Proteomics as a tool for understanding molecular mechanisms of human diseases

Proteomics is a set of analytical methods which enable qualitative and quantitative characterization of the proteome. Expression proteomics quantitatively compares proteomes of cells, tissues, body fluids or other biological materials to find differencies in protein expression and, based on these differencies, to describe the biological processes occuring in investigated organisms. An initial material for expression proteomic studies are complex mixtures containing thousands of proteins, which are analyzed using separation (electrophoretic and chromatographic) methods, and identified, possibly quantified using mass spectrometry. The aim of this Thesis is to demonstrate the application of the tools of expression proteomics in solving diverse challenges in biomedicine. We employed various proteomic approaches and tools for studying molecular mechanisms of human diseases using pacient biological samples, or a model organism and a cell culture. Education of patients with left ventricular assist device HeartMateII

The thesis discusses the education of patients with left ventricular support HeartMate II. Theoretical part is focused on education and its ethical and legal aspects, on the description of the educational process. Furthermore it focuses on heart supports, especially on left ventricular assist device HeartMate II. To provide a comprehensive overview, I have included also information about the anatomy and physiology of the cardiovascular system and the section on heart failure. I described the specifics of nursing care for patients with implanted HeartMate II system and the educational topics that should be part of the education of these patients. The empirical part was prepared as a quantitative research, where research sample was 47 respondents. Respondents consisted of nurses who educate patients after implantation of the HeartMate II at the cardiovascular surgical intensive care unit. To create the feedback I complemented a quantitative research with structured interviews with patients after implantation of the HeartMate II. The aim of the research was to determine the status and range of education of patients with left ventricular assist device HeartMate II. ECMO in University hospital Pilsen

The presented dissertation concerns of extracorporeal membrane oxygenation (ECMO) in adults within the University hospital in Pilsen. The ECMO has been used in Pilsen from 2011. The first part (theoretical one) illustrates in general the ECMO, a life saving method for a treatment of failured lungs or heart. The sites of cannulation, the management and monitoring of ECMO patient are then described. Possible complications as a result of ECMO application and its settlement are mentioned. The second part consists of a retrospective insight, which shows data of 43 ECMO patients for a period of three years 2011- 2013 in the University Hospital Pilsen and a questionary survey. The survey involves 83 non- medical healthcare workers, who are in charge of the ECMO patients, at three intensive care and resuscitation units where the ECMO is used. It finds out what do the medics know about the method. The outcome of this dissertation is a guideline of nursing care of an adult patient on ECMO and most common difficult situations and solutions, that could be faced in the course of nursing an ECMO patient. The adenylyl cyclase signaling system in normal and failing myocardium

This bachelor thesis describes a signal pathway of adenylyl cyclase, which plays a key role in the modulation of heart rate and force of contraction. This pathway begins with membrane-bound β- adrenergic receptors that activate the enzyme adenylyl cyclase. Adenylyl cyclase produces second messengers by reverting ATP to cAMP. Several changes occur in this pathway in failing heart. The most striking changes occur in β-adrenergic receptors, but there are some changes on the level of adenylyl cyclase and G proteins as well. Most of these changes are related to chronic high levels of catecholamines, especially norepinephrine. Some medications try to reverse these adverse effects of norepinephrine. β-blockers are traditional drugs for treating heart failure. However, adenylyl cyclase may be also considered as potential target for pharmacotherapeutic interventions in the future. Powered by TCPDF (www.tcpdf.org)

Problems of patients with long-term cardiac support

Bachelor thesis is divided into theoretical and empirical part. The theoretical part is devoted to heart failure, given that it is patients with end stage heart failure is a long-term cardiac support implemented. It is described surgical treatment of heart failure and various cardiac support. A separate section is devoted to long-term cardiac support-Heart Mate II. Conclusion The theoretical part is devoted to the specifics of treatment of patients with established long-term cardiac support. The empirical part of the paper maps out the problems of patients with established long- term cardiac support. The method used to implement the survey questionnaire. The studied group consisted of 21 patients with established long-term cardiac support in outpatient care. Reconnaissance survey conducted in the outpatient department of heart failure IKEM in Prague. The work shows that although the introduction of cardiac support for patients and mentally demanding affects their daily life, most of them feel improvements in your health and well-established cardiac support perceived as a chance to wait for a heart transplant.
Allooimmunosensitization in left ventricular assist device recipients and impact on post-transplantation outcome

Background: In recent years mechanical circulatory assist devices became an established option in bridging patients with refractory heart failure to heart transplantation. One of the alleged limitations of mechanical devices is a high degree of antibody production with possible deleterious effect on subsequent heart transplantation outcome. Aim: The main goal of this study is to assess the role of antibodies on the outcome of surgical treatment of patients with end- stage heart failure. Method: Firstly, we present a literature review on the current state of knowledge of possible immunologic mechanisms involved in antibody production in left ventricular assist device (LVAD) recipients, new methods of antibody detection, desensitization strategies and overview of published evidence assessing the impact of sensitization on post-transplantation outcome. In the experimental part of our study we prospectively evaluated the presence of anti-Angiotensin II Type 1 Receptor (AT1R) antibodies in 83 Heart Mate II (HMII) recipients who were implanted at our institution between 2008 and 2012 and survived the first 60 days. The aim of this study was to describe hypertrophic caridomyopathy in different cat breeds. The review characterizes forementioned disease regarding genetic base, considering the most affected breeds and informs of up to date possibilites of diagnostic and treatment. The work is extended by the prevalence of the disease in particular cat breeds in chosen region of Czech republic based on survey.
Evaluation of biosignals and quality assessment of heart assist devices

The first part of this bachelor thesis deals with technical problems of apparatuses for heart assistance Heart Mate II LVAD and Levitronix, which are connected with components of apparatuses, parametrs of apparatuses, function of apparatuses, indications and contraindications. In the next part there is description of various biosignals, which are measured according to application of heart support aparatuses. The next aim of this bachelor thesis is suggestion of evalution biosignals, which are monitored and collection of clinical data in which I will statistically evaluated the results and these results will be compared each other and with physiological values. The final aim of this thesis is a reflection of the advantages and disadvantages of the apparatus and discussion.
