Correlation between inspiratory muscle strength and fitness in patients before lung transplantation

Introduction: Exercise capacity is frequently limited in lung transplantation candidates which can result in worse clinical outcomes of the transplantation itself. There can be several factors limiting exercise capacity, the purpose of this thesis is to determine the influence of inspiratory muscle strenght on reduced exercise capacity in these patients. The aim of this study is also to evaluate whether the Timed Up and Go Test is feasible for exercise capacity assessment in lung transplantation candidates as an alternative for the Six- Minute Walking Test. Methods: 14 patients with end-stage chronic lung disease considered for the lung transplantation waiting list inclusion performed Six-Minute Walking Test, Timed Up and Go Test and inspiratory muscle strenght evaluation using POWERbreathe® K5 (S-Index value). Correlations of the results of these tests were investigated. They were also evaluated separately for a group of patients with inspiratory muscle weakness (S-Index values lower than 80 % of predicted values). Abstract Severe hearing loss is standardly treated by cochlear implantation. Considering the anatomical proximity and interconnection to the vestibulocochlear nerve, it is possible to influence vestibular functions. The aim of this diploma thesis is to evaluate the effect of unilateral cochlear implantation on postural stability and vestibular function in adult patients with severe hearing loss caused postlingually. Eleven patients aged 28-72 were examined the day before surgery, the day after surgery, and on average 25 days after surgery. The evaluation consisted of short-form Dizziness Handicap Inventory (DHI), Timed Up and Go testing (TUG), static stabilometry and the perception of the subjective visual vertical (SVV) static and dynamic. Testing of the mobility and dynamic ba lance of seniors

The theoretical part deals with problems of old age and aging and its impact on balance, mobility and walking alone. It summarizes the knowledge about execution, use and reference and other values of the Timed Up and Go, clinical test used in senior population mostly in testing mobility, walking, dynamic balance and risk of falling. The research part contains an experiment, where two groups of people - group of seniors aged 66 - 87 years and control group aged 43 - 60 years - were examined by the TUG test and its variant with a cognitive task. The results of this work show, that some seniors achieved a higher TUG score than healthy population of foreign studies. In a group of seniors TUG was able to distinguish different level of functional mobility and it's determinants. In combination with assessment of the quality of performance and cognitive task, which causes decrease of score and performance in the elderly, can serve as a fast and available screening functional status and mobility.
