Monitoring of the core muscles activity in postural stability examination by Dynamic Neuromuscular Stabilization tests using surface electromyography in patients with low back pain

The Master's thesis is focused on monitoring of the core muscles and m. biceps femoris activity in postural stability examination by Dynamic Neuromuscular Stabilization tests using surface electromyography in patients with LBP. Trunk extension test, trunk flexion test and squat test were chosen. The aim of this study was comparing muscle activity timing, symmetry of muscle activation and average activation between healthy subjects and patients with LBP. The changes of muscle activation pattern were observed and average activation was determined. In theoretical part the principles of DNS, surface electromyography and the relationship between postural stabilization and LBP are discussed. Two groups were measured - patients with LBP and a control group, 20 subjects together. The onset times were measured and rank of activation for each test and for each subject was determined. Then the average order for whole group was specified. Measured data were evaluated according to Wilcoxon signed-rank test. In average activation there was analysed change of activation in time. Results were compared between two measured groups. There is significant difference between the two groups in the rank of activation of m. ES l. dx. in trunk extension test and m. EO in squat test. Jnjury of anterior cruciat ligament (ACL) is frequent. ACL lesion has serious consequences on the knee joint biomechanics as well as neurophysiological changes. This study sum111arize the facts in the field of ACL lesion. in ter111s ofin jury, therapy and the return of a patient to daily life. The present research into anterior cruciate ligament (ACL) lesion has shown varying results. We investigated the electro111yographic (EMG) activity of fe111oral 111uscles (se111i111e111branosus 111. (SEMI). biceps femoris m. (BF), vastus medialis 111. (VM), vastus lateralis m. (VL)) during specific physical activities (PA) in the closed kinetic chain (CKC). The main focus was on the protective pattern of femoral111uscles activity in knees with insufficient stability, due to ACL les ion. The work, for your easier orientation in it, is devided up into four groups described us: hip joint disorder, inguinal hernia, sportsman's injury and referred pain. The effect of the cencept of DNS in patients with chronic vertebrogenic syndrome of lumbar spire

The main aim of the presented diploma thesis was to record objective and subjective changes after therapy with DNS concept in a group of 13 people with LBP. Moiré projection topography was used to record postural changes after 5.5 week therapeutic intervention. It was not clear, whether DNS exercise would help them to achieve visible postural changes and whether this technique is able to record these changes, which would mean that it is a suitable option for objectivisation of therapies focusing on correction of stabilisation and postural functions. Pedoscan was used for further objectivisation of the effect of DNS concept. The chosen correlate of the mentioned objective evaluation was Oswestry disability index, evaluating limitations of common everyday activities resulting from pain in the lower back, and the Numeric pain scale. Patients underwent 5 hour-long physiotherapeutic lessons, the recommended frequency of exercises in the household environment was 3-4 times per day. The probands exercised with the use of DNS concept according to the individual examination findings, especially in the area of the integrated spine stabilisation system. Monitoring of the impact of external load on timing of shoulder girdle muscles during abduction in scapular plane - pilot sEMG study

Bachelor's thesis "Monitoring of the impact of external load on timing of shoulder girdle muscles during abduction in scapular plane - pilot sEMG study" is focused on the changes of muscle activation pattern in response to increase of strength demands. Theoretical part contains a review on factors influencing the movement outcome and the strength capability of musculoskeletal system. Experimental part is based on pilot sEMG study monitoring the impact of external load on timing of shoulder girdle muscles during abduction in scapular plane. Thirteen subjects (10 females, 3 males) at the age range from 20 to 30 years (median 21 years) were included in this study. The experiment consisted of 3 model situations: A) without external load, B) with light load (1kg) and C) with excess load (3-10kg). All muscles were activated earlier with increasing load. The differences between mean onset times of individual muscles were decreasing with increasing load. Results of this study support the theory of coordination-strength funcional reserve of musculoskeletal system. Discussion summarizes possible consequences and possibilities of using this phenomenon; both in testing, therapy and monitoring its effects.
Iliotibial band syndrome

This bachelor's thesis deals with the problematics of the iliotibial band syndrome. The iliotibial band syndrome is one of the most common knee injuries in runners, but we can also come across cyclists or triathletes with this injury. The thesis divides into two parts. The existing data and information about the matter are presented in the theoretical part. Anatomy and function of the iliotibial band, aetiology of the syndrome and its development, the mechanism of the overuse and then final diagnostics and treatment are described. In the practical part is presented a case study of an amateur cyclist with the iliotibial band syndrome, which constitutes an initial examination, treatment and autotherapy plan and subsequent final examination. After 10 therapy sessions, the patient did not notice any changes in his condition, but he gave a lower pain rating than at the beginning and the range of active and passive movement on the affected leg was increased.
Coxarthrosis therapy through posture optimalization

v anglickém jazyce This thesis strives to find relationship between human posture and the origin and progression of coxarthrosis. The main idea is built on the fact that the degenerative joint disease often arises on the basis of the chronical overload in the functional decentered position. The theoretical section is focused on trunk and hip muscles coordination that is defined in the ontogenesis. Moreover, the information about joint loading and stabilization is mentioned. The practical part consists of two case studies, which are observing subjective and objective changes after therapy. There are no local techniques in this therapy.

The data were acquired using kinematic analysis Qualisys and processed using software Qualisys track manager and Visual3D. The subject of research was the range of motion in hip joint in frontal and sagittal plane, than the movement of pelvis in frontal plane, movement of pelvis against thorax in transversal plane and deviation of the axis of spine in frontal plane. These parameters were documented using angular deviation of the segments, deviation of the segments in the plane or shifting of the markers in coordinate system. Kinematic analysis dinamic neuromuscular stabilization tests in people with low back pain.

The main target of this thesis was to detect objective changes in the implementation of four postural locomotor tests DNS and during walking. The movements were recorded using the optical MoCap - the kinematic analysis Qualisys. The particular interest of the investigation was to determine the current stabilization of the spine system in individual subjects aged 25 to 55 years, which was documented by means of the angular deflection of each segment, the flat segment deviation or the shift of the marker in a coordinate system. The measurement results were compared between ten individuals with LBP and ten persons without any difficulty who were placed into the control group. The data were collected using the software QTM and Visual3D and subsequently were statistically processed. A statistically significant difference was observed during walking when comparing the angular deflection of the pelvis to the chest at the rotation and lateroflexion (ř). The significant difference of the pelvis shift toward the chest during flexion and rotation (ř) was showed in a deep squat test, but the difference was not confirmed in lateroflexion (ř).
