View of the patient and a view of the nurse on quality of life

The bachelor thesis deals with the quality of life of polymorbid women patients, seniors hospitalized in the institution of subsequent care. The aim of the work was to determine whether and to what extent there is a diference between the selected patients' view and the nurses' view of the patients'quality of life. The theoretical part primarily focuses on the old age and ageing process, its causes, positive and negative sides, and therapy options in terms of scientific studies. It also describes the terms related to the bachelor thesis, such as geriatrics, geriatric patient, but also the concept of quality of life and its measurement approaches. The empirical part studies the quality of life of six patients and there was chosen a qualitative approach. It was a recorded interview, which was transcribed into a protocol and analysed. It resulted into a theoretical model, which has these parts: Period before the illness, Turning- point, Current state, Memories and Future perspectives. The chosen patients compared their current state, their present quality of life in the way how they experience it during the illness with the quality of life before the illness, and with their view to the future. In my bachelor thesis I was dealing with the problem of morbid obesity (BMI 40 and more) with combined surgery according to worldwide criteria IFSO - International Federation for the Surgery of Obesity and Metabolic Disorders. I consider this topic as a big issue nowadays which is confirmed also by statistics WHO - World Health Organisation. Aim of my bachelor thesis was to describe the hospitalisation of the patient with minigastric bypass for morbid obesity. Further I was trying to find out whether the clinical practise is or is not different compared to nursing care which is stated in literature, health magazines and professional websites. Information and documentation needed for the theoretical part of my thesis was found in search of National Health Library, database Medvik, PubMed and others. Also, I have used information from IFSO, EASO - European Association for the Study of Obesity, SAGES - Society of American Gastrointestinal and Endoscopic Surgeons and Czech Obesitological company. The form used in bachelor thesis is case study based on the search of the particular patient, model used is according Marjory Gordon and nursery process. Nursing care of the patient after gastric bypass for morbid obesity

Bachelor thesis is focused on treatment options for morbid obesity, particularly surgical methods of treatment. The aim of bariatric surgery is to achieve substantial and complete cure of comorbidities when sick morbid obesity occured. The main objective is to demonstrate the patient with morbid obesity solved by Minigastric bypass and uncomplicated course of hospitalization. The first partial objective is to monitor the influence of the force motivation during recovery, followed by development. The second partial objective is to monitor the success of the surgery. These data are obtained through interview, observation and analysis of patient¨s medical records. For the solving case study the nursing model Marjory Gordon was used. The phases of the nursing process will be carried out from case history, followed by setting the goals, intervention, implementation and evaluation of the overall care plan. During my practice and after writing my bachelor thesis I came to the conclusion that the motivation, support of loved ones and positive attitude toward accepting herself with accepting the new lifestyle, is essential for successful undergoing this type of surgery and the further development. Annotation: The purpose of this Master's thesis is to describe objectively topic of medical social workers' communication in centers of post-acute care, to highlight importance and difficulty of social work with senior citizens and to propose eventual changes leading to eventual communication improvement. Social work here is complicated because senior citizens are often lonely or may have problematic family relationships or may suffer from dementia and quite often are polymorbid patients. For the thesis were used statistics from The Centre of Postacute Care - Motol University Hospital, then method of questionnaire realized with medical social workers from centers of post-acute care in Czech Republic, and method of structured interview with patients in Center of Postacute Care - Motol University Hospital.
