohrožen nejrůznějšími zdravotními komplikacemi a psychickými problémy. Se zkvalitněním léčebné a ošetřovatelské péče počet těchto komplikací u pacientů po poranění páteře významně klesá. Cílem bakalářské práce bylo zjistit rozdíly v ošetřovatelské péči u pacientů s paraplegií a kvadruplegií na spinálních jednotkách a na chirurgických odděleních, dále posouzení rozdílů ošetřovatelské péče u pacientů s paraplegií a kvadruplegií. Výzkumná část této práce byla vytvořena pomocí kvantitativního šetření formou anonymního dotazníku a k doplnění odpovědí sester pracujících na spinální jednotce byla ještě použita kvantitativní metoda sběru dat pomocí nezúčastněného strukturovaného skrytého pozorování. Výzkumný soubor metody pozorování tvořilo 32 pacientů z toho 16 pacientů s paraplegií a 16 pacientů s kvadruplegií ze Spinální jednotky Úrazové nemocnice Brno a Fakultní nemocnice Brno. Výzkumné šetření formou anonymního dotazníku bylo prováděno na spinálních jednotkách a na chirurgických odděleních v České republice. Dotazník obsahoval celkem 30 otázek. Bylo rozdáno 133 dotazníků, jejich návratnost byla 78,9 %. Celkem bylo stanoveno pět hypotéz. První hypotéza: Na spinální jednotce se zaměřuje ošetřovatelská péče u pacientů s paraplegií a kvadruplegií hlavně na podporu soběstačnosti a na aktivizaci. Tato hypotéza se potvrdila. Druhá hypotéza: Na chirurgických odděleních je ošetřovatelská péče u pacientů s paraplegií a kvadruplegií zaměřena na uspokojování základních potřeb. Výsledky byla tato hypotéza také potvrzena. Třetí hypotéza: U pacientů s kvadruplegií je z pohledu sester větší riziko vzniku imobilizačního syndromu než u pacientů s paraplegií. Šetřením byla tato hypotéza potvrzena. Čtvrtá hypotéza: U pacientů s kvadruplegií je z pohledu sester větší riziko vzniku infekce než u pacientů s paraplegií. V tomto případě nebyla hypotéza potvrzena. Pátá hypotéza: U pacientů s kvadruplegií je z pohledu sester větší riziko vzniku psychického onemocnění než u pacientů s paraplegií. Získané výsledky tuto poslední hypotézu nepotvrdily. Stanovené cíle práce byly splněny na základě potvrzení tří hypotéz z pěti stanovených. Bakalářská práce může sloužit jako studijní materiál pro studenty Jihočeské univerzity Zdravotně sociální fakulty a také pro zdravotníky přicházející do styku s pacienty s paraplegií a kvadruplegií.Consequences of spinal cord impairment with subsequent paralysis have adverse influence on practically all aspects of patient?s life. A patient with paraplegia or quadriplegia is not only limited in terms of motor or sensory loss in some parts of the body, but is often threatened by various health complications and mental problems. The number of such complications in patients remarkably decreases with improvement of treatment and nursing care. The aim of the Bachelor Thesis was to find differences in nursing care for patients with paraplegia and quadriplegia at spinal units and surgery wards, then an assessment of differences in nursing care for paraplegic and quadriplegic patients. The research part of the Thesis was based on quantitative examination in the form of an anonymous questionnaire and quantitative data collection method by means of indifferent concealed observation was moreover used for complementation of the answers of nurses working at the spinal unit. The research sample for the monitoring consisted of 32 patients, 16 of whom with paraplegia and 16 with quadriplegia, from the Spinal Unit of the Casualty Hospital Brno and the Faculty Hospital Brno. The anonymous questionnaire research was performed at spinal units and casualty wards in the Czech Republic. The questionnaire consisted of 30 questions. 133 questionnaires were distributed, 78,9 % returned. There were five hypotheses set in total. The first hypothesis: Nursing care for paraplegic and quadriplegic patients at a spinal unit is mainly focused on the support of self-sufficiency and activation. The hypothesis was confirmed. The second hypothesis: Nursing care for paraplegic and quadriplegic patients at surgical wards is focused on satisfying the basic needs. The results confirmed this hypothesis as well. The third hypothesis: The risk of immobilization syndrome occurrence is higher in quadriplegic patients than in paraplegic ones from the point of view of nurses. This hypothesis was confirmed by the research. The fourth hypothesis: The risk of infection occurrence is higher in quadriplegic patients than in paraplegic ones from the point of view of nurses. In this case the hypothesis was not confirmed. The fifth hypothesis: The risk of mental disorder occurrence is higher in quadriplegic patients than in paraplegic ones from the point of view of nurses. The results obtained did not confirm this hypothesis. The set goals of the Thesis were met upon confirmation of three hypotheses of five. The Bachelor Thesis may be used as studying material for students of the South Bohemian University, Faculty of Health & Social Studies, for healthcare staff involved in caring for paraplegic and quadriplegic patients.
