Achiral and chiral high performance liquid chromatography of drugs and their metabolites III.

Nabumetone is non-steroidal anti-inflammatory prodrug that is converted into (6-methoxynaphthalen-2-yl)acetic acid in the liver. The differences of in vitro nabumetone biotransformation (rat vs human liver cytosolic and microsomal fractions) were studied in previous communication. Study of stereospecificity of enzymes and the calculation of enantiomeric ratios of the biotransformed chiral metabolites is an equally important characteristic of enzymes such as the Michaelis constant. Calculation of enantiomeric ratios of chiral metabolites biotransformed from prochiral drugs in vitro is used to identify new enzymes responsible for the biotransformation of a given prochiral drug. This work deals with the optimization of separation and detection conditions for achiral and chiral LLE-HPLC-PDA analysis of nabumetone and its six metabolites in extracts from various biomatrixes (human liver microsomal and cytosolic fractions and minipig blood plasma). Differences in biotransformation of nabumetone and racemic mixture of 3-HO-nabumetone between man and woman were studied in vitro in extracts from liver cytosolic and microsomal fractions, using chiral LLE-HPLC-PDA analysis. Methods Patients with ALS (n = 21) were non-randomly assigned into two groups. First group (group R) received a 3-week in-patient rehabilitation programme in the Rehabilitation Hospital Beroun. Second group, control group (group K), received a home exercise programme. Patients from both groups underwent spirometry examination and evaluation of functional skills via the Amyotrophic lateral sclerosis functional rating scale revised questionnaire (ALSFRS-R) three weeks apart. Calculations of parameters of biologically active compounds by the quantum mechanical methods.

8 ABSTRACT The aim of this work is to calculate the parameters of substituted bases and chosen phenylguanidinium cations, earlier synthesized at the Department of inorganic and organic chemistry on the Faculty of Pharmacy CU in Hradec Králové, using quantum chemical methods. I solved chosen problem by using computer programs Gaussian 03W and Hyperchem 8.0.10. I used program Gaussian to optimize the molecules and calculate the energy of the HOMO and LUMO, total energy and the atomic partial charge using the Mulliken population analysis. I used programe HyperChem to calculate the Van der Waals molecular volume; Van der Waals molecular surface; logP; the molar refraction; solvent-accessible surface of the probe size 1.0 Å; 1.2 Å; 1.4 Å; 1.6 Å and the atomic partial charge using methods PEOE. All parameters of selected substituted phenylguanidines above have been successfully calculated, and listed in the tables in the chapter called results. The calculated results were discussed in chapter called discussion. The calculated parameters will be used for further correlations and QSAR studies that will help to understand the biological activity of substituted phenylguanidines and hence their effect on the human organism.
Physiotherapy procedures in patients after disc surgery in lumbar spine

This thesis focuses on problems of physiotherapy procedures after a surgery of intervertebral disc in the meaning of intensity, timing and therapeutic procedures. In the general part it gives comprehensive overview about functional anatomy, kinesiologi, biomechanics of the spine and about the function of intervertebral disk. It also deals with methods of investigation and surgical interventions. In the special part, the thesis focuses on the most appropriate choice of timing, intesity and mainly on special methods which are the most commonly used in patients after surgery of intervertebral disc. It gives the basic charakteristics of each special methods in relation to patients after surgery of intervertebral disc. The practical part of this thesis includes patient case report after surgery of intervertebral disc which describes the examination before neurosurgical intervention, surgery, hospitalization and outpatient rehabilitation.

Obstetrical palsy of upper limb results from a perinatal lesion of brachial plexus nervous bunch. The incidence of this disease ranges from 0.42 to 5.1 / 1,000 live births. If the palsy isn't diagnosed early and treated adequately, child will be threatened by major consequences later. This work provides the reader with the current view on the surgical treatment of patients with brachial plexus birth palsy. It gives a comprehensive overview on early neurosurgical intervention and secondary orthopaedic reconstruction. It submits opinions on indicating criteria. It describes the main charakteristic and results of operations. It refers to the fact that the choice of a surgical procedure can be essential in therapy. The aim of this work is also to characterize the fundamental aspects of postoperative physiotherapy, which simultaneously goes along with the process of intervention therapy. The practical part of this work includes a case of a patient after a neurosurgical performance. It describes the preoperative examinations, the performed surgery and the implementation of postoperative rehabilitation.
Examination of vegetative system in hereditary neuropathy

Aim: The aim of this thesis "Examination of vegetative system in hereditary neuropathy" is to consider the level of physical fitness, physical actvity level and quality of autonomic function in patients with hereditary neuropathy Charcot Marie Tooth and an assessment of interactions of these parameters, especially with regard to possible influence by the presence of autonomic neuropathy and with regard to neurological symptoms. Background: Although vegetative neuropathy was mentioned in the first publication about CMT, it is not widely discussed topic as part of it. Methods: Seventeen probands underwent spiroergometry, heart rate variability test, six minute walk test, rating by the CMT Neuropathy Score and Overall neuropathy disability scale and completing the IPAQ questionnaire. Results: We found symptoms of autonomic neuropathy in group of patients with CMT, especially high-frequency part of heart rate variability spectrum has a particularly significant decrease. Patients with autonomic neuropathy have shown a low fitness and functional capacity, tendency to obesity and hypertension. The observed correlation of autonomic neuropathy and neurological impairment was very weak. The effect of physiotherapy on function of postural mobility in high-performance sportsmen (tennis players)

This thesis, entitled "The effect of physiotherapy on the function of postural motorics in competitive sportsmen (tennis players)", deals with how the activation of the deep stabilizing muscles affects postural functions in adolescent tennis players. A group of 13 competitive tennis players, aged between 10 and 17 years, were used as our probands. These subjects were examined with a bespoke set of tests of functions and then they were individually educated to train with a set of stabilization functions. After the therapy, they were re-examined with this set of tests again and the results of both sets of tests were compared and evaluated. In particular, we evaluated the changes in reactions of individual players, the changes in separate tests and in groups of tests, and the position of players in match rankings. The aim of this thesis was, through the use of the questionnaire study, to gain information on possibilities of utilization of physiotherapeutic procedures and their subjective effects on patients with inherited and diabetic neuropathies. There are also a prosthetic care and surgical solution of leg's deformities, contribution of spa systems in comparison with an ambulatory ( non-resident ) care and the life style's adjustments regarding the disease included in the survey.
