Svatava Neuwirthová Title: A case report of physiotherapeutic care for a patient with Parkinson's disease Objectives: The main objective of this bachelor thesis i to process a case study of a ptient diagnosed with Parkinson's disease in the special part and in the theorethical part to provide information about this diagnosis, particularly focusing on etiopathogenesis, diagnosis ant treatment. Methods: The general part includes theoretical information about Parkinson's disease concerning clinical presentation, ethiopathogenesis, pathophysiology, diagnostic options and teratment, including rehabilitation. The special part contains the case report of the patient, whoch was developer during the bachelor's internship at the Nemocnice milosrdných sester svatého Karla Boromejského. Results: The objectives of both the theorethical and special parts were fulfilled. Conclusion: Based on the evaluation of the therapy's efect, it can be stated that physiotherapeutic care was succesful because it had a partially positive impact on the patient's condition. Parkinson's disease is a neurodegenerative progressive disease that cannot be cured even today. However, it is possible to influence and slow down its progress to a certain extent. Symptomatic treatment is used to suppress the disease, of which physiotherapy is an integral part. In Parkinson's disease, physiotherapy can suppress the symptoms of this disease, improve the patient's condition and significantly help the patient maintain a higher quality of life for a longer period of time. The first aim of the bachelor thesis was "To draw attention to the importance of physiotherapy in a patient with Parkinson's disease". The second aim was "To map the possibilities of physiotherapy in a patient with Parkinson's disease". Both of these goals were fulfilled and described in the theoretical part in subchapter 1.8.3. In addition, the practical part of this thesis refers to the first one, where it is possible to see the changes in the condition of the patient with Parkinson's that occur when applying group exercise to his regimen. The third aim was "To design an exercise that would lead to the improvement of the condition of the patient with Parkinson's". This goal is dealt with in the practical part of this thesis. Based on the knowledge gained in the theoretical part, an exercise unit was created, which was then applied for two months to a group of seven probands. To evaluate its effect on the condition of patients, input and output kinesiological analyses were compared, including clinical tests to examine fitness, coordination and postural stability (FTSST, TUG, BBS, Pull Test, Push & Release Test).

Aerobic training in the treatment of gait disorders in patients with Parkinson's disease

This bachelor thesis investigates the effect of aerobic training on gait disorders in patients with Parkinson's disease. The specific features of gait in this disease are short, shuffling steps, slowed movement, impaired balance and reduced upper limb synkinesis. All of these features can, apart from the application of medication, be influenced to some extent by regular exercise. The aim of this bachelor thesis was to design a suitable exercise unit for these patients and to describe the effect of aerobic training on gait disorders. The theoretical part is an introduction to the issue of Parkinson's disease. Subchapters describe the definition, incidence, research history, anatomy and physiology of the basal ganglia, etiology, clinical features, stages, diagnosis and treatment. Furthermore, there is compared gait analysis of Parkinson's disease patients with healthy individuals and what the term aerobic training means. The practical part is processed in the form of qualitative research. Based on the input examination of three patients diagnosed with Parkinson's disease, I created an exercise unit, which can be found in Appendix 3. After three months, I did an output examination and compared if the created exercise unit had a positive effect in gait disorders or not. From the results, the positive effect of aerobic training on gait disorder in Parkinson's disease patients could not be clearly demonstrated. For a valid result, research would need a larger number of patients. However, patients evaluated the exercise unit positively and I believe that this work could find further application, for example in the clinical practice of physiotherapists or as an educational material for patients with Parkinson's disease and their family members.

My bachelor's thesis is an object responding to the space that surrounds it. It points out the contradiction between organic and austere form. In my work, I materialize abstract experiences through metonymically selected building elements related to the places in which I have these experiences. As an extraction of elements appearing in the architecture of the Southern Mediterranean, a form of ornament appears in the work, which is understood by a figurative comparison to the experience of travelling on holiday. Another element, representing a figurative experience of returning "to reality", are the lux spheres. Their character is linked to the architecture of Central Europe. I explore the relationship between the different types of buildings, which I then freely develop. My perception of reality enhances the recognition of the new with the already known. I let the objects represent and capture these two opposing perceptions with implications and emphasis on their differences. In this way, I come to find in my work a similarity between geometric austerity, which signifies perfectionism, and the intuitive unfolding of ornament, which inserts the theme of organicity into the object. An important aspect of the thesis is processuality. I understand my work as a border between conceptual and intuitive process.

The influence of rhytmical movemet on patients with Parkinson's disease

This bachelor thesis deals with the issue of a progressive neurological disease called Morbus Parkinson, which is translated to Czech as Parkinson's disease. This disease irreversibly damages the central nervous system and cannot be cured yet. It is only possible to alleviate the symptoms by pharmacological treatment and improve the quality of life of the patients with the help of rehabilitation. The aim of the work is to describe the main issues of the disease, to clarify the possibilities of physiotherapy and to design a rhythmic exercise unit on the basis of which educational videos for home exercise of patients with Parkinson's disease were created. The first part summarizes general knowledge about Parkinson's disease, history, development and rehabilitation. Most of the work deals with the research itself, which was attended by 3 clients with Parkinson's disease. The individual case reports consist of an initial entrance examination, which also includes, for example, tests evaluating stability and gait. This is followed by two months, during which the exercises took place once a week and then the current state was compared with the results of the initial examination. The studies found and the subjective evaluation of probands show that it is appropriate to include rhythmic exercises in the rehabilitation. Due to the state of emergency lasting continuously from 5th October 2020, it was not possible to conduct group exercises, so the exercises were conducted with two patients online, which allowed patients to exercise safely at home and were not at risk of infection. One patient underwent exercises normally after loosening of the regulations issued by the government. The work can serve as a source of information for the lay and professional public and methods of solution can be used in physiotherapeutic practice.

The effect of a regular exercise on the health of patients with Parkinson´s disease

The bachelor thesis deals with the influence of regular exercise on the health of patients suffering from Parkinson's disease. This disease still remains an incurable disease that affects the human central nervous system and leads to its irreversible damage. Until now, the treatment of the disease has only made it possible to alleviate individual symptoms in the long term and thus maintain the patient's quality of life. An important part from the early stages of the disease is not only a pharmacological treatment but also a rehabilitation. The theoretical part of this work deals with a summary of information about Parkinson's disease, its origin, clinical picture, diagnosis and treatment options. The practical part of the work was processed by the method of qualitative research, which was done on the basis of an input and output examination, medical history taking, an interview and an evaluation of patients' ability to walk. The obtained data were compiled in the form of case reports. The study group consisted of 3 patients with a diagnosis of Parkinson's disease, who regularly attended group exercises once a week. The first goal was to evaluate the patient's condition during regular group exercise and the condition after 8 weeks without a group exercise. The second goal was to create an exercise unit for patients suffering from Parkinson's disease. Due to the small number of probands studied, it is certainly not possible to state whether the regular exercise once a week was sufficient in the treatment of Parkinson's disease. However, it was obvious that the patients felt good during the exercise, it positively affected their psychosocial condition and also their condition. Although I did not come to clear conclusions, I presume that the chosen therapy was beneficial.

Difficulties of the EU - Cohesion Policy Implementation Systém in the Czech Republic in the Programming period 2007-2013

The thesis Difficulties of the EU - Cohesion Policy Implementation System in the Czech Republic in the Programming period 2007-2013 deals with application of the theories of the European integration on two case studies. In the first part of the thesis the general history of the EU - structural policy is described. The second part is dedicated to the history of the structural policy in the Czech Republic. The third part is concerned with the theoretical definition of the topic and leads to a definition of the hypothesis and the research method. The main theoretical approach for the case studies is the multi-level governance theory, which describes multi-level sharing of authority among different types of actors. None of those actors has a power to define final version of the given policy. The case studies, which in detail examine the development and the efficiency of the implementation system of structural policy in Czech Republic, prove that this environment can cause the coordination dilemma connected with overuse of the free-riding tactics and high transaction costs. The combination of those elements leads to highly rigid system of implementation.
