Poláchová, Petra ; Vondráčková, Petra (advisor) ; Jandáč, Tomáš (referee)
Background: Treatment and aftercare experience shows that addiction clients do not orient themselves to their experience, perceiving it as threatening with a habitual tendency to avoid it. Negative emotions can act as triggers for relapse, for this reason it is important to pay attention to the topic of emotions also in the aftercare phase. Aims: The aim of this paper is to describe how negative emotional states and so-called core emotions contribute to the maintenance of addictive behaviour in the aftercare phase. In the practical part, the therapeutic work with a client is presented, focusing on the treatment of two relapses, within the theoretical framework of the cognitive-behavioral model of relapse prevention and using elements of the psychotherapeutic approach of Emotion- Focused Therapy. Methods: A qualitative research method was chosen in the form of a case study of a client of a aftercare centre who had experienced two relapses. Ethical aspects of the work were observed. Results: The case study illustrates the client's motivation to avoid negative emotional states through the use of alcohol, thereby easily achieving the necessary calming and release from intrapsychic and external environmental pressures. Two main sources can be considered as triggers for relapses: feelings of loneliness...

Biochemical predictors of the treatment failure in pediatric IBD patients
Zárubová, Kristýna ; Bronský, Jiří (advisor) ; Hrdý, Jiří (referee) ; Petrášová, Miroslava (referee)
Up to 20 % of patients with IBD (inflammatory bowel disease) is diagnosed in childhood, the course of the disease tends to often be more severe and the treatment more difficult than in case of adults. Therefore, it is necessary to commence or eventually change the treatment in order to prevent development of complications. Early detection of relapse plays significant role for further course of the disease and general prognosis of the patient. Changes to laboratory parameters (e.g. to the level of fecal calprotectin) can significantly prevent the development of clinical symptoms and may indicate the necessity for control endoscopy, further escalation or change in therapy. The primary focus of this thesis are patients who underwent surgical bowel resection, who constitute one of the risk groups. Studies show that as many as 70 % of patients suffering from Crohn's disease reach the point of bowel surgery within the first 10 years of the diagnosis. According to the results, a promising predictor of relapse of the disease seems to be serum albumin, which may relate to the overall nutritional status of the organism. We have also examined the effectivity of using fecal calprotectin and CRP as relapse predictors. Testing directly in the resected bowel did not prove usability of calprotectin as a predictor,...
Family psychoeducation and relapse prevention in psychotic disorders
Pleštilová, Lenka ; Bankovská Motlová, Lucie (advisor)
Relapse prevention is a primary long-term clinical goal in treatment of psychotic disorders. Pharmacotherapy by itself is not sufficient, because compliance of patients in remission is usually low and there are more factors contributing to relapse, especially stress of patients resulting from emotionally demanding situations in the family. PREDUKA (PREventive EDUcational programme for relapse prevention) is a six-hour professionally-led group programme for patients with psychotic disorders in ambulant therapy and for their relatives. In a field questionnaire survey we mapped 1) theoretical knowledge of participants, 2) usage of practical advice obtained from PREDUKA and 3) general benefit of programme. Participants from years 2007 - 2009 were included. The sample consisted of 14 patients and 22 relatives, 27 women and 11 men. The average age of patients was 28,6 years, that of relatives 44,4 years. High return of questionnaires from patients confirms their goodwill to cooperate. Knowledge of participants about psychosis and their therapy is very good, compliance of patients from our sample is high. Participants usually follow advice obtained from PREDUKA. Common is the ignorance of important phone numbers (crisis centre, attending psychiatrist), so we suggest to include a page in a workbook for these dates....
