Safe administration of injectable medications

Introduction: Drug administration is an essential part of nursing practice. Following the example of other countries, the safety of drug administration is coming to the fore. Nowadays, patient safety is a key element in the provision of quality nursing care. The aim of this thesis was to map the procedure of drug administration by intramuscular and subcutaneous routes. Methodology and research sample: For the purpose of this thesis, the method of participant observation was chosen, which took place in the departments of internal medicine and surgery from August to October 2023. All observations were recorded in a pre-designed record sheet and the data were statistically analysed using SASD and SPSS software. The analysis was based on first and second level classification, contingency tables with absolute and relative frequencies and sign scheme were constructed. The results were then organised into graphs and tables. Results: The results of the thesis indicate that the nurses observed do not follow the labelling of injections to a considerable extent, which can lead to errors. Furthermore, in a significant number of cases the nurses do not take care to properly disinfect the injection site of the injectable drug, which leads to a higher likelihood of signs of inflammation at the injection site. A surprising finding was the precision in hand sanitation. This behaviour is very important because, in combination with careful disinfection of the injection site, it is more likely to prevent the introduction of infection into the patient's body. Conclusion: The thesis provides a comprehensive view on the issue of safe administration of injectable medicinal products. As a result of this work, it will be presented at a student conference and an article will be written on the subject.

Bachelor thesis "Image of the Police of the Czech republic in Mladá fronta Dnes and Právo daily newspapers" analyses the way of presentation of the Police of the Czech republic in Mladá fronta Dnes and Právo daily newspapers in a distance of ten years using a content analysis. It explores and compares years of 1996 and 2006 and documents development and movement in the way of imagery of this state institution. The thesis deals with the situation in the Czech society after the year of 1989 from the perspective of police work and theme in its introduction. There are theoreticly described the method of research and its process in further parts and the results analysis in the end of the thesis.

The thesis is based on individual decisions and annual reports of the Office for the Protection of Competition and the Ministry of Regional Development. The aim of this thesis is to analyze the most prevalent mistakes that occur in the public procurement process, specifically in the last five years. The partial objective is also to evaluate the reasons why the failures occur and to compute their impacts on the efficiency of the procurement process and the cost-effectiveness of the public funds spent. The thesis is divided into two parts, theoretical and practical part. The first part provides the theoretical background of the professional information that is the subject of this work and is necessary for the interconnection with the practical part. The thesis itself analysis the most frequent mistakes, including the measures of the Office contributing to the elimination of errors. In the end there is a recommendation for the contracting authorities.
The administration of risky drugs as a nursing problem

This bachelor thesis deals with an issue of hazardous medication administration and with risks in their storage, ordering and disposal. The aim of the thesis was to find out a quality of experience and knowledge of nurses in the field of hazardous medication as well as their knowledge of the specifics connected with administration of this medication. To fulfil the aim, two research questions and two hypotheses were set. What is the nurses´ experience in the field of hazardous medication administration? What is the nurses´ knowledge of the specifics of hazardous medication administration? Hypothesis no.1: General nurses´ knowledge of specific administration of hazardous medication is influenced by the length of their practical work. Hypothesis no.2: General nurses´ knowledge of specific administration of hazardous medication is sufficient. The research survey was performed through a combination of qualitative and quantitative research methods. In the framework of the qualitative technique, a half-structured interview with individual general nurses was performed (11 respondents) and in the framework of the quantitative research, a non-standardised questionnaire for general nurses (100 respondents) was used. From results of the quantitative research survey is obvious that the general nurses (87%) have knowledge in the field of hazardous medication and they are sufficiently informed about the specifics connected with hazardous medication administration. They know the whereabouts of internal directives of their hospital in the case of emergency, which instruct them how to react in a particular moment or situation. Only one respondent stated that she had been informed only perfunctorily and signing the directive had been the most important. Results of the qualitative research survey revealed that most of the respondents know the hazardous medication, where and how they are stored and what is the right way of their disposal. Four respondents admitted no knowledge of LASA medication. This fact was surprising because the respondents were general nurses with a long practical experience. It was presumed that they would know this field after so many years. In the conclusion, it is possible to state that most of the respondents have very good knowledge of an issue of administration, storage and disposal of hazardous medication. It is possible to state that those respondents who have no sufficient knowledge of hazardous medication issue, have insufficient or missing training in this field. Following this, the thesis was supplied with an information leaflet with a clear outlook of principles of dealing with medication, its storage and disposal, which is inserted as a supplement in the bachelor thesis.
The safety and quality of health services provided in the field of perioperative care

Bachelor thesis "The safety and quality of health services in Perioperative Care" deals with quality control of nursing care in selected inpatient health care facilities as well as prevention and management of potential risks associated with the provision of nursing care. The empirical part of the thesis focuses on the awareness of perioperative nurses regarding the use of and adherence to standards, recommended work practices and quality control of nursing care in operating theaters. Custom survey was carried out at the University Hospital Motol in Prague 5 and simultaneously at the General University Hospital in Prague 2. The obtained data were evaluated using statistics CZ 12 and are presented in tables and figures. In the final discussion the information obtained were compared with the results of previously completed studies, and recommendations were set for improving the quality and safety of nursing care in operating theaters.
The issue of public procurement financed by means of grants provided by the European Union

The thesis deals with the issue of public procurement co-financed through EU subsidies. The theoretical part in the first part focuses on the law of public procurement, its presentation in the Czech Republic and explanation of important terms that are related to public procurement. Another part is devoted to EU legislation and methodological guidelines regulating the area of public procurement co-financed by the EU. In the analytical part they are identified and evaluated the most common errors that occur in public procurement co-financed by the EU in the period 2009 - 2016 according to the Managing Authorities and intermediate bodies, which are complemented by national and European practices of the courts and decisional practice of the Office for the Protection of Competition. Another section contains an overview and analysis of the errors made by contract owners according to the Office and misconduct in public procurement under the Rural Development Programme and Operational Programme for Fisheries in the Ministry of Agriculture and the State Agricultural Intervention Fund in the years 2014 - 2016 according to the Supreme Audit Office. At the end there is an analysis of public procurement in the RDP 2014 - 2020. In conclusion, recommendations are made to eliminate errors and summarized results of the work.
The issue of public procurement financed by means of subsidies provided by the European Union

The thesis deals with the issue of public procurement financed by means of subsidies provided by the European Union. Its theoretical part concerns with public procurement co-financed from EU budget. This first part of my thesis focuses generally on explanation of basis terms concerning public procurement. Moreover, it outlines our national and European legislation and methods interfering in the process of public procurement co-financed from the EU budget. The thesis presents oversight bodies emphasizing the function of the Audit Authority. It pays attention to structural funds during the 2007-2013 programming period focusing on the Regional Operational Programme (ROP) North-West. The practical part concentrates on analysis and records of the most frequent mistakes in public procurement within the ROP North-West by means of analysing a sample of cases identified by the Audit Authority. The thesis uses the outputs in the form of a summary of most frequent mistakes in public procurement of the selected operational programme as the foundation for formulation of measures and suggestions for elimination of mistakes in public procurement and for recommendations on amelioration and efficiency improvement of the public procurement system.
Preventing misconduct in the operating room from the perspective of nurse anesthetist

This thesis deals with often discussed issue and that is patient´s safety. The topic of the thesis is Error prevention in an operating theater from the nurse anesthetist´s perspective. The current findings show that errors in the operating theater are a big problem in which the patient´s but also the medical staff´s health is threatened. The theoretical part is devoted to the issue of misconducts, the most common mistakes that can occur, which mostly occur by the medical staff in the operating theater. This part is also devoted to the monitoring of emergencies, whether an extraordinary event is an educational element to prevent misconducts. It also looks at the sources of errors. The practical part is processed by a qualitative research made in the ARO department. The interviews are processed by open coding technique, using pencil and paper. In the interviews, the respondents answered questions directly about their work in the operating theater. The interviews are focused on possible risks which a patient may be exposed to from the transfer to the operating theater until the end of the surgery. The respondents also mention error prevention possibilities in the operating theater. The aim of the thesis was to find out which are the most common mistakes made by nurse anesthetists; how the nurses prevent errors and how they perceive misconduct by themselves. The results should show some gaps in preventive measures in the operating theaters.
Improvement of the patient?s safety in intraoperative nursing care.

Abstract - Improvement of the patient?s safety in intraoperative nursing care Every activity brings about a certain rate of risk in contemporaneous healthcare facilities. Every nurse has to ensure patient?s safety. Successful management of this task requires a good overview of the patients to whom the care is provided, and of the risks which could endanger the patients, cause them harms or worsen their health condition. The present Bachelor?s Dissertation focused on the topic: ?Improvement of the patient?s safety in intraoperative nursing care?, follows two objectives. The first one is to map the most frequent risks of undesirable events which threaten to the patient during the intraoperative nursing care; the other one is to assess the rate of risk of undesirable events in the intraoperative nursing care. In accordance with the proposed objectives, two hypotheses were stated: H1: The most frequent risk of an undesirable event that threatens to a patient during the operative intervention is burn; and H2: Confusion of the patient?s identity in the intraoperative nursing care achieves the highest risk rate. The Dissertation was drawn up with the help of quantitative research investigation. The data collection method used was based on a survey where a questionnaire approach was applied. The questionnaire was non-standardised and included 21 items. The research dataset was formed of 54 perioperative nurses working in operating theatres of Nemocnice České Budějovice a.s. It implied from the research investigation that the most frequent undesirable events that threaten to patients during intraoperative nursing care are, from the viewpoint of perioperative nurses, allergies to medicine, to disinfection (3.47), sore galls (3,35) and early infection in the wound (2,90). Patient?s burn caused by a neutral electrode (1.98) was on the fifth place of undesirable events. The first hypothesis ?The most frequent risk of an undesirable event that threatens to a patient during the operative intervention is burn? was not confirmed. The undesirable events leading to the most serious consequences in the field of patient?s health condition include confusion of the patient?s identity (4.88), fire arising due to contacts of the coagulation instrument and alcohol disinfection (4.83) and confusion of the side to be operated on (4.80). Through the subsequent evaluation of the results of the questionnaire survey it was found out that the highest rate of risk in the intraoperative nursing care is achieved by confusion of the patient?s identity (3.90), confusion of the side to be operated on (3,85), fire arising due to contacts of the coagulation instrument and alcohol disinfection (3.84). The second hypothesis ?Confusion of the patient?s identity in the intraoperative nursing care achieves the highest risk rate?, was confirmed. The deliverable of the Dissertation is a proposal for safety mechanisms in the field of prevention of confusions of the patient?s identity and of the side to be operated on, as well as basic principles for safe application of the neutral electrode.
Prevention of mistaken identity of patients in health care institution

Mistaking patients is one of the undesirable errors in health care. When disclosed, it makes the entire medical facility look bad and degrades its prestige. An individual?s error also affects the other members of the medical staff. This topic is very current, which is one of the reasons why I chose it for my bachelor?s thesis paper. Safe identification of patients is one of the principal requirements in the provision of high-quality care to patients. Failure of this requirement may lead to mistaking patients, organs, confusing which side to operate on or the surgery itself. The objective of this bachelor?s thesis paper is to identify the risk factors in the individual areas of nursing care as part of the prevention of mistaking patients and to determine what methods are most frequently used in the prevention of mistaking patients at selected hospital wards. Two hypotheses were identified. H1: The strongest risk factor increasing the probability of mistaking patients is insufficient staff-patient communication; and H2: The most frequently used method of prevention of mistaking patients is the use of identification wrist bands. The research survey was executed in the form of inquiry, using a questionnaire. The questionnaire was intended for nurses. The task of the respondents was to evaluate the risk of mistaking patients when a particular risk factor is in action. The risk factors were classified into five groups: communication, documentation administration, administering drugs, surgical procedures and patient identification. In the following part of the survey I determined the methods of prevention of mistaking patients that the respondents most often use in their work. The assessment of the individual statements concerning the issue in question did not confirm Hypothesis 1. The questioned nurses think that communication among staff is the strongest risk factor (mean: 3.96). The strongest risk factor ? failure to provide information about patients ? was found in the field of communication between the nurse and the physician and between the nurses. Hypothesis 2 was confirmed. According to the answers supplied by the nurses, identification wrist bands are the most frequently used method in the field of the prevention of mistaking patients (mean: 4.83). The second most frequently used method of the prevention of mistaking patients is the use of active verbal identification (mean: 4.59). The results of the survey were submitted to the Regional Hospital in Pardubice in the form of a summary chart. The chart contains the strongest risk factors of mistaking patients and preventive mechanisms leading to their reduction. The survey aims to contribute to the elimination of risk factors of mistaking patients and thus increase the safety of patients when hospitalised.
