Obesity in children of school age from the perspective of nurses working in GP

Abstract Obesity in School Children from the Point of View of a Nurse Working in a GP's Surgery The thesis focuses on the problems of nursing care of children with overweight and obesity. This issue is very urgent nowadays, not only in the Czech Republic, but it is a global problem. Our country holds one of the leading positions in children obesity. Children suffering from overweight and obesity have not only health problems resulting from obesity complications, but also problems in the field of psyche and social relations, particularly among their peers. This is why the right insight into the issue and finding an acceptable solution for the future is so important. The theoretical part deals with the problems of children obesity, the specifics of nursing care of these children within a surgery of a general paediatrician. It also focuses on the needs of children with normal weight and on the needs of overweight children. Four aims were set within the research. The first aim was to map the situation in obesity among school and pre-school children in Benešov District. The second aim was to map services (possibilities of further assistance, treatment) that parents and practitioners may use in treatment of children with overweight and obesity in Benešov District. The third aim was to compare success rate of obesity treatment in children that have already undergone obesity treatment under specialist supervision and success rate of weight reduction in children trying themselves. The fourth aim was to map the problems of cooperation of a nurse with parents and school in treatment of a child with obesity. We used a qualitative research method in the research part of the thesis. The questioning method by means of semi structured interview was applied. Three research samples were prepared for the research purposes. The first research sample consisted of 10 nurses working in paediatrician practitioner surgeries in Benešov District. The second research sample consisted of 10 persons intentionally chosen from among parents of children with obesity that are trying to reduce the weight of their children themselves or under assistance of an obesity specialist. The children were of school and pre-school age. The third research sample consisted of 10 paediatrician practitioners practicing in Benešov District. Open-coding technique paper and pencil method was applied on interview processing. A document analysis was used for the mapping of the services in Benešov District and the whole Central Bohemia Region. Six research questions were set: 1. What are the possibilities of specialist help to obese children? 2. Are parents willing to help their obese children with overweight reduction? 3. Do parents of obese children also have problems with overweight? 4. What experience do nurses from paediatrician practitioner surgeries have with care about obese children? 5. What does cooperation between parents and a nurse from a paediatrician practitioner surgery looks like? 6. How does a nurse working in a paediatrician practitioner's surgery cooperates with the school? The thesis conclusion shows that the problem of children obesity is not just the matter of Benešov District, but it is a global problem, it is urgent and has an increasing trend.
The Respect for the Rights of Seniors in Nursing Homes

Old age and aging become an increasingly topical issue. All periods of human life are very important, but old age is the last stage of life, it is the crowning of human life. Aging and the period of old age brings many changes. An important change in seniors' lives may be moving in a residential care, for example a residential home for elderly people. This change can be very difficult and problematic. Human rights should be still respected as well as a person's needs should be met. Nevertheless peoples' needs change with age, some grow more important and others become minor. Every person in the world without exception has freedom and rights that should be respected no matter the race, nationality, religion, education, sexual orientation, gender or age. The topic of this thesis is The Respect for the Rights of Seniors in Nursing Homes. The aim is to map different points of view on respect for the rights of older people in particular nursing homes. Two sub-goals have been set. The first sub-goal is to find out how employees of a concrete nursing home perceive the respect for rights of elderly people. The second sub-goal is to find out how the seniors perceive the respect for their rights by employees in a concrete nursing house. The strategy of qualitative research, the questioning method and the semi-structured interview technique were used during the research. The main research question was set What are the risk areas while respecting the human rights in a concrete nursing home? To answer the main research question other six more specific component research questions were stated. The research showed that employees of a particular home for the elderly perceive that the rights are respected for the domains of physiological needs, the needs of safety and security, the needs of solidarity and love, respect and recognition needs and the need of self-realization. The most difficult areas of human rights to respect are the right to personal protection, equality and the dignity. According to seniors' answers we can understand that the biggest limitation in hygiene and movement domain is experienced by immobile seniors. Those who stay mobile do not feel isolated and they feel their freedom. On the other hand the seniors in the category below sixty years of age feel limited by their age and this has an impact on their feeling of isolation and unsociability. I have given the results of this work to the Nursing Home XY as a feedback from seniors on services provided in this nursing home. Results have also been used in the Nursing Home XY during the Quality Standard no.2 revision called "Protection of Rights".
Value System of Senior Citizens in Nursing Home Domov seniorů Jindřichův Hradec

This thesis focuses on the values preferred by senior citizens in old people's homes. Values are an integral part of every human life. The old age brings about a reorientation of values; there is a fixation on the family when senior citizens need to feel a sense of necessity and love. Accommodation of needs is a basis for the development of a value system where its value hierarchy is a structured system governed by importance. Old people feel a need for communication but this need is not very well saturated. With increasing age, feelings of loneliness occur and biological needs become accentuated. The empirical part of the thesis defines the objective to find out what life values are preferred by the senior citizens in the Old People's Nursing Home in Jindřichův Hradec. The method for data collection was a qualitative research, the method of case studies. The research group was selected using the method of quota purposive sampling. It consisted of six users of the OPNH; they were communicative men and women aged 65-85 years who received care allowance of the 2nd degree. The key technique used for data collection was an interview using a manual that was used with the six respondents at the OPNH in Jindřichův Hradec. A total of 15 users were interviewed who met the quota conditions but only six users were willing to participate in the research and to be interviewed. The last research question examined what their view of the life in the OPNH is. The research showed that the respondents were affected most by their stay at a triple room, which restricted their quality of sleep, privacy and adaptation. Retaining the dignity and communication with the staff is most important for the respondents. It is possible to comment answering the main research question What life values are preferred by the senior citizens in the Old People's Nursing Home in Jindřichův Hradec' by stating that the respondents feel as the most important their self-sufficiency and health, which undoubtedly affect the fulfilment of all the other needs, and the family that is the purpose of lives of the respondents, it is their driving force and they have something to live for. This thesis should highlight the importance of the issues preferred by users in the Old People's Nursing Home, the importance of cooperation with their families, how important is communication that helps the users to adapt to the new environment and then the creating of individual plans to maintain self-sufficiency of the senior citizens and, in the event they can no longer be self-sufficient and there is a need of help from another person, it is necessary to realize how important it is for the users to maintain their dignity. This thesis helps to justify the importance of the value system of senior citizens. It also expresses the efforts to improve the quality of life of senior citizens in residential social-service facilities while making social work more effective.
Nursing care about people with infective endocarditis after the cardiacsurgery procedure

Infective endocarditis is inflammation of endocardium which affects mainly the heart valves. It is a very serious illness, often with acute process which even today leads to death in 20-30% of cases. The treatment is long-term and it has impact on various domains in future life. Patients are given high doses of antibiotics and in the case of unsuccessful conservative treatment and vast microbial vegetations with the risk of embolism cardiac surgery procedure is necessary. The appropriate and right nursing care leading to positive mental attitude of a patient is very important during the long-term hospitalisation, as well as education in secondary prevention of this illness. The aim of this work was: 1) to find out how the patient deals with the long-term hospitalisation. 2) to define the patient´s needs during the long-term hospitalisation. 3) to find out the patient´s foreknowledge of the secondary prevention of infective endokarditis. 4) to find out the nurses´ possibilities in the domain of uprating the nursing care about the patients with infective endokarditis. To reach this aim one investigative question was determined: How the nursing care about the patients with infective endocarditis during the long-term hospitalisation is filled up? To process this work qualitative research using Strauss´s and Glaser´s data analysis was used. The data gathering was carried out in the form of half-structured interview with the nurses of cardiosurgical department of Hospital of České Budějovice as well as with the patients with infective endocarditis after the cardiosurgical performance. All the interviews were agreed to be recorded. Literal transcription of the interviews followed afterwards. The rewritten interviews were encoded and processed into figures. Hospitalisation brings certain negatives to the patients, such as fear about their future and life, lack of information and fear from the operation itself as well. Regarding the serious health state the patient´s life values change. They start to appreciate their life, they become more cautious and focus on prevention. Nurses are aware of the most important needs of long-term hospitalized patients. They try to saturate these needs and make their stay in hospital as comfortable as possible. Their knowledge of infective endocarditis helps nurses choose the appropriate way of education of the patients. The support of family is also very important. In spite of all effort of the hospital staff negative relationship between the nurses and patients can occur during the long-term hospitalisation. Nurses try to prevent the possible problems by changing the staff. Most patients found helpfulness of the staff a very positive part of their hospitalisation. The results show the lack of patients´ information about the risk of the origin of infective endocarditis and profylaxy. It would be very helpful to focus on more intensive education of the patients which could prevent this ilness.
Comparison of the quality of life children in orphanages and substitute family care in the Central region

The dissertation is about quality of life in children's homes and children referred to foster family care in Central Bohemian Region. Theoretical part charecterizes quality of life, needs and rights of a child, family and their functions. In next part it describes foster care, deals with foster family care, institutional care and question of transformation and unification of system of care of endangered children in The Czech Republic. The empirical part is pointed to comparison overall quality of children life in children's homes and foster care, their orientation in values, satisfaction in particular parts of their life. The results point to spheres where stated groups are different, where are the same and offer possibilities for next research in this area.
Analysis of the needs of seniors - users of care service of Municipal Charity České Budějovice.

This bachelor thesis deals with needs of seniors - users of care service of Municipal Charity České Budějovice. The theoretic part explains terms such as senility and aging, needs of seniors, help and care provided by the family and by the day care, as well as introduces follow-up services for the seniors. At the end of the theoretic part, legislative contexts of providing social services are specified. The empiric part analyses the needs, or services provided by the family and the care service for the users.
Employee motivation and employee benefits

The goal of this thesis is to get acquainted with the issue of employee motivation from a theoretical perspective, and then analyze the incentive system in a selected company - Sellier & Bellot. In conclusion, I would like to evaluate the lessons learned and propose some changes and recommendations for improving motivation in the analyzed company. The work is divided into four parts. The first three are rather theoretical. The first part deals with the explanation of the concept of motivation and characterizes its types and sources. The second part focuses on the motivation of negotiation at work, where the author presents the most important motivational theories dealing with the factors that encourage motivation and, on the contrary, the ones that distort motivation. The third chapter deals with the issue of employee benefits, especially their importance and classification. Then in the practical part, the knowledge acquired in the theoretical part is applied. The author tried to analyze the system of motivation in the selected company and conducted her own research through a questionnaire survey among employees of the company.
Nursing care provided to clients of the Jewish religion in the Czech Republic and Austria

The main aim of the thesis was to find out the possibilities of providing nursing care to Jewish clients in CZ and Austria, to make a comparison of providing nursing care in both countries and also to find out knowledge of nurses concerning the care of clients with Jewish religion. We put four research questions and this: What are the specifics of nursing care of clients with Jewish religion? What are the differences in the care of clients with Jewish religion between the Czech Republic and Austria? What kind of possibilities of nursing care have in their disposal clients with Jewish religion in the Czech Republic and in Austria? What knowledge have sisters caring for a client with Jewish religion? Qualitative investigation was carried out. It had a form of a personalized non-standardized interview and an analysis of Internet sources. Research sample consisted of 4 nurses from the Czech Republic and Austria. Resource analysis was carried out from the website. Interviews were recorded on a tape recorder and afterwards were overwritten. Interviews with Austrian colleagues were translated in advance. Interviews constitute the basis from which categorization was created, by using the framework of analysis, which was also illustrated by diagrams. The source analysis together with an interview with a nurse from the Austrian institution was illustrated in the diagram too. From the results it is clear that all nurses from the Czech Republic as well as from Austria are aware of the specifics of nursing care of a Jewish client. Differences of the care of clients with Jewish religion between the Czech Republic and Austria are dependent primarily on the knowledge of nurses. Austrian sisters proved that their knowledge is deeper, more accurate and more specific, which is reflected on nursing care. Czech nurses have in comparison with Austrian ones just basic knowledge. This fact can´t cover the nursing care in holistic approach. he results indicate that institutions for clients with Jewish religion are both in the Czech Republic and in Austria. The Austrian one has in comparison to the Czech one a long tradition and many times larger capacity. It should be noted that in the Austrian institution not only clients with Jewish religion are hospitalized. The observance of Jewish traditions and customs is a matter of course in both institutions. Surprisingly, despite the multicultural nursing in the study plan, which the Austrian nurses did not attended, their knowledge was a lot better than of their Czech colleagues. Nowadays a holistic approach is required and without such knowledge it cannot be fulfilled appropriately. This thesis should point out at the necessity of nurses´ education in the multicultural nursing issues.
