Szikorová, Nikola ; Malý, Josef (advisor) ; Rejmanová, Anna (referee)
Analysis of the effect of pharmacotherapy on the risk of fall in inpatients II Author: Nikola Szikorová Tutor: PharmDr. Josef Malý, Ph.D. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University Introduction and objectives: Falls are one of the most common problems of hospitalized patients. Their occurrence is associated with many problems and therefore should be avoided. Effective prevention involves analyzing risk factors and implementing interventions to minimize these risks. The aim of this work was to analyze the influence of drugs and other risk factors on the fall of hospitalized patients. Methods: Data was collected from April to December 2017 in 4 hospitals in South Bohemia. Only patients who had fallen during hospitalization participated in the study. An online database was created for the study to collect patient data (personal history, medications used, laboratory values and others). Population and individual fall risk-increasing drugs were analyzed. For each individual fall risk-increasing drug, the type of drug problem that could have been the cause of a fall was classified. Risk minimization interventions have been assigned to each drug problem. In addition to fall risk-increasing drug analysis, other risk factors that increased the risk...

Objective: The aim of this study was to analyze the effect of the fall risk-increasing drugs to falls in hospitalized patients and to recommend the right interventions to prevent the fall. Methodology: For the analysation, classification and quantification of the fall risk-increasing drugs, drug related problems and interventions a special online database was created. All of the doctors, clinical pharmacists and nurses were collecting details about the pacients and about how the fall happened. Fall risk-increasing drugs and drug-related problems were then analyzed by clinical pharmacists. Potential interventions were suggested. Also other...

Bucková, Veronika ; Horký, Pavel (advisor) ; Ťupová, Lenka (referee)
Antidepressant (SARI, NDRI, RIMA) Consumption in Czech Republic Charles University Faculty of Pharmacy in Hradec Králové Department of Social and Clinical Pharmacy Author: Veronika Bucková Supervisor: PharmDr. Pavel Horký, Ph.D. Antidepressants are drugs used mainly to treat pathologically low mood and anxiety disorders. The antidepressants focused in this work from the SARI, NDRI and RIMA groups are also used in the treatment of insomnia, smoking cessation, as support in weight reduction and in some psychogenic sexual dysfunctions. The goal of this work is to analyze the consumption of antidepressants from the SARI, NDRI and RIMA groups in Czech Republic. The data for this work were obtained from the database of the State Institute for Drug Control on the basis of mandatory reporting of the volume of drugs supplied by distributors to pharmacies and other medical facilities. Consumption was monitored in the period from 1.1.2008 to 31.12.2020. The DUR type methodology was used and consumption was expressed in the number of defined daily doses per thousand inhabitants per day (DID). The results confirm the rising trend of antidepressant consumption only in the case of trazodone (SARI), whose consumption increased 3.8 times compared to 2008. The reason for this growth is the use of trazodone as an...

Drug Information Centre service analysis V. - Adverse drug reactions Author: Adam Badura Supervisor: PharmDr. Kateřina Malá, Ph.D. Consultant: PharmDr. Petra Rozsívalová Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University, The Czech Republic Introduction: Drug information centre (DIC) of the Faculty of Pharmacy of Charles University and University Hospital Hradec Králové was established in 1994. Its activities include among others information provision for healthcare professionals by answering drug related enquiries. One of the main subjects of enquiries in practice are adverse drug reactions. Aim: Aim of this study is analysis of drug-related enquiries focused on adverse drug reactions processed by DIC in a period of 2015-2021. This study also includes complete answers to two drug-related enquiries that deal with adverse drug reactions. Methods: Drug related enquiries focused on adverse reactions were analysed according to factors including but not limited to the type of enquirer, age and gender of patient affected by adverse drug reactions, presence of ATC (Anatomical Therapeutic Chemical) codes, types of adverse drug reactions according to SOC (Subject Organ Class) classification, utilisation of drug information resources and recommended...

SSRI antidepressant consumption in Czech republic Author: Júlia Behrová Supervisor: PharmDr. Pavel Horký, PhD. Consultant: PharmDr. Eva Zimčíková, Ph.D Introduction: Selective serotonin reuptake inhibitors increase the available amount of neurotransmitter serotonin in synapses, resulting in their antidepressant and anxiolytic effect. Objective: The objective of this diploma thesis was to evaluate the consumption of SSRI antidepressants in the Czech Republic in the period from 01. 01. 2008 - 31. 12. 2018, based on the data on consumption from the State Institute of Drug Control (SIDC). Methods: The research consisted of a retrospective analysis of the SIDC database. The study included all approved SSRI in the Czech Republic. Drug Utilization Review was used in the thesis as a methodology. Drug consumption is calculated as the number of defined daily doses per thousand inhabitants per day (DID). The data on the number of residents was acquired from the Czech Statistical Office. Descriptive statistics and ATC/DDD methodology were applied. Results: The consumption of sertraline and escitalopram increased significantly during the period under review. For sertraline, the DID increased from an initial 8.623 to 15.513, increasing consumption by 1.8 fold. The greatest increase in consumption was observed...

MUDr. Olga Matoušková - the dissertation theses The influence of individual genetic predisposition to the pharmacokinetics and pharmacodynamics of chosen opioids ABSTRACT Introduction: The aim of this thesis is to study the influence of polymorphism of CYP2D6 and MDR1 on the pharmacokinetics and pharmacodynamics of tramadol in healthy volunteers using measurement. A secondary objective is to evaluate these polymorphisms in relation to the analgesic efficacy and side effects of piritramide for acute postoperative pain. Materials and methods: In two prospective work studying the influence of genetic predisposition on the pharmacokinetic and pharmacodynamic parameters of tramadol, we included a total of 90 healthy volunteers. Clinical studies on opioid analgesia and influence of genetic predisposition to the pharmaco-therapeutic effects and side effects in patients with acute postoperative pain, we included a total of 161 patients with acute postoperative pain. Polymorphism genotyping CYP2D6 and MDR1 gene we performed PCR - RFLP analysis, to determine concentrations of tramadol and metabolite, we used gas and liquid chromatography and pharmacodynamic effects of opioids was evaluated by pupilometric measurement and visual analogue scale. Results and conclusion: Variability of the opioid effect is influenced by...

Dual antidepressants are prescribed in treatment of depressive disorder, anxiety disorder, in pain therapy and insomnia. Besides that, there are trials to prove the efficacy of dual antidepressants in different indications. Generally, antidepressant consumption in the Czech Republic is increasing. The question is, what the real proportion of dual antidepressant consumption is. Objective: As the objective, dual antidepressant consumption was analysed. Methods: The consumption of antidepressants venlafaxine, duloxetine, mirtazapine and mianserin was performed in the period from 1 January 2008 to 31 December 2018. The State Institute for Drug Control (SÚKL) database was used. The data were described by descriptive statistics. The drug consumption in Defined Daily Doses (DDD) was converted into Defined Daily Dose/1,000 inhabitants/day (DID). In addition, comparison of use of particular dual antidepressant doses was made. Results: Overall, the relative consumption of dual...
