Standardized examinations of spasticity in physiotherapy

Title: Standardized examinations of spasticity in physiotherapy Abstract: This bachelor's thesis is aimed at giving an overview of the most commonly used standardized examinations of spasticity in physiotherapy and their comparison. It deals mainly with the best known methods of examinations that are used in practice. The thesis is divided into two parts - theoretical and practical. In the theoretical part, muscle tone and its regulation and spasticity as one of the upper motor neurone syndrome symptoms are specified. Moreover, the methods of examination (Ashworth scale and its modifications, Tardieu scale and five-step clinical assessment of spastic paresis according to Gracies) are presented and described in detail there. The practical part contains two case reports of patients after a cerebrovascular accident. Examinations and evaluations of spasticity, including Modified Frenchay scale or walk tests, which are described in the theoretical part, are applied to patients. The main contribution of this thesis is to create a comprehensive comparison of standardized examinations of spasticity through studies and my personal experience. Use of the Modified Frenchay scale in stroke patients with spastic paresis

The objective of this thesis is to describe the Modified Frenchay scale and its use in stroke patients with spastic paresis. The thesis is divided into two parts - theoretical and practical. In the theoretical part, the issue of spastic paresis in stroke patients is specified; and in addition to this the methods of examination and assessment of spasticity are presented and described in detail. A large part of the thesis deals with the history of Frenchay arm test and its modification - Modified Frenchay scale. The theoretical part also contains a description of the phases and types of grasp, some of which can be observed during the assessment with Modified Frenchay scale. The practical part consists of a questionnaire mapping the situation in the institutions in the Czech republic, that use the Modified Frenchay scale in their practice, and of two case reports of patients after stroke. Examinations and evaluations of spasticity, including Modified Frenchay scale, which are described in the theoretical part, are applied in these case reports. The practical part also contains an evaluation of inter-rater reliability of the Modified Frenchay scale. Clinical Utility of Occupational Therapy Standardisee Test "Action Research Arm Test (ARAT)"

The Action Research Arm Test (ARAT) is a standardized tool, which is used for fine and gross motor skills evaluation of hemiparetic patients after stroke. It is widely used abroad thanks to its excellent psychometric properties and easy administration. It is also used in many foreign studies to evaluate the function of upper extremity, especially for neurological patients. Unfortunately, so far it has been used only sporadically in the Czech Republic. In the theoretical part of this thesis the ARAT and the Modified Frenchay Scale (referred as "MFS") is described. In the practical part, the MFS is compared with the ARAT. The main aim of this diploma thesis is a pilot comparison of sensitivity of upper limb motor skills evaluation done by ARAT and MFS. Thirty patients after stroke were tested in the Rehabilitation Center Kladruby in three weeks. They were further divided into three intervention groups according to the severity of the paresis (light, medium, heavy). The analysis of the results shows that the sensitivity of both diagnostic tools is statistically comparable. According to the Spearman correlation coefficient, the ARAT and the MFS correlate very strongly in range of 0.96-0.98. The Bachelor thesis is focused on description of individual points of the 0-10 scale of the modified version of the Frenchay arm test in patients after cerebrovascular stroke (CVA). The thesis is divided into two parts - theoretical and practical. The theoretical part describes the issue of CVA in detail. It includes statistics, types of strokes and clinical symptoms. Further, in the theoretical part, tests for upper limb examination are mentioned. The Modified Frenchay Scale and the Frenchay Arm Test are described in more detail, two other tests are mentioned only marginally. The theoretical part also mentions the types and phases of grip that we can observe in the Modified Frenchay Scale and the list of common daily activities. The practical part of the bachelor thesis contains three case reports of patients after CVA. Patients in the bachelor thesis represent the acute, subacute and chronic phase of CVA. The case reports include descriptions of each task of the Modified Frenchay Scale and their scoring on the scale 0-10. Attachments include a manual with suggested descriptions for the individual points of the 0-10 scale of the modified version of the Frenchay arm test.
