Endocrine function of adipose tissue and its role in development of atherosclerosis.

Adipose tissue represents a multi-functional endocrine organ. Obesity may lead to the increased release of proinflammatory factors and adhesion molecules from adipose tissue, thus playing an important role in the development of atherosclerosis. Recent research studies were focused on the mechanisms of the endothelial dysfunction, low-grade inflammation and adipose tissue endocrine function dysregulation, and their mutual relationships. These processes are of a great interest due to their clinical relevance and increasing numbers of patients suffering from cardiovascular diseases or being at high risk of early atherosclerosis progression. Exact mechanisms and interrelationships of these factors are yet to be fully clarified. (...) Results of this project may represent another step on the way to the detailed explanation of the mechanisms and relationships between adipose tissue endocrine function or dysfunction and atherosclerosis development. Better understanding and ability to prevent or treat the negative metabolic effects of adipose tissue-derived molecules would be of great importance for numbers of patients at high risk of cardiovascular diseases.

Surgical sex conversion

The aim of this thesis is to concentrate to inform the comunity with less known problems of nursing the patients with disorders of sexual identification - with transexuality. The persons with this disorder have genetic and somatic equipment belong to one sex, however their sexual identification is orientated to contrary gender. Surgical conversion of sex of transexuals forms oni y one of the aspects by complex approach to this complicated disorder. The decision when the surgery is indicated is up to sexologist and authorized by the committee. The commision decision is based on detailed and prolonged psychological approach to everyone. The observation is pointed at adaptation for new social status a:fter longtime hormonal treatment and follows a:fter operation and o:fficial conversion ofthe gender as well. Islet transplantation represents an alternative approach in the treatment of patients with type-1 diabetes mellitus. It makes possible to give insulin-producing tissue to patients, which could not underwent surgery operative due to ill health. The second benefit of tissue transplantation is utilization of pancreases unsuitable for organ transplantation in term of surgical criteria. The dissertation includes two studies focused on study of some problems of islet transplantation. In the first study was tested a new method for induction of microchimerism by bone marrow transplantation and its effect on islet allograft survival. We reported, that short-term tacrolimus therapy significantly prolonged insulin independence in fully allogeneic rat model even if the immunosupresants were discontinued. Conditioning of recipients with bone marrow cells transplantation had not additional effect. In the second study there is presented an approach for imaging of intrahepatally-sited islets. The technique is based on labeling of islets by non-toxic superparamagnetic agent during two day culture. After transplantation islets are imaged as hypointense spots on T2 weighing magnetic resonance sections in both in vitro and in vivo experiments.
