Quality of life at home parenteral nutriton

Bachelor thesis deals with quality of the life at home parenteral nutrition. Theoretical part briefly describes nutritional support, malnutrition and compares artificial nutrition. Further is described knowledge related to parenteral nutrition, its indications, contraindications, methods of filling; complications associated with venous access and the administration of nutrition itself. Other chapters focus on the home parenteral nutrition and last part deals with the notion: quality of life. To meet the main purpose of the work four objectives were set. The first objective was to determine the life quality of the patients on HPN. The second objective was to determine how patients on HPN were satisfied with support of the nutritional clinic. The third objective was to assess the extent to which symptoms affect the quality of life patients on HPN. A fourth objective was focused on comparing the quality of the life patients on HPN with and without stoma. The research part is compiled from anonymous standardized questionnaire dealing with the evaluation of quality of the life from the perspective of the patients themselves. Sense of nutrition care in institutions for the seniors

Malnutrition is a common and not fully revealed clinical problem in facilities for the elderly. Despite significant medical advances it remains an important and highly prevalent public health problem in developed countries. An early identification and appropriate nutritional support can help stop malnutrition and the negative consequences associated with poor nutritional condition. The risk of malnutrition increases with age and low levels of proper care. This thesis presents the findings of the situation of nutritional care in selected homes for the elderly through participation on NutritionDay Worldwide 2015 (nDay), which is compared to results in the world. It summarizes the knowledge about the individual organization and provision of nutritional care in selected homes. To the project nDay, held on 11/19 2015, home for the elderly in Kolín, home for the elderly PRAGOM CS, s r. o. in Pňov-Předhradí and a social care home Hagibor in Prague were involved. Respondents of these homes make up 9.4% of respondents in the Czech Republic. The result is that three selected facilities, which are compared with similar ones in the world, have a higher incidence of malnutrition. This thesis deals with the topic that is related to drugs. This problem is quite broad content, it can accommodate several respects. The author chose a certain part, namely the crime of unauthorized production and other handling of narcotic and psychotropic substances and poisons under § 283 of the Criminal Code, in the context of the current situation. The theme of the drugs problem was processed in many dissertations, but it should be noted, has continued to discover new knowledge and to change trends in the drug scene. The work consists of seven separate chapters. The first chapter introduces the reader to the issue, which is related to drugs and also defines the basic concepts relating to drugs and drug-related crime. This chapter is important for understanding the following chapters. The second part of the introductory chapter describes the division of drugs and about each group is given basic information. The second chapter provides a history of the legislation related to drugs from Austro- Hungary until the present. Long-term vascular access for home parenteral nutrition and their complications

Home parenteral nutrition is a treatment enabling the patients with intestinal failure to live their lifes in their home environment. The possibility to commence and practise home parenteral nutrition requires not only a medical and pharmaceutical team, but also paramedical staff, which plays a signifiant part mainly in education of the patient and his close relatives. The study's aim was to characterize the patients involved in the home parenteral nutrition programme (HPN) and catheters used for this purpose and as well to analyze complication rate of the long term catheters related to their use and subsequent care. Into the retrospective quantitative study were included patiens treated with home parenteral nutrition since january 2009 to september 2015 within one nutrition care center. Data collection was realized by research in medical documentation. Altogether 135 patients were included in the study (93 female and 42 male), who have hade 227 catheters inserted in total (114 portcatheters, 73 Broviac catheters and 40 PICC) with 86 187 catheter days. The most frequent basic disease was cancer (36%) and the most prevalent indication to HPN was a short bowel syndrome (50%). The Nutrition Day at the standard and intensive care beds in the teaching hospital

The Bachelor thesis consists of two parts - the theoretical and the practical one. The theoretical part is divided into several chapters; the first one is focused on the main information about malnutrition such as its definition, prevalence, classification, causes, impact and the methods of diagnosing. The following chapter describes the algorithm for nutritional care; the rest of the chapters deal with the artificial nutrition, where the indications, contraindications, complications and other essential information related to enteral and parenteral nutrition are described. The practical part is dedicated to the worldwide programme "Nutrition day" and also to the evaluation of the data given by patients of the 4th internal clinic VFN in Prague during the programme. The research part was carried out on grounds of quantitative survey through a standardized questionnaire. There are two aims to the study; the first one was to find out the food intake of all patients of the 4th internal clinic. This goal was not accomplished, because not every patient placed in the standard ward was willing to complete the questionnaire. Respect to human dignity of critically ill patients at ICU and ARD

This bachelor's thesis is focused on the issue of respect for dignity of patients at Intensive Care Unit (ICU) and Anaesthesiology and Resuscitation Department (ARD). The main objective of this work was to determine how nurses perceive and respect the human dignity of patients in intensive care. Furthermore, to determine which procedures are the most harmful to dignity and whether nurses respect the rights of patients and the Code of Ethics. The theoretical part is divided into three chapters. The first chapter defines the basic terms: human dignity, intimacy, shame, personality, morality, ethics and patients' rights. The second chapter is focused on document Essence of Care 2010, part of which deals with the issue of respect for human dignity in health and social care. The third chapter describes the important virtues, qualities and communication skills of nurses. The practical part describes objectives, methodology and organization of research. The research group involved 86 nurses (10 men, 76 women). All respondents work at ICU and ARD at the General University Hospital in Prague. Knowledge of procedures and skills in cardiopulmonary resuscitation on nurses working in the field of anesthesia and intensive medicine

This thesis deals with the knowledge of processes and practical skills of cardiopulmonary resuscitation among nurses working in the field of anesthesia and intensive medicine. The theoretical part of this paper defines the notion of cardiopulmonary resuscitation, deals with its history, changing definitions, and the latest recommendations. It also describes and specifies terms such as competence, knowledge of the processes and the role of the nurse in the system. The empirical part, using a questionnaire survey, assesses orientation of nurses and nurses specialists in the latest recommendations in cardiopulmonary resuscitation and the related knowledge and competences in the use of resuscitation itself. The questionnaire survey was answered by 152 respondents from 3 large hospitals in Prague. There were also the results of its investigation, compared with the results of research conducted by Václav Kukol in 2010 - 2011 as part of their thesis. In the final discussion paper evaluates the confirmation of the hypotheses. Then attach the recommendations for practice.
Refeeding syndrome

Refeeding syndrome (RFS) is a serious metabolic disorder caused by improper procedure of realimentation in patients, who are starved or severely malnourished. All clinical features of the syndrome are result of many metabolic changes, which are mainly caused by depletion of phosphate, potassium, magnesium and thiamine, as well as retention of sodium and disturbed glucose metabolism. The theoretical part of this thesis summarizes the current knowledge about the prevention of the RFS. The research part of the thesis explores the influence of intense realimentation in high-risk patients, particularly the values of various biochemical parameters before or during the early beginning of the realimentation. We measured values of indirect calorimetry, BMI, serum concentrations of phosphate, potassium, magnesium, prealbumin, albumin, zinc, iron and vitamin D. We also monitored concentrations of phosphorus, potassium, magnesium and cortisol in urine. We reported reduced values of potassium in urine for all patients (5/5) and reduced values of phosphate in urine for four patients (4/5). In four patients (4/5) we also monitored decreased levels of serum prealbumin, including two patients even below the value of 0.1 g/l, which is considered to be a significant risk factor for the development of the RFS.
