The quality of children's life following burn injury

Background: The bachelor's thesis deals with the assessment of the long term health- related the quality of life (HRQOL) in children up 2 to 8 years of age following burn inury by The Brisbane Burn Scar Impact Profile (BBSIP) questionnaire in the Czech Republic. Objectives: The aim of the theoretical part of this thesis was to provide a review of the literature and individual studies about HRQOL in children following burn injury and its context. The aim of the empirical part was to assess the influence of individual socio- demographic and clinical determinants on the HRQOL in these patients, identify, which of these variables most predict the HRQOL and implicate these findings into the context of nursing practice regarding comprehensive care for pediatric patients with burns. In this study we also examined the psychometric properties of the czech version of the BBSIP questionnaire on a sample of pediatric patients in the Czech Republic. Methods: The study population comprised n = 1221 children aged up 2 to 8 years, who were hospitalized at the Department of Burn Medicine, 3rd Medical Faculty, Charles University and the Královské Vinohrady University Hospital in the period 2016-2021 and met inclusive and exclusive criteria. EPIDEMIOLOGY ( EPIDEMIOLOGIC RESEARCH) OF BURN INJURIES IN CHILDREN Lenka Eichlová Supervisor: MUDr. Monika Tokarik Background: Burn injury belongs to main cause of morbidity and mortality in children. More than 100 000 people sustain flash burn every year, 40% of them forms children. Near minimus insider babies into two years come to scorch mostly owing second men. Cardinal cause of thermic injury off elderly is by personal endeavour. Burns differ by mechanism, range, depth and localization. An age is other crucial factor determinants prognosis of patient. And just therefore children's burns are very specific disbranch of medicine. Object: I tried to created a labour, in which I elaborate statistical data of newly hospitalized patiens on clinic of burns in FNKV behind the year 2008. Patients: The set contained 373 newly hospitalized patiens. I have divided them according to age, sex, mechanism of scorch, range, depths and localization of scorch. Further I followed longitude of hospitalization, way of applied therapy and occurence of complications. Results: Predomination of burns is in boyhood (65%). The highest occurence of burns is in the group at the age of 0-2. The most frequent reason of burn in this group is caused by scalding hot liquid and further contact with hot body. Fluid resuscitation in treatment of burn shock

Treatment of burns can be divided into 3 phases. During first 36 hours fluid resuscitation and securing airways are the most important aspects of therapy. The fluid resuscitation is complex, and protocols are used to increase the consistency and success of treatment. The adequacy of the protocols should be continuously monitored and adjusted to the patients response. Current studies are exploring the use of antioxidants to minimize the oxidant stress during the burn shock phase. The aim of this thesis is to review current strategies in fluid resuscitation during the first 36 hours after burns.

According to the demografic researches, the population is getting older. The death is caused not only by cardiovascular or tumorous diseases, but it include external fault as well. The seniors are more endangered. On the one side there are more possibilities to injure themselves, on the other they have more chronic diseases, that may influenced the progress of therapy. Because the subpopulation of seniors will be getting bigger in the next years, it will be great to think of some possible methods of prevention and of their practical use. That our senior days could be spend not only in quantity, but in quality as well.
Occupational Therapy for Adult Patients with Burn Trauma Comparison of Approaches in the Czech Republic and Sweden

of the Thesis: The aim of this bachelor thesis is to describe the occupational therapy interventions for adult people with burn trauma and to identify the differences between occupational approaches for adult patients with burn trauma in the Czech Republic and Sweden. The process of treatment of people with burn trauma is described from medical and occupational therapy point of view in the theoretical part. The partial occupational interventions, which are used for the aforementioned target group, are also discussed. The summarized information about Czech and Swedish health service is also provided and analysed, including the statistical data about hospitalized patients with burn trauma in those two countries. The partial differences in the occupational therapy for adult patients with burn trauma in the Czech Republic and Sweden are described in the practical part. The differences were determined by using various procedures of methods of qualitative data collection. Occupational therapy has not been a common part of the treatment process of burn trauma in the Czech Republic yet, although it is very likely it would positively affect the quality of patients' lives. Compartment syndrome in pre-hospital urgent care

This bachelor thesis focuses on conditions associated with compartment syndrome development in prehospital care. Pressure increase in a closed anatomical space is characteristic for this condition, leading to circulation and neural sensitivity affection in the specified area. In the prehospital care, we may meet with the compartment syndrome in fractures, burns, allergic reactions, traumas caused by an accident, electrotraumas; further in crush syndrome, traffic accidents, craniocerebral injuries and other conditions associated with swelling. This thesis focuses on burns, fractures, electrotraumas, intra-abdominal hypertension and conditions caused by snake bite. Inconsiderate or inadequate manipulation with affected patient may worsen his/her condition and lead to deterioration of his/her health condition or even fatal consequences. The aim of this thesis was to map the theoretical and practical knowledge of non-medical health care professionals working in prehospital care, in treatment of conditions able to delay compartment syndrome development or even to accelerate it. Data collection for the research was performed using questionnaires and semistandardised interviews. Questionnaires were filled by 20 randomly chosen respondents. 12 respondents were interviewed. Correct answers evaluation is listed in the tables in the practical part of this thesis. Interviews transcript is a part of the practical part as well, with colour coded correct and incorrect information provided by the interviewed medical rescuers. A sample test and list of posed questions with sub-questions are included in the attachment. Both parts of the research were performed with medical rescuers and general medicine nurses working at medical rescue services of southbohemian region and the region of Vysočina. The research found that the term of compartment syndrome is well known by medical rescuers and general nurses working at medical rescue services. However, they are not aware of its association with prehospital care. In some cases, the answers of the responders differed significantly, as for treatment of acute conditions associated with this problem, from the recommended procedures according to the literature. Furthermore, the research analysed the type and amount of used fluids in pre-hospital care. This question showed interesting findings as well. Lastly, the research analysed the options of acute fasciotomy in circular neck burns in prehospital care. The respondents interpreted this procedure as life-saving, as this condition is life-threatening for patients and quick transport without releasing incisions is very risky. It is a procedure that should be known to medical rescuers, at least at the theoretical level, and considered life-saving, even if they do not have sufficient competences to perform it. Respondents showed interest and effort for deepening of their knowledge about this procedure during the research. This situation may be improved by additional training in traumatology, surgery and intensive care not only in medical rescuers working in medical rescue services, but in students of these specialisations as well. This thesis could be a base for deeper investigation of this topic, currently insufficiently known; furthermore, for increasing specialised knowledge of medical rescuers in treatment of oedematous conditions and increasing of health care quality in prehospital acute care.
