Nursing process in prevention of immobilization syndrome

The bachelor's thesis deals with the issue of immobilization syndrome. It aims to point out the risks associated with long-term immobilization, recognize the early symptoms of immobilization syndrome and prevent complications. The theoretical part focuses on the characteristics of the immobilization syndrome in individual organ systems, where related changes and prevention are described in detail. The empirical part consists of a questionnaire survey, which focuses on the knowledge of nurses from the standard and ICU departments. The verified knowledge concerns the prevention and nature of the immobilization syndrome. A total of 99 respondents across the Faculty Hospital in Motola participated in the survey. The obtained data were processed and then the knowledge of nurses was compared based on education, type of workplace and length of practice. Objectives: The aim of the bachelor's thesis was to provide a comprehensive overview of relevant findings on immobilization syndrome. Prevention of complications in the treatment of patients after stroke

The thesis deals with prevention and appearance of complications with the patients after a stroke. These patients are endangered by many complications, connected not only with the disease itself but also with their health before hospitalization. For the purposes of my thesis, I chose a qualitative method. It is a specific method which uses given problem in details and gives a complex perspective of all examined criteria. I collected the data with the help of example studies which I examined further on and compared it. The results of my research show that the most frequent complications endanger the chosen patients. Further on I deal with interventions with which I intentionally prevented the complications. In the last part I evaluate the results of preventive arrangements about how the respondents percieved my treatment care.
Verticalization of patients with peripheral ECMO in terms of nursing care

INTRODUCTION: ECMO is an increasingly popular method of support used in cases with severe respiratory or circulatory failure. Treatment is often associated with significant immobilization of the patient, which may take up to several weeks, resulting in deterioration of physical fitness, extending lenght of hospitalization, and a return back to normal life is very difficult. In recent years, however, efforts have been made to enable mobilization even to those patients who may be conscious, in order to maintain their muscle power. OBJECTIVES AND METHODOLOGY: The aim of this thesis was to obtain information about nurses' experience with mobilisation of patients treated by extracorporal membrane oxygenation, to evaluate an occurrence of the most frequent barriers that prevent them from mobilizing paticents and to evaluate differences in nursing attitude among the addressed workplaces. The research was carried out in the form of a questionnaire of my own creation. The research sample consisted of nurses working in Intensive Care Units under cardiocenters in the Czech Republic where the ECMO method is used. The immobile ill person at home

The aim of this thesis was to find out the most frequent problems of the immobile patients and the people who care about them at home. This thesis is divided in two parts. The first part is theoretical and the second part empirical, which is based on the scientific reasearch itself. The theoretical part contains the definition and the contents of the expression immobility and the immobility syndrome. Then we follow with the most frequent demonstrations of the immobility syndrome. In the next topic there are needs of the immobile patients and the family that cares about them, then we mentioned the help for the members of the family and the home care agencies. The part that is very important is the area of the surroundings of the immobile patients that need more attention. In the last section we mention the allowances for disabled people and the areas where the patients can find help. The empirical part of this thesis was based on the quality approach and with the assistance of the semi-structural dialogue. The dialogues were carried out with the immobile patients at their homes and the people who take care of them as well. The information we got were afterwards analysed and put in the tables. The results of the research showed the most frequent problems of the patients and the people who take care of them at home and that is non-sufficient awareness of the care at home and the help of the home care agencies, social services and some other kinds aids. Concerning the patients it was mainly the problem of hygiene, food, emptying, building amendments at home. It is very important to mention that all the problems mentioned by both sides patients and their helpers wouldn´t appear if they were sufficiently informed before leaving hospital and going home.
Issues of nursing care in prevention of contractures of immobile persons

Contractures as one of the physical symptoms of immobilization syndrome influence further components of a personality and the life quality. This is why prevention of the occurrence of contractures is one of the roles of a nurse. She does it by means of the rehabilitation treatment elements, i.e. positioning, mobilization, verticalization, in cooperation with a multidisciplinary team. The aim of the thesis was to examine to what extent nurses are informed on contractures in immobile persons and to learn about the role of a nurse in prevention of contractures in immobile persons and to examine the level of cooperation between a nurse and a physiotherapist on the prevention of contractures in immobile persons. To meet the goals, we chose qualitative research in the form of semi structured interviews with nurses and physiotherapists. We analysed the results by means of open coding, the pencil and paper method. The results processed this way were categorized. They have shown that the questioned nurses consider their theoretical knowledge and practical skills in prevention of contractures sufficient and they mainly achieved them through the practice. Nevertheless, some unclear points that might affect the provided care appear in some replies. The results indicate that insufficient attention is paid to contractions by the interviewed nurses. They particularly prevent them by positioning, but mobilization and verticalization stand aside. Some of the respondents add the elements of the basal stimulation. The questioned nurses except others cooperate with a physiotherapist, which leads to overleaping of their functions, particularly in positioning and verticalization. The work has revealed insufficient orientation of the nursing care to the prevention of the occurrence of contractions among the questioned nurses. This finding as well as the other results of the research might be used as an input for a seminar tailored to this topic.

Nutritional care about immobile patient on standard department of internal type

Nutritional care for immobile clients at a standard department of internal type becomes still more important, because nutritional care is important for maintaining good health and supporting quality of life. Nutritional status of immobile clients is very important to diagnose as early as possible in order to avoid all risks of potential complications. Adequate nutrition is also very important to maintain good mental health. The central objective of this work was to explore the specifics of nursing care in the field of nutritional care for immobile client at a standard department of internal type. Following this goal, three research questions were set up: How is the nutritional status of patients at a standard department of internal type assessed? What are the possibilities of application of nutrition for immobile client at a standard department of internal type? What are the specifics of nursing care in the fields of nutrition of immobile client at a standard department of internal type? The empirical part of the thesis was processed using qualitative research, which was realized through semi-structured interview technique. The interview contained 3134 questions, which focused on nutrition care for immobile clients on standard unit of internal type. The research sample consisted of nurses working in the standard department of internal type in the České Budějovice Hospital, a.s. Overall, this study leads to two findings, both positive and negative. Within the positive findings we can say that nurses provide adequate care in the application of nutrition per os. Negative findings relates to the fact that the interviewed nurses provide only basic care in case of the introduction of enteral and parenteral nutrition. They do not know more detailed specifics of enteral and parenteral nutrition. Based on this finding, we think it would be useful to organize regular seminars and trainings for nurses to expand their knowledge about this type of care, or at least remind it. This thesis can serve as an informational material for nurses in practice or students of other health professions. This work can serve also as a basis for further research.
Comparison of chosen nursing in prevention of immobility syndrome in a senior nursing home in Germany and the Czech Republic for lying immobile patients.

As the population is getting older there is increased number of geriatric patients. However there is also increased number of immobile seniors, who are not able to take care of themselves. Comprehensive health and social security is being provided in homes for elderly, which fulfil the role of long-term institutional care. Nurses must be trained for this highly professional and knowledgeable nursing care aimed at geriatric problems. For geriatric clients of home for elderly it´s important realization of prevention, treatment and social measures in the form of an individual approach. That these measures can be implemented it is necessary to evaluate the health status of seniors, their physical performance, self-sufficiency and mental functions in connection with the social situation and to formulate a nursing diagnosis. As a result of long-term bed rest can lead to health complications affecting all body systems, so-called immobilization syndrome. How to prevent secondary damage that threatens the patients White Theo arge already ill is focused rehabilitation nursing. Among rehabilitation resources in the prevention of immobilization syndrome in nursing practice arge included: positioning, verticalization, mobilization. Inseparable part of prevention measures is preventing health complications in case of immobile clients homes for the elderly arge suitably selected positioning aids, mobilizing resources, well-equipped bed with antidekubitus system. In the thesis there were two objectives. To Compare keeping preventive measures which prevent the formation of pressure ulcers and muscle contractures in nursing care for immobile patients in the Czech and German retirement homes. To map out the possibilities of aids in the prevention of complications in immobile clients in the Czech and German retirement homes. Bachelor thesis has been made by using qualitative research. For collection of data was used a method in-depth interviews with four Czech and four German nurses. This method involved non-standardized interviews through open-ended questions that were recorded by dictaphone and transcribed verbatim. The data analysis was carried out by hand coding or using a pencil and paper. By analyzing the data four categories were created, each divided into the home for the elderly in the Czech Republic and the home for the elderly in Germany: 1.Nurses knowledge ,2.Implementation of preventive measures , 3.Equipment for immobile clients, 4.General knowledge related to immobile clients. Research survey found out that there is not a big difference in adherence to preventive measures to prevent the formation of decubitus and muscle contractures in nursing care for immobile patients in Czech and German homes for the elderly and comparable are also the possibilities of using tools to prevent complications in case of immobile clients. The biggest gap in nursing care for clients is bad knowledge of contractures in both homes for the elderly. Also keeping documentation and records relevant to the implementation of preventive measures in the Czech home for the elderly is not satisfactory. Mobilization record is missing and positioning record should be more detailed. We therefore propose the creation of positioning and mobilization recording modeled according to the German retirement home and handing it to the leadership of retirement home in the Czech.
Prevention of immobilization syndrome in elderly hospitalized at the Department of aftercare in the context of the multidisciplinary approach.

This bachelor thesis focuses on an actual topic in the care of the elderly. It is prevention of immobilization syndrome of the elderly in context of a multidisciplinary approach. The thesis is focused on the elderly hospitalized at an after-care unit. In the introduction to the theoretical part the current state of a quality of an elderly life is assessed. Subsequently particular terms related to old age and senescence are defined. An inseparable part of the assessment of the quality of the elderly life is the classification of self-sufficiency and efficiency. Here the International Classification of Functioning, Disability and Health is mentioned as a meaningful tool for the availability of social services, but also as a clinical tool. The definition of immobilization syndrome emphasizes that this is a geriatric syndrome, which is closely connected to hypomobility, decondition, instability, falls, sarcopenia and other geriatric symptoms and syndromes. The main topic of the thesis - the multidisciplinary approach - is summarized in seventh subchapter and its sections. Individual team members and their basic activities while working with the client are defined here. The research part of the thesis is divided into several chapters. It is processed in a form of qualitative research. It follows the theoretical observations and assesses the work of particular members of the multidisciplinary team in practice. The analysis of personal documents was used on purpose, medical records of selected clients were used as source. Case reports were created afterwards. Case reports are processed using the medical records of clients from the after-care unit. They are also supplemented by the client's point of view, using questions within a semi-structured interview. In the course of case studies, commentary from involved observation is inserted. The conclusion consists of evaluation tables. Detailed expression , comparison of the results and overall summary are in chapter "Discussion". The research question should be answered whether the attitude to clients of the after-care unit is multidisciplinary and whether it corresponds with the needs of clients. The most important finding of this bachelor thesis is that a major factor is the client's diagnosis, and the individual team member's approach to the client. In "Discussion" chapter a comparison based on specialized literature is provided, discussing the long-term care in the international context. Research may be used as feedback to organizations providing long-term care to ensure better conditions of quality of elderly life. It can also be used as a basis for the realization of specific preventive measures. It would definitely be useful to chart the basic research situation in other healthcare facilities or private facilities providing health and social services.
Leisure activities in the home for the elderly to prevent immobilization syndrome

This bachelor thesis with the topic: "Leisure time activities in the Home for seniors as prevention of the immobilization syndrome" is divided into two sections - theoretical and practical. In the theoretical section the terms relating to old age and types of leisure time activities offered by the Home for seniors are described. In the thesis conclusion I describe the immobilization syndrome also occurring in the elderly. Recently, there has been a large increase in senior population, and many senior people have been trying to keep active. Even when they are in Homes for seniors, they still try to find some stimulating activities that can bring joy to their lives. In the practical section I describe my survey research and its results and the answers to the research questions. The survey was carried out in the Home for seniors in Pacov, using a qualitative method of investigation by semi-structured interviews. The interviews were conducted with 6 nurses working in the Home for seniors and with 7 clients of the Home for seniors in Pacov. First of all, we set two goals that we wanted to achieve. The first goal was to find out the influence of leisure time activities on the immobilization syndrome, the second one was to give an account of the ways in which leisure time activities are offered to seniors, and to check whether seniors make use of those opportunities. To meet these targets, we stated the following research questions: How the offered leisure time activities in the Home for seniors affect the immobilization syndrome development in seniors? What leisure time activities are offered by the Home for seniors? What leisure time activities are used most by seniors? The research questions were answered in the interviews which revealed that in the Home for seniors the activities most used by clients are ergotherapy and group exercises. The clients taking part in leisure time activities feel themselves in a good condition and are happy that such activities are available in the Home. The activities are also welcome by the nurses in the Home for seniors, because the elderly feel relaxed, are happier and the nurses have less work to do when clients are in a good physical and mental condition. The output of this work is represented by a booklet which was created to serve other Homes for seniors as information on leisure time activities for seniors.
