The Role of an ICU Nurse in DAS Algorithm

We are investigating this topic because there is no complex view of the role of an ICU nurse in difficult airway (DA). This is despite the fact that the nurse is an inseparable part of the team. In the case of inadequate management of the team securing the airway, DA can end up being fatal for the patient. It is necessary to realize that if the nurse knows her role, she can play an important part in the DA and minimize this risk. The main aim of our study was to find out the role of an ICU nurse in DA or in the DAS algorithm to minimize the permanent damage to the patient. The sample of our research consisted of 10 intentionally selected participants, working at the Clinic of Burn Medicine of the Královské Vinohrady University Hospital in the ICU and at the Clinic of Anaesthesiology and Resuscitation of the 3rd Faculty of Medicine of the Charles University and the Královské Vinohrady University Hospital. Our research is qualitative. Data collection was done by semi-structured interview method. Interviews with participants were recorded, then transcribed and presented using annotated transcription. Reasons and Frequency of Ventilatory Circuit Disconnection in Patients on Artificial Pulmonary Ventilation

The thesis deals with the reasons for disconnecting the ventilation circuit and the frequency of individual causes. It is part of a broader and more comprehensive study whose common goal is to combat ventilator-associated pneumonia (VAP). The theoretical part of the thesis briefly describes the definition of VAP and its preventive elements. However, it highlights the conflict between manufacturers' recommendations and the results of the latest studies regarding the interval for replacing individual components of the ventilation circuit. Furthermore, it draws attention to the limited and insufficient emphasis placed on this issue both in the theoretical sphere and in clinical practice. The practical part of the thesis presents the results of the conducted observation, whose main objective is to identify the most common cause of ventilation circuit disconnection. The assumption was a high proportion of the frequency of replacing individual circuit components,especiallythe passive heat and moisture exchanger,which was confirmed by the results of the research survey. This process is necessary to keep the airways clear and open. Suctioning can lead to a number of-post-suction complications such as asystole, bradycardia/tachycardia, desaturation and changes in blood pressure values. Preoxygenation before suction from the lower respiratory tract appears to be an effective method to prevent the-complications described above. Current research indicates that preoxygenation prevents a decrease in saturation. This information led us to the question of whether preoxygenation can positively affect other physiological functions as well. Pulmonary rehabilitation and respiratory physiotherapy techniques are an important element of therapy in patients with chronic respiratory disease. However, during the covid pandemic, it turned out that these methods could have positive effects in the treatment of the long-lasting post-covid symptoms. Therefore, this thesis briefly summarizes the persistent symptoms caused by the COVID-19, and further focuses primarily on the benefits of the pulmonary rehabilitation and the respiratory physiotherapy. It also focuses on the proper implementation of these techniques in intensive care units. Although nurses are most often in contact with patients in healthcare facilities, their knowledge and skills are often insufficient and awareness of this issue is often minimal. The aim of this master thesis is to summarize the techniques of the pulmonary rehabilitation and the respiratory physiotherapy, a description of the procedure and benefits of specific methods that can be performed with patients by nursing staff in addition to the work of physiotherapist. Patients' experience with intensive care

This diploma thesis inquires into the experience of the ill with intensive care. Patients associate intensive care with both negative and positive experiences. They are sensitive to remembering the experience, especially a negative one, and often consider the topic uncomfortable. The main goal of this thesis was to ascertain the patient experience with nursing care provided in the intensive care environment. The secondary goal was to determine the differences in experiences of patients with the COVID-19 diagnosis and without it. The qualitative phenomenological research was carried out in a single stage. Interpretative phenomenological analysis was used as the research method. Data were collected using semi-structured interview with eight selected patients. The results indicate that the nurses play an important role in the intensive care environment. Patients, both with COVID-19 diagnosis and without, leave the intensive care environment with positive and also negative experiences. Negative experiences are largely associated with the act of providing nursing care, nursing activities, the environment, general attitude and the behavior of individual members of the medical staff. The options for family members of dying person to engage in nursing care at intensive care unit

v AJ This diploma thesis deals with possibilities that would improve the involvement of the dying's family members in nursing care in the intensive care unit. The aim of the thesis was to acquaint with the issue of family involvement in nursing care in the environment of intensive care through the theoretical part, where the following chapters are reflected in the problem. Crucial for the diploma thesis is quantitative research, which used a questionnaire survey focused on non-medical health professionals in an intensive care environment, to find out the experience, skills and knowledge of respondents with the involvement of the family in nursing care for the dying. The result is the discovery of factors that limit the family's involvement in care. Furthermore, finding out ways in which the medical staff work with the family, which proved to be a major problem. The research finds the solution in greater activity of the medical staff regarding offering opportunities to the family to be involved in care. The possibilities of palliative care of adults in the district of Pribram and its continuity

In my thesis we were focusing on providing palliative care to adults in the district area of Pribram. The main goal was to map out real possibilities of palliative care and its availability and continuity for patients in the designated area. The main question of the research was: "What are the possibilities, availability and continuity of palliative care to adult patients in the area of Pribram?" As a secondary goal we wanted to find out the amount and process of using palliative care with selected general practitioners in the area. Additional question we asked was: "Is specialized palliative care in the district area available to all who need it?" Both our questions were answered and goals completed. To elaborate the topic we chose qualitative research using the method of grounded theory. After processing the theoretical part and creating semi-structured interviews, we addressed a total of 16 participants from three areas - general practitioners from the district, paramedics and all organizations in the district area that are offering specialized palliative care to adult patients. Terminal extubation within DCD and its perception by ICU's health workers

Despite the fact that hundreds of transplants take place in the Czech Republic every year, hundreds of other patients will remain on the waiting list and not all them will undergo transplant that would save their lives. Absence of routine clinical practice of DCD may also play a role in this. If it will start to be a part of routine local transplant practice as well as DBD, the number of available organs for transplantation will go to increase. The aim of my work has been description of the DCD problematics in the Czech Republic and to find out how medical staff perceive the procedure of terminal extubation, which is part of the DCD process. My work has a theoretical and practical part. In the theoretical part I summarize the available information about DCD, including the course of the DCD process. Furthermore, the work deals with the process of terminal extubation, including perception of the terminal mechanical ventilation by medical staff. I carried out the research part of the work by using a quantitative method and it took place in two phases. The tool of the research method was a non-standardized questionnaire in both phases.
